Some callers of pm_runtime_get_sync() and other runtime PM helper
functions, scsi_autopm_get_host() and scsi_autopm_get_device() in
particular, need to distinguish error codes returned when runtime PM
is disabled (i.e. power.disable_depth is nonzero for the given
device) from error codes returned in other situations. For this
reason, make the runtime PM helper functions return -EACCES when
power.disable_depth is nonzero and ensure that this error code
won't be returned by them in any other circumstances. Modify
scsi_autopm_get_host() and scsi_autopm_get_device() to check the
error code returned by pm_runtime_get_sync() and ignore -EACCES.

- Modified the design such that each BFA sub-module will provide
the amount of DMA and KVA memory needed by it and queues the
same request to the global dma and kva info queues.
- During the memory allocation we iterate over this queue to allocate
the dma and kva memory requested by sub-modules.
- The change is needed to avoid requesting the aggregate amount of memory
needed by all the BFA sub-modules as one contiguous chunk.

- If flash controller is halted unconditionally, this results in
illegal write access to flash controller register domain. Since
flash controller registers are only accessible once s_clk is started
- added logic to check for WGN status and halt flash controller only
if it is already running.
- Added check to wait for flash controller halt to be completed before
proceeding with s_clk/l_clk initializations.
- Removed unnecessary reset logic for PMM 1T memory and moved memory
initialization after flash access enable.
- Disable Brocade-1860 asic MBOX interrupt before PLL initialization.
- Remove reset enable for S_CLK/L_CLK after both PLL initializations
are complete.

This patch breaks the SCSI cmd completion into two parts:
1. The bh will allocate and queued work to the cmd specific CPU IO
completion kthread. The CPU for the cmd is from the sc->request->cpu.
2. The CPU specific IO completion kthread will call the scsi_cmd_resp
routine to do the actual cmd completion.
In the normal case, these IO completion kthreads should complete before
the blk IO times out at 60s. However, in the case when these kthreads
are blocked for whatever reason and exceeded the timeout, the call
to conn_destroy will have to iterate and exhaust all related work in the
percpu work list for all online CPUs. This will guarantee the protection
of the work->session and conn pointers before they get freed.
Also modified the event coalescing formula to have at least the
event_coal_min outstanding cmds in the pipeline so the SCSI producer
would not get underrun.
Also changed the following SCSI parameters:
- can_queue from 1024 to 2048
- cmds_per_lun from 24 to 128

iscsi_sw_tcp_conn_restore_callbacks could have set
the sk_user_data field to NULL then iscsi_sw_tcp_data_ready
could read that and try to access the NULL pointer. This
adds some checks for NULL sk_user_data in the sk
callback functions and it uses the sk_callback_lock to
set/get that sk_user_data field.

be2iscsi passes the boot functions its phba object which is
allocated in the shost, but iscsi_ibft passes in a object
allocated for each item to display. The problem is that
iscsi_boot_sysfs was managing the lifetime of the object
passed in and doing a kfree on release. This causes a double
free for be2iscsi which frees the shost in its pci_remove.
This patch fixes the problem by adding a release callback
which the drivers can call kfree or a put() type of function
(needed for be2iscsi which will do a get/put on the shost).

When NPIV port destroy handler is called it does not do all the cleanup
required for the given NPIV port. This was happening as some of the
lport cleanup moved to fcoe_interface_cleanup() routine, which is not
called as part of the vport delete process.
This patch rearranges the sequence in which the fcoe_if_destory() and
fcoe_interface_cleanup() functions are being called from various places
in the code. It now matches the sequence they are constructed during the
create process for both N_Port as well as NPIV port.

Post an FCH_EVT_LIPRESET event on lport reset as
as lport reset occurs on FIP cleat virtual link,
this could be due to change in fcoe vlan and this
event will allow user app fcoemon to switch to
new fcoe vlan.

Problem: Linux based SW target (TCM) connected to windows initiator
was unable to satisfy write request of size > 2K.
Fix: Existing linux implememtation of FCoE stack is expecting sequence
number to match w.r.t incoming framme. When DDP is used on target in
response to write request from initiator, SW stack is notified only
when last data frame arrives and only the pakcket header of last data
frame is posted to NetRx queue of storage. When that last packet was
processed in libfc:Exchange layer, implementation was expecting
sequence number to match, but in this case sequence number which is
embedded in FC Header is assigned by windows initaitor, hence due to
sequence number mismatch post-processing which shall result into
sending RSP is not done. Enhanced the code to utilize the sequence
number of incoming last frame and process the packet so that, it will
eventually complete the write request by sending write response (RSP)
GOOD.
Notes/Dependencies: This patch is validated using windows and linux
initiator to make sure, it doesn't break anything.

Problem: Selection of RX queue on target is based on RX-ID. FCoE used
8 Net Rx queues. HW post the packets based on rx_id %
num_rx_queue. Due to this has based filtering, only one CPU is busy
servicing incoming request including post-processing of incoming
request. This is gating factor because
1. Only one CPU is utilized 100% while others CPUs are not used at all.
2. CPU which received request assign "sequence' by selecting exchange
from per CPU pool (num_ddp_context / num_online_cpus,
approxi.). Due to which if if rate of incoming request is higher
than rate of servicing request, existing code path end of sending
"BUSY" response (SAM_STAT_BUSY because unable to allocate
exchange).
Fix: Fan-out incoming request to all other CPUs excluding the CPU
which is receiving all incoiming request. This path also addresses,
selecting same CPU based on rx_id from received frame for completion
of the request such as "releasing exchange to the per CPU Pool". This
fix is applicable for FCoE target since initiator code path already
takes care of selecting CPU to complete post-processing of request
once OX_ID is assigned.

Problem: Earlier mechanism of selection of CPU was, to select the same CPU
which has received incoming request. Hence in case of rx_id = 0xFFFF,
request was always posted to same NetRx queue, hence only 1 CPU is utilized
for handling the command. It was also causing problem of "running out of
exchanges from per CPU pool of exchanges (in case of DDP offload)
Fix: Implemented new algo. to select CPU for post-processing of
incoming commands when rx_id is unknown. This is simple Round robin
algo. for CPU selection.

Problem: When initiator sends write command to target, target tries to
assign new sequence. It allocates new exchangeID (RX_ID)
always from non-offloaded pool (Non-offload EMA)
Fix: Enhanced fcoe_oem_match routine to look at F_CTL flags and if it
is exchange responder and command type is WRITEDATA, then function
returns TRUE instead of FALSE. This function is used to determine
which pool to use (offload pool of exchange is used only if this
function returns TRUE).

Problem: Existing RPORT state machine continues witg FLOGI/PLOGI
process only after it receices beacon from other end. Once claiming
stage is over (either clain notify or clain repose), beacon is sent
and state machine enters into operational mode where it initiates the
rlogin process (FLOGI/PLOGI) to the peer but before this rlogin is
initiated, exitsing implementation checks if it received beacon from
other end, it beacon is not received yet, rlogin process is not
initiated. Other end initiates FLOGI but peer end keeps on rejecting
FLOGI, hence after 3 retries other end deletes associated rport, then
sends a beacon. Once the beacon is received, peer end now initiates
rlogin to the peer end but since associated rport is deleted FLOGI is
neither accepted nor the reject response send out because rport is
deleted. Hence unable to proceed withg FLOGI/PLOGI process and fails
to establish VN2VN connection.
Fix: VN2VN spec is not standard yet but based on exitsing collateral
on T11, it appears that, both end shall send beacon and enter into
'operational mode' without explictly waiting for beacon from other
end. Fix is to allow the RPORT login process as long as respective
RPORT is created (as part of claim notification / claim response) even
though state of RPORT is INIT. Means don't wait for beacon from peer
end, if peer end initiates FLOGI (means peer end exist and
responding).
Notes: This patch is preparing the FCoE stack for target wrt
offload. This is generic patch and harmless even if applied on storage
initiator because 'else if' condition of function 'fcoe_oem_found'
shall evaluate to TRUE only for targets.

struct scsi_lun is also just a struct with an array of 8 octets (64 bits)
but using it instead in iscsi structs lets us call scsilun_to_int
without a cast, and also lets us copy it using assignment, instead of
memcpy().

Properly handling of target reset in multi-initiator environment
Clean up in broadcast change handling:
(1) Need to look at the status of each task management request, and retry
the TM when there are failures.
(2) Need quiescence IO so the driver doesn't take on more IO request while
it's in the middle of sending TM request to firmware
(3) Add support to keep track of how many pending broadcast AEN events
are received while the broadcast handling is active, then loop back at
the end of this routine if there were any events received.
Clean up in mpt2sas_scsih_issue_tm routine:
(1) Make sure proper status is returned when host reset fails
(2) Clean up sanity checks near end of routine, insuring all outstanding
IOs were completed.

This feature is to override the default
max_sectors setting at load time, taking max_sectors as an
command line option when loading the driver. The setting is
currently hard-coded in the driver to 8192 sectors (4MB transfers).
If max_sectors is specified at load time, minimum specified
setting will be 64, and the maximum is 8192. The driver will
modify the setting to be on even boundary. If max_sectors is not
specified, the driver will default to 8192.

mpt2sas driver revision q header update:
(1) Modified the descriptions of the LocalAddress bit in the
Flags field of the MPI SGE Format description and the MPI
Simple Element.
(2) Modified Data Location Address Space bits in the Flags field
of the IEEE Chain Element.
(3) Added more detail to the description of the DataLength field
for the SCSI IO Request and Target Assist Request. Removed
restriction on using chained SGLs when using multicast or
bidirectional support.

Driver was incorrectly using the SLAB_CACHE_DMA flag when creating a cache
for SGLs. fnic device does not have 24-bit DMA restrictions. Remove the flag
and allocations from ZONE_DMA.

based FW READY method.

register to see if there is a state change in FW, and sends the READY event.

Noticed that when the sysfs interface of the SCSI SES
driver was used to request a fault indication the LED
flashed but the buzzer didn't sound. So it was doing
what REQUEST IDENT (locate) should do.
Changelog:
- fix the setting of REQUEST FAULT for the device slot
and array device slot elements in the enclosure control
diagnostic page
- note the potentially defective code that reads the
FAULT SENSED and FAULT REQUESTED bits from the enclosure
status diagnostic page

The ei->SenseLen field is filled in by the Smart Array. For requests to
logical drives, it will not exceed 32 bytes, so should be ok, but for physical
requests it depends on the target device, not the Smart Array. It's conceivable
that this could exceed the 32 byte size of ei->SenseInfo. In that case, the memcpy
would read past the end of ei->SenseInfo, copying data from the next command,
as if it were sense data, or, if it happened to be the very last command in the
block of allocated commands, could fall off the end of the allocated area and
crash. I'm not aware of anyone ever encountering this behavior, but it could
conceivably happen. This bug was found by Coverity.

If a Virtual I/O server fails in a dual virtual I/O server multipath
configuration, ensure we delete all remote ports so that path failover
can occur. For a single path configuration, the remote ports will
go into devloss state.

Deleting NPIV port causes a kernel panic when the NPIV port is in the same zone
as the physical port and shares the same LUN. This happens due to the fact that
vport destroy and unsolicited ELS are scheduled to run on the same workqueue,
and vport destroy destroys the lport and the unsolicited ELS tries to access
the invalid lport. This patch fixes this issue by maintaining a list of valid
lports and verifying if the lport is valid or not before accessing it.

New FW/HSI (7.0):
- Added support to 578xx chips
- Improved HSI - much less driver's direct access to the FW internal
memory needed.
New implementation of the HSI handling layer in the bnx2x (bnx2x_sp.c):
- Introduced chip dependent objects that have chip independent interfaces
for configuration of MACs, multicast addresses, Rx mode, indirection table,
fast path queues and function initialization/cleanup.
- Objects functionality is based on the private function pointers, which
allows not only a per-chip but also PF/VF differentiation while still
preserving the same interface towards the driver.
- Objects interface is not influenced by the HSI changes which do not require
providing new parameters keeping the code outside the bnx2x_sp.c invariant
with regard to such HSI chnages.

In certain circumstances, we can get an oops from a torn down device.
Most notably this is from CD roms trying to call scsi_ioctl. The root
cause of the problem is the fact that after scsi_remove_device() has
been called, the queue is fully torn down. This is actually wrong
since the queue can be used until the sdev release function is called.
Therefore, we add an extra reference to the queue which is released in
sdev->release, so the queue always exists.

- Remove the memset in the lpfc_sli4_remove_rpi_hdrs call.
- Correct swapping of SGE word 2 relative to offset value
- Reorganize CQ and EQ usage to comply with SLI4 Specification.
- Expand the driver to check the rn bit. Only detect an error if the error bit
is set and the RN bit is NOT set.
- If mailbox completion code is not success AND the mailbox status is success,
then and only then will the driver overwrite the mailbox status.
- When driver initializing device, if the device is on a PCIe bus, set
PCI's "needs fundamental reset" bit so that EEH uses fundamental reset
instead of hot reset for recovery.
- Prevent driver from using new WWN when changed in firmware (until driver
reload)
- When HBA reports maximum SGE size > 0xffffffff (infinite), override
with 0x80000000.
- Fixed potential missed SLI4 device initialization failure conditions.

Commit 56dd2c06 "libsas: Don't issue commands to devices that have been
hot-removed" edited Darrick's original patch to remove setting 'gone' in
the sas_deform_port() path because that prevented scsi sync cache
commands from being issued when the driver was unloaded. However, this
allows true device gone notifications (as signaled port phy events) to
trigger sync cache commands to devices that are known to be unreachable.
Teach libsas which sas_deform_port() invocations are likely device gone
events.

The presense of a writeq() implementation on 32-bit x86 that splits the
64-bit write into two 32-bit writes turns out to break the mpt2sas driver
(and in general is risky for drivers as was discussed in
<http://lkml.kernel.org/r/adaab6c1h7c.fsf@cisco.com>). To fix this,
revert 2c5643b1c5 ("x86: provide readq()/writeq() on 32-bit too") and
follow-on cleanups.
This unfortunately leads to pushing non-atomic definitions of readq() and
write() to various x86-only drivers that in the meantime started using the
definitions in the x86 version of <asm/io.h>. However as discussed
exhaustively, this is actually the right thing to do, because the right
way to split a 64-bit transaction is hardware dependent and therefore
belongs in the hardware driver (eg mpt2sas needs a spinlock to make sure
no other accesses occur in between the two halves of the access).
Build tested on 32- and 64-bit x86 allmodconfig.

In error recovery, most scsi error recovery stages will send a TUR command
for every bad command when a driver's error handler reports success. When
several bad commands to the same device, this results in a device
being probed multiple times.
This becomes very problematic if the device or connection is in a state
where the device still doesn't respond to commands even after a recovery
function returns success. The error handler must wait for the test
commands to time out. The time waiting for the redundant commands can
drastically lengthen error recovery.
This patch alters the scsi mid-layer's error routines to send test commands
once per device instead of once per bad command. This can drastically
lower error recovery time.

Some kernel transport drivers unconditionally disable
retrieval of the Caching mode page. One such for example is
the BBB/CBI transport over USB. Such a restraint is too
harsh as some devices do support the Caching mode
page. Unconditionally enabling the retrieval of this mode
page over those transports at their transport code level may
result in some devices failing and becoming unusable.
This patch implements a method of retrieving the Caching
mode page without unconditionally enabling it in the
transports which unconditionally disable it. The idea is to
ask for all supported pages, page code 0x3F, and then search
for the Caching mode page in the mode parameter data
returned. The sd driver already asks for all the mode pages
supported by the attached device by setting the page code to
0x3F in order to find out if the media is write protected by
reading the WP bit in the Device Specific Parameter
field. It then attempts to retrieve only the Caching mode
page by setting the page code to 8 and actually attempting
to retrieve it if and only if the transport allows it.
The method implemented here is that if the transport doesn't
allow retrieval of the Caching mode page and the device is
not RBC, then we ask for all pages supported by setting the
page code to 0x3F (similarly to how the WP bit is retrieved
above), and then we search for the Caching mode page in the
mode parameter data returned.
With this patch, devices over SATA, report this (no change):

Modified the event coalescing code for iSCSI offload to combat both
corner cases and optimize performance as follows:
1. Added mechanism to loop back a second time to process any leftover
CQEs that was generated by the hardware during the time the driver is
busy processing previous CQEs in the bh. This not only helps the
performance but also fixes the corner case when no more CQEs are being
generated in the pipeline; so those leftover CQEs will get a a chance
to be processed.
2. Added ARM_CQE_FP to distinguish between fast path arming versus
slow path arming. This change will guarantee that the CQEs will
always get a chance to be re-armed during fast path completions.
3. Removed the inline event coalescing division for perf optimization.
Also fixed a division-by-zero error when the event_coal_div module
param was set to 0.
4. Changed the default SQ WQEs size from 256 to 128 to match chip
default.
5. Changed the cmd_per_lun from 32 to 24.

The number of chip's internal command cell, which is use to generate
SCSI cmd packets to the target, was not initialized correctly by
the driver when the sq_size is changed from the default 128.
This, in turn, will create a problem where the chip's transmit pipe
will erroneously reuse an old command cell that is no longer valid.
The fix is to correctly initialize the chip's command cell upon setup.

Since queue_work does not requeue, there is no need to check
if a work is in progress using the AF_DPC_SCHEDULED flag.
queue_work would return if work is pending without adding the
work, do_dpc would again get invoked from qla4xxx_timer if
there is still DPC flags set.

Complete the cmd if sense length is zero. For cases where sense
data spans across multiple iocb's by FW, we need to hold on to the
I/O (ha->status_srb != NULL) till we have processed them all and
copied the sense data from internal buffer to scsi_cmd sense buffer.

Root cause: When kernel crashes, bfa IOC state machine and FW doesn't get
a notification and hence are not cleanly shutdown. So registers holding
driver/IOC state information are not reset back to valid disabled/parking
values. This causes subsequent driver initialization to hang during kdump
kernel boot.
Fix description: during the initialization of first PCI function, reset
corresponding register when unclean shutown is detect by reading chip
registers. This will make sure that ioc/fw gets clean re-initialization.

If the driver is getting flooded with interrupts, there's a possibility
that the interrupt service routine could falsely detect a stuck interrupt
condition and reset the adapter.
This patch changes the logic such that the routine will loop back into
the command processing code one more time after detecting the stuck
interrupt signature. If there are no commands to process after that pass,
and the interrupt is still not cleared, then the driver will print the
"Error clearing HRRQ" message and reset the adapter.

libfcoe's interface consists of create, destroy, enable,
disable and create_vn2vn. These are currently module
paramaters added durring the module initialization. A
concern arose that the module parameters were being added
with write permissions before the module had completed
initialization. The following code was added to each

This check was called out as unhelpful as the module can
go dead at any time and therefore its state isn't a reliable
thing to look at as a sign of stability and initialization
completion. Also, that functional interfaces like these
should be added after module initialization.
This patch removes the unnecessary checks and hopes to
disprove the concern about initialization ordering.
Recent fcoe transport rework changes now require fcoe
transports to register with libfcoe before any operation
can take place. libfcoe may access some static variables
but nothing that could cause a problem. Once a fcoe transport
is registered, libfcoe is usable and any interface calls will
be functional.

Currently, when seq_send() fails in fc_fcp_send_data(),
fc_fcp_retry_cmd() would complete this failed I/O directly and let
scsi-ml retry. However, target side is not notified which may hang the
target. Instead, we should just bail out from from fc_fcp_send_data
and let scsi-ml times it out and aborts this I/O instead.

In this case fsp was freed before error handler was invoked,
this is fixed by having SRR fsp reference freed by exch
destructor so that fsp will be always held until it exch
is freed.
Also don't reset fsp->recov_seq since this is needed by
SRR error handler to do exch done.

In cases exch is already timed out then exch layer could
end up calling resp handler again for its response frame
received after timeout, though in this case fc_exch_timeout
handler would have already called resp with FC_EX_TIMEOUT.
This would cause REC response handler to release its
fsp pkt hold twice instead once and possibly similar issues
with other ELS exchanges in this race.
To avoid this race have resp updated under exch lock
in rx path, the resp would get set to NULL in case
of FC_EX_TIMEOUT under the same lock to prevent resp
callback after FC_EX_TIMEOUT.

Host doesnt handle CVL to NPIV instantiated ports correctly.
- As per FC-BB-5 Rev 2 CVLs with no VN_Port descriptors shall be treated as
implicit logout of ALL vn_ports.
- CVL for NPIV ports should be handled before physical port even if descriptor
for physical port appears before NPIV ports

Driver was a sending a SEP request during interrupt context which
required to go to sleep.
The fix is to rearrange the code so a fake event
MPT2SAS_TURN_ON_FAULT_LED is fired from interrupt context, then later
during the kernel worker threads processing, the SEP request is issued
to firmware.

Since for-2.6.40/core was forked off the 2.6.39 devel tree, we've
had churn in the core area that makes it difficult to handle
patches for eg cfq or blk-throttle. Instead of requiring that they
be based in older versions with bugs that have been fixed later
in the rc cycle, merge in 2.6.39 final.
Also fixes up conflicts in the below files.

device" added an of_match pointer to struct device to cache the
of_match_table entry discovered at driver match time. This was unsafe
because matching is not an atomic operation with probing a driver. If
two or more drivers are attempted to be matched to a driver at the
same time, then the cached matching entry pointer could get
overwritten.
This patch reverts the of_match cache pointer and reworks all users to
call of_match_device() directly instead.

Commit c21e6beb removed our queue request_fn re-enter
protection, and defaulted to always running the queues from
kblockd to be safe. This was a known potential slow down,
but should be safe.
Unfortunately this is causing big performance regressions for
some, so we need to improve this logic. Looking into the details
of the re-enter, the real issue is on requeue of requests.
Requeue of requests upon seeing a BUSY condition from the device
ends up re-running the queue, causing traces like this:
scsi_request_fn()
scsi_dispatch_cmd()
scsi_queue_insert()
__scsi_queue_insert()
scsi_run_queue()
scsi_request_fn()
...
potentially causing the issue we want to avoid. So special
case the requeue re-run of the queue, but improve it to offload
the entire run of local queue and starved queue from a single
workqueue callback. This is a lot better than potentially
kicking off a workqueue run for each device seen.
This also fixes the issue of the local device going into recursion,
since the above mentioned commit never moved that queue run out
of line.

Eliminate duplicate code by refactoring the calls to qla2xxx_read_sfp()
in qla2x00_get_thermal_temp(). This keeps the parameter values separate
from the mailbox register mechanics. This also allows qla2xxx_read_sfp()
to be the sole "spec" for READ SFP semantics.

When reading a single byte using the READ SFP mailbox command, the
single byte of data is returned in MB[1] and not MB[8].
The reason that MB[8] was being used is that the spec was unclear
as it evolved over time; and we have not needed to read a single
byte until recently.

The driver keeps a copy of the fw_version within the ha structure.
For ISP82xx, this local copy doesn't get updated, and as a result,
the old firmware version ends up getting displayed. This patch fixes
this issue.

Bumping ref count during fc_vport_terminate() was the cause. vport
delete would wait for ref count to drop to zero and that would never

This patch ensures qla82xx_watchdog is not being run for the vport. It also

on driver load, if reset_devices is set, and the hard reset
attempts fail, try to bring up the controller to the point that
a command can be sent, and send it a soft reset command, then
after the reset undo whatever driver initialization was done to get
it to the point to take a command, and re-do it after the reset.
This is to get kdump to work on all the "non-resettable" controllers
(except 64xx controllers which can't be reset due to the potentially
shared cache module.)

The bit-2-doorbell reset method seemed to cause (survivable) NMIs
on some systems and (unsurvivable) IOCK NMIs on some G7 servers.
Firmware guys implemented a new doorbell method to alleviate these
problems triggered by bit 5 of the doorbell register. We want to
use it if it's available.

Detect failure of controller reset by noticing if the 32 bytes of
"driver version" we store on the hardware in the config table
fail to get zeroed out. Previously we noticed if the controller
did not transition to "simple mode", but this did not detect reset
failure if the controller was already in simple mode prior to
the reset attempt (e.g. due to module parameter hpsa_simple_mode=1).

is causing a potential NULL deref in scsi_run_queue() because the
q->queuedata may already be NULL by the time this function is called.
Since we shouldn't be running a queue that is being torn down, simply

The code currently computes an offset into a dma_alloc_coherent() area
on the assumption that the alignment is imprecise. In fact, the API
guarantees PAGE_SIZE alignment, so the offset calculation is always
zero: remove it.

I removed outer loops in ...wait_msgint_ready the sleeptime and
retrycount are in fact never changed so I changed them into
defines. In arcmsr_flush_hba_cache is a loop removed, which printed
the same printk 100 times, one line in log is enough I think. The
arcmsr_sleep_for_bus_reset has lost a functionality with the latest
patches, The only thing the function does is a long sleep, so it's
replaced with a ssleep.

On systems with very large numbers (> 1600 or so) of SCSI devices,
cat /proc/scsi/scsi ends up failing with -ENOMEM. This is due to
the show routine simply iterating over all of the devices with
bus_for_each_dev(), and trying to dump all of them into the buffer
at the same time. On my test system (using scsi_debug with 4064 devices),
the output ends up being ~ 632k, far more than kmalloc will typically allow.
This patch defines its own seq_file opreations to iterate over the scsi
devices.The result is that each show() operation only dumps ~ 180 bytes
into the buffer at a time so we don't run out of memory.
If the "Attached devices" header isn't required, we can dump the
sfile->private bit completely.

This patch fixes an issue seen where an event occurs
which causes the ibmvscsi driver to reset its CRQ. Upon
re-registering its CRQ, it receives H_CLOSED, indicating
the Virtual I/O Server is not yet ready to receive commands.
This resulted in the ibmvscsi driver essentially offlining
the adapter and not recovering. The fix is to re-enable
our interrupt so that when the Virtual I/O server is ready
and sends a CRQ init, we will be able to receive it and
resume initialization of the VSCSI adapter.

Currently the size of the dump generated by the driver is limited
in 4MB, which is insufficient to gather much useful data from the
new 64 bit adapters.
This patch makes the needed changes to increase the dump limit
for the 64 bit adapters to 32MB, or even to a bigger value in the
future, but keeping the current limitations for the legacy 32 bit
adapters.

The SCSI ALUA handler currently fails to attach to devices
reporting an UNAVAILABLE/OFFLINE AAS. But given that an
UNAVAILABLE/OFFLINE AAS can transition to other states
like ACTIVE/OPTIMIZED, ACTIVE/NON-OPTIMIZED, etc. as per
SPC4, this ALUA handler behavior should be rectified so
as to attach to devices which also report an
UNAVAILABLE/OFFLINE AAS.

If the session is not ready yet, we ask the SCSI-ml to retry. However, if the
session is just uploaded, we should not retry, but instead call scsi_done to

fc_remove_host() waits for flushing the workqueue, but it is stuck at flushing
the first work. The first work doesnt complete, because it is waiting for async
layer to complete the IOs. The async layer cannot complete the IO as the
terminate_rport_io for the second work was not called, which will be called
only when the first work completes. Hence the deadlock. To resolve this
deadlock, the workqueue allocation has been modified from
create_singlethread_workqueue() to alloc_workqueue().
In addition, fc_terminate_rport_io() should be called before the
scsi_flush_work() to avoid the similar deadlock as above.

This patch includes fixes for two issues releated to firmware download
implementation: 1) Merged memory leak fix provided by Jesper Juhl
<jj@chaosbits.net>. Basically we need to call release_firmware() after
request_firmware(). 2) fixed issues with the firmware download interface

During performance testing on P7 machines it was observed that the interrupt
service routine was doing unnecessary MMIO operations.
This patch rearranges the logic of the routine and moves some of the code out
of the main routine. The result is that there are now fewer MMIO operations in
the performance path of the code.
As a result of the above change, an existing condition was exposed where the
driver could get an "unexpected" hrrq interrupt. The original code would flag
the interrupt as unexpected and then reset the adapter. After further analysis
it was confirmed that this condition can occasionally occur and that the
interrupt can safely be ignored. Additional code in this patch detects this
condition, clears the interrupt and allows the driver to continue without
resetting the adapter.

This patch has Support for the new solid state device product SSS6200
from LSI and relavent features w.r.t SSS6200.
The major feature added in this driver is supporting Direct-I/O to the
SSS6200 storage.There are some additional changes done to avoid exposing
the RAID member disks to the OS and hiding/exposing drives based on the
OEM Specific Flag in Manufacturing Page10 (this is required to handle
specific changes in the SSS6200 firmware).
Each and every changes are listed below.
1. Hiding IR related messages.
For SSS6200, the driver is modified not to print IR related events.
Even if the debugging is enabled the IR related messages will not be displayed.
In some places if there is a need to display a message related to IR the
string "IR" is replaced with string "DD" and the string "volume" is replaced
with "direct drive". But the function names are not changed hence there are
some places where the reference to volume can be seen if debug level is set.
2. Removed RAID transport support
In Linux the user can retrieve RAID volume information from the sysfs directory.
This support is removed for SSS6200.
3. Direct I/O support.
The driver tries to enable direct I/O when a volume is reported to the driver
by the firmware through IRCC events and the driver does this just before
reporting to the OS, hence all the OS issued I/O can go through direct path
if they can, The first validation is to see whether the manufacturing page10
flag is set to expose all drives always. If that is set, the driver will not
enable direct I/O and displays the message "DDIO" is disabled globally as
drives are exposed. The driver checks whether there is more than one volume
in the controller, if so the direct I/O will be disabled globally for all
volumes in the controller and the message displayed will be "DDIO is disabled
globally as number of drives > 1.
If retrieving number of PD is failed the driver will not enable direct I/O
and displays the message Failure in computing number of drives DDIO disabled.
If memory allocation for RAIDVolumePage0 is failed, the driver will not enable
direct I/O and displays the message Memory allocation failure for
RVPG0 DDIO disabled. If retrieving RAIDVolumePage0 is failed the driver will
not enable direct I/O and displays the message Failure in retrieving
RVPG0 DDIO disabled
If the number of PD in a volume is greater than 8, then the direct I/O will
be disabled.
If any of individual drives handle retrieval is failed then the DD-IO will
be disabled.
If the volume is not RAID0 or if the block size is not 512 then the DD-IO will
be disabled.
If the volume size is greater than 2TB then the DD-IO will be disabled.
If the driver is not able to find a valid stripe exponent using the configured
stripe size then the DD-IO will be disabled
When the DD-IO is enabled the driver will check every I/O request issued to
the storage and checks whether the request is either
READ6/WRITE6/READ10/WRITE10, if it is and if the complete I/O transfer
is within a stripe size then the I/O is redirected to
the drive directly instead of the volume.
On completion of every I/O, if the completion is failure means if the reply
is address reply with a reply frame associated with it, then the type of I/O
will be checked, if the I/O is direct then the I/O will be retried to
the volume once.

Currently fcoe interface cleanup is done after ndo_fcoe_disable
and that prevents logoff going out to the peer, so this patch
moves all netdev cleanup and its releasing inside
fcoe_interface_cleanup to have log off before ndo_fcoe_disable
disables the fcoe.
This patch also fixes asymmetric rtnl locking around fcoe_if_destroy,
as currently this function requires rtnl held by its caller
and then have this func drops the lock, instead now don't have
any processing under rtnl inside fcoe_if_destroy, this required
moving few func to get build working again.

Added REC_TOV_CONST intent was to have rec tov as e_d_tov + 1s
but currently it is e_d_tov + 1ms since e_d_tov is stored in ms
unit.
Also returned rec tov by get_fsp_rec_tov is in ms and this ms tov
is used as-is with fc_fcp_timer_set expecting jiffies tov.
Fixed this by having get_fsp_rec_tov return rec tov in jiffies
as e_d_tov + 1s and then use jiffies tov w/ fc_fcp_timer_set.
Also some cleanup, no need to cache get_fsp_rec_tov return value
in local rec_tov at various places.

When rmmoving the underlying fcoe transport driver module by force when
it's attached and in use, the correspoding netdev mapping should be
cleaned up properly as well, otherwise the lookup for a given netdev
for the transport would still return non NULL pointer, causing "unable
to handle paging request" bug.

The host_lock is still used to protect the can_queue
value in the Scsi_Host, but it doesn't need to be held
and released by each caller. This patch moves the lock

The check of module state being MODULE_STATE_LIVE is no longer needed for the
individual fcoe transport driver, e.g., fcoe.ko, as sysfs entries now go to
libfcoe now, if it reaches fcoe.ko, it has to be already registered. The module
state check for libfcoe will guard the possible race condition of sysfs being
writable before module_init function is called and after module_exit.

These checks were initially added to avoid a lockdep
false positive when dealing with the s_active, rtnl
and fcoe_config_mutex mutexes. Recently the create,
destroy, enable and disable sysfs entries were moved
from fcoe.ko to libfcoe.ko. With this change the mutex
usage was shuffled around and the lockdep false
positive stopped happening. We can now remove these
checks.

This code was incorrectly ported from fcoe.c when the
fcoe transport infrastructure was put into place. It
was originally needed in fcoe.c when dealing with
the rtnl mutex. In that code it was only needed to
avoid a lockdep false positive. In libfcoe we don't
deal with the rtnl mutex, we don't get the lockdep
false positive and therefore we don't need these
checks.

In testing it was noticed that Extended Delay after Reset flag was being set
for gscsi and volume set devices. This had a negative effect on performance
for volume sets. The fix is to only set the flag for gscsi devices.

Before driver's own internal state is marked as PLOGI/PRLI
complete. This additional check closes a window seen with
dual-personality initiator/target devices where a driver's
PLOGI/PRLI request occurs within the window after the target's
PLOGI request has completed, but prior to the target's PRLI
arriving and processed by the firmware. Without this additional
check, the firmware will return port-information stating that the
port neither supports target nor initiator functions, causing the
driver to register the rport prematurely to the FC-transport
without the proper 'roles' being set.

The upper 16-bits of the handle for all I/O in multi-queue supported
drivers carries the ID of the request queue it was submitted on. When
using Abort I/O IOCB, the driver needs to also populate the upper
16-bits in the handle_to_abort field so the fw can correlate with the
actual I/O.

This makes sure that when a driver calls the ethtool's
get/set_settings() callback of another driver, the data passed to it
is clean. This guarantees that speed_hi will be zeroed correctly if
the called callback doesn't explicitely set it: we are sure we don't
get a corrupted speed from the underlying driver. We also take care of
setting the cmd field appropriately (ETHTOOL_GSET/SSET).
This applies to dev_ethtool_get_settings(), which now makes sure it
sets up that ethtool command parameter correctly before passing it to
drivers. This also means that whoever calls dev_ethtool_get_settings()
does not have to clean the ethtool command parameter. This function
also becomes an exported symbol instead of an inline.
All drivers visible to make allyesconfig under x86_64 have been
updated.

There's a code path in pmcraid that can be reached via device ioctl that
causes all sorts of ugliness, including heap corruption or triggering
the OOM killer due to consecutive allocation of large numbers of pages.
Not especially relevant from a security perspective, since users must
have CAP_SYS_ADMIN to open the character device.
First, the user can call pmcraid_chr_ioctl() with a type
PMCRAID_PASSTHROUGH_IOCTL. A pmcraid_passthrough_ioctl_buffer
is copied in, and the request_size variable is set to
buffer->ioarcb.data_transfer_length, which is an arbitrary 32-bit signed
value provided by the user.
If a negative value is provided here, bad things can happen. For
example, pmcraid_build_passthrough_ioadls() is called with this
request_size, which immediately calls pmcraid_alloc_sglist() with a
negative size. The resulting math on allocating a scatter list can
result in an overflow in the kzalloc() call (if num_elem is 0, the
sglist will be smaller than expected), or if num_elem is unexpectedly
large the subsequent loop will call alloc_pages() repeatedly, a high
number of pages will be allocated and the OOM killer might be invoked.
Prevent this value from being negative in pmcraid_ioctl_passthrough().

SCSI uses request_queue->queuedata == NULL as a signal that the queue
is dying. We set this state in the sdev release function. However,
this allows a small window where we release the last reference but
haven't quite got to this stage yet and so something will try to take
a reference in scsi_request_fn and oops. It's very rare, but we had a
report here, so we're pushing this as a bug fix
The actual fix is to set request_queue->queuedata to NULL in
scsi_remove_device() before we drop the reference. This causes
correct automatic rejects from scsi_request_fn as people who hold
additional references try to submit work and prevents anything from
getting a new reference to the sdev that way.

At two points in handling device ioctls via /dev/mpt2ctl, user-supplied
length values are used to copy data from userspace into heap buffers
without bounds checking, allowing controllable heap corruption and
subsequently privilege escalation.
Additionally, user-supplied values are used to determine the size of a
copy_to_user() as well as the offset into the buffer to be read, with no
bounds checking, allowing users to read arbitrary kernel memory.

primarily to avoid issuing polling commands while burning is in
progress. This behavior doesn't fit other types of devices with
removeable media where polling commands don't have adverse side
effects and door locking usually doesn't exist.
This patch introduces new genhd flag which controls the auto-blocking
behavior and uses it to enable auto-blocking only on optical devices.
Note for stable: 2.6.38 and later only

We are currently using this flag to check whether it's safe
to call into ->request_fn(). If it is set, we punt to kblockd.
But we get a lot of false positives and excessive punts to
kblockd, which hurts performance.
The only real abuser of this infrastructure is SCSI. So export
the async queue run and convert SCSI over to use that. There's
room for improvement in that SCSI need not always use the async
call, but this fixes our performance issue and they can fix that
up in due time.

Instead of overloading __blk_run_queue to force an offload to kblockd
add a new blk_run_queue_async helper to do it explicitly. I've kept
the blk_queue_stopped check for now, but I suspect it's not needed
as the check we do when the workqueue items runs should be enough.

Stop including <linux/delay.h> in x86 header files which don't
need it. This will let the compiler complain when this header is
not included by source files when it should, so that
contributors can fix the problem before building on other
architectures starts to fail.

Fix up conflicts in fs/{aio.c,super.c}
Disable Interrupt MBX completion will disable the interrupt on
successful completion. Fixed the bug where driver was waiting for
Interrupt to come in for its completion. Now driver will poll for
disable interrupt MBX completion.

Driver does not detect a new CQE (completion queue entry) if a thread receives
the wakup when it is in TASK_RUNNING state. Fix is to set the state to
TASK_INTERRUPTIBLE while holding the fp_work_lock.
Also, Use __set_current_task() since it is now set inside a spinlock with
synchronization.
Two other related optimizations:
1. After we exit the while (!kthread_should_stop()) loop, use
__set_current_state() since synchronization is no longer needed.
2. Remove set_current_state(TASK_RUNNING) after schedule() since it
should always be TASK_RUNNING after schedule().

remove ha flag AF_HBA_GOING_AWAY and added flag AF_HA_REMOVAL
to mark the other ISP-4xxx port to indicate that the driver is

Driver has capability to add device dynamically and present
them to OS, driver no longer need to wait for DDBs to come
online during driver initialization.
Driver still issues a relogin for DDBs that are not online,
but no longer wait for DDB to come online.

FW is not able to process mbox command if FW state is failed. This will cause
mbox command to timeout and adapter reset. We have separate function to detect
FW failed state and do adapter reset. So to avoid mbox command timeout, do not
process mbox command in case of FW state failed.

There have been many complaints that an enclosure with subenclosures
isn't attached to by the ses driver. Until now, though, no-one had
been willing to provide access to one.
Subenclosures are added simply by flattening the tree (i.e. all
subenclosure devices show up under the one main device). This may have
consequences if the naming is only unique per subenclosure, but that's a
bug for another day. The tested array had no page 7, so no device
naming at all. It also only had the disk devices on one of its
subenclosures (all the others had power, fans, temperature and various
sensors), so testing of this is fairly rudimentary.

bnx2x now uses seperate MTUs for networking and FCoE. FCoE MTU is fixed to 2500
and bnx2fc now needs to match this logic by using FCOE_MTU instead of
netdev->mtu.

bnx2fc only operates on bnx2x hardware devices and not master bonding
devices, so there is no need to check for bonding. Even if the bnx2x
device is "enslaved" into a bonding device, FCoE is unaffected as it
has its own MAC address and queues.

Cleanup commands are issued to the firmware to cleanup any stuck ios
that are supposed to be implicitly aborted. In the worst case we can
have all scsi ios filling up the free_list and we may not be able to
allocate cleanup tasks. So the driver has to reserve free_list entries
to be able to allocate the cleanup tasks. This reserve free_list common
to all cpus is allocated as one additional entry in the per cpu
free_lists.
In bnx2fc_cmd_alloc(), there is a related fix to use get_cpu() for the
free_list_index. This will prevent using the wrong index if the CPU
is preempted.

With kernel debugging enabled, holding cq_lock when calling
bnx2fc_free_session_resc() which calls iounmap() leads to a warning
stack trace [INFO: HARDIRQ-safe -> HARDIRQ-unsafe lock order detected].
iounmap() grabs a HARDIRQ-unsafe vmlist lock, so holding
spin_lock_bh(cq_lock) when calling iounmap() will trigger the LOCKDEP
warning. Since cq_lock is required only to guard against deletion, hold
the lock just before freeing the cq.

Periodically poll adapter registers to detect pci adapter failure
(reads return -1). On failure, take port offline, set error indicators
and wake up worker threads. Threads will take adapter offline.

enclosure page 7 gives us the "pretty" names of the enclosure slots.
Without a page 7, we can still use the enclosure code as long as we
make up numeric names for the slots. Unfortunately, the current code
fails to add any devices because the check for page 10 is in the wrong
place if we have no page 7. Fix it so that devices show up even if
the enclosure has no page 7.

This patch reduces the number of sequential pointer derefs in
drivers/scsi/scsi_error.c
This has been submitted a number of times over a couple of years. I
believe this version adresses all comments it has gathered over time.
Please apply or reject with a reason.
The benefits are:
- makes the code easier to read. Lots of sequential derefs of the same
pointers is not easy on the eye.
- theoretically at least, just dereferencing the pointers once can
allow the compiler to generally slightly faster code, so in theory
this could also be a micro speed optimization.
- reduces size of object file (tiny effect: on x86-64, in at least one
configuration, the text size decreased from 9439 bytes to 9400)
- removes some pointless (mostly trailing) whitespace.

A useful test case for error recovery is multiple,
consecutive medium errors. When scsi_debug is started
with "opts=2" a MEDIUM ERROR is generated when block
0x1234 (4660) is read. The patch extends that to
10 consecutive blocks from 0x1234 (i.e. blocks 4660 to
4669 inclusive).

BTW Change /dev/sg1 (bsg device works just as well) to
/dev/sdb to see why, with faulty media, you do not want
to use the block layer interface. Reason: time block
layer takes to do useless retries and collateral damage
to data in its 4 KB blocks (O_DIRECT mitigates the
latter).

I think this stems from a misunderstanding of how the ata error handler
works. ata_scsi_cmd_error_handler() gets called with a passed in list
of commands to handle. However, that list may still not be empty when
it exits. The command ata_scsi_port_error_handler() must be called
(which takes no list) before the list will be completely emptied. This
bites the sas error handler because the two are called from different
functions and the original list has gone out of scope before
ata_scsi_port_error_handler() is called. leading to some commands
dangling on bare stack, which is a potential memory corruption issue.
Fix this by manually deleting all outstanding commands from the on-stack
list before it goes out of scope.

This attribute, requested by Redhat, allows kexec-tools to know
whether the controller can honor the reset_devices kernel parameter
and actually reset the controller. For kdump to work properly it
is necessary that the reset_devices parameter be honored. This
attribute enables kexec-tools to warn the user if they attempt to
designate a non-resettable controller as the dump device.

Update scsi_debug to support the Logical Block Provisioning commands and
bits as defined in SBC3r26. The old tp* parameters have been
transitioned to the new lbp* scheme found in the draft standard.
The old tpu option to enable UNMAP is now called lbpu. tpws to signal
support for WRITE SAME(16) with the UNMAP bit set is now lbpws. Support
for WRITE SAME(10) with the UNMAP bit set is also available using the
lpuws10 parameter.
Limiting the maximum number of blocks per WRITE SAME command has been
implemented and is available via the write_same_length module parameter.
As part of the renaming process the parameter lists have been sorted
alphabetically (request from Doug).

SBC3r26 contains many changes to the Logical Block Provisioning
interfaces (formerly known as Thin Provisioning ditto). This patch
implements support for both the old and new schemes using the same
heuristic as before (whether the LBP VPD page is present).
The new code also allows the provisioning mode (i.e. choice of command)
to be overridden on a per-device basis via sysfs. Two additional modes
are supported in this version:
- WRITE SAME(10) with the UNMAP bit set
- WRITE SAME(10) without the UNMAP bit set. This allows us to support
devices that predate the TP/LBP enhancements in SBC3 and which work
by way zero-detection
Switching between modes has been consolidated in a helper function that
also updates the block layer topology according to the limitations of
the chosen command.
I experimented with trying WRITE SAME(16) if UNMAP fails, WRITE SAME(10)
if WRITE SAME(16) fails, etc. but found several devices that got
cranky. So for now we'll disable discard if one of the commands
fail. The user still has the option of selecting a different mode in
sysfs.

When debugging DIF/DIX it is very helpful to be able to see which DIX
operation is associated with the scsi_cmnd. Include the protection op in

and delay users have been converted to use the new API for that.

It was always abuse to reuse the plugging infrastructure for this,
convert it to the (new) real API for delaying queueing a bit. A
default delay of 3 msec is defined, to match the previous
behaviour.

- Remove the dependency of cxgb4 and cxgb4vf on INET. cxgb3 really
depends on INET, keep it but add it directly to the driver's Kconfig
entry.
- Make the iSCSI drivers cxgb3i and cxgb4i available in the SCSI menu
without requiring any options in the net driver menu to be enabled
first. Add needed selects so the iSCSI drivers can build their
corresponding net drivers.
- Remove CHELSIO_T*_DEPENDS.

__blk_run_queue() automatically either calls q->request_fn() directly
or schedules kblockd depending on whether the function is recursed.
blk-flush implementation needs to be able to explicitly choose
kblockd. Add @force_kblockd.
All the current users are converted to specify %false for the
parameter and this patch doesn't introduce any behavior change.
stable: This is prerequisite for fixing ide oops caused by the new
blk-flush implementation.

The conversion is quite complex given that the libata new error
handler has to be hooked into the current libsas timeout and error
handling. The way this is done is to process all the failed commands
via libsas first, but if they have no underlying sas task (and they're
on a sata device) assume they are destined for the libata error
handler and send them accordingly.
Finally, activate the port recovery of the libata error handler for
each port known to the host. This is somewhat suboptimal, since that
port may not need recovering, but given the current architecture of
the libata error handler, it's the only way; and the spurious
activation is harmless.

This driver is for Broadcom Netxtreme II 57712 chip. The following
patch contains the driver sources for bnx2fc driver. libfc/libfcoe
changes to enable bnx2fc have already gone through the fcoe
tree. bnx2fc is a SCSI low level driver that interfaces with SCSI
midlayer, libfc, libfcoe, cnic modules. bnx2fc driver uses services
of libfc for slow path operations such as FIP and fabric
discovery. The fast path IO perations are performed after offloading
the session information to the underlying FCoE firmware.

Reset using "fcoeadm -r" also needs to restart FIP before
doing libfc lport reset, this is needed for new switch firmware
requiring FIP solicitation before doing FLOGI again during reset.
So this patch does this by doing fcoe_ctlr_link_down and then
fcoe_ctlr_link_up to reset the interface.
The fcoe_ctlr_link_down call path also does lport reset
and then fcoe_ctlr_link_up re-starts the fabric login after
doing FIP solicitation first to get reset feature working
again.

When L2 driver is unloaded, libfcoe_destroy tries to access the fcoe
transport structure matching the netdev. However, since the netdev is
unregistered by that time, it fails to do so. Hence the stale mappings
exists in the fcoe-netdev list. Handle NETDEV_UREGISTER device
notification mechanism to remove the stale fcoe-netdev mapping.

EM anchors list initialization for only master port was not enough to
keep npiv working as described here:-

When abort for an exchange timed out it didn't release the reference to
the exchange resulting in a memory leak.
After discussion with the author of the patch (CC) that introduced this
bug it was suggested to revert that patch.

When an fcoe interface is being destroyed; in the process the
fcoe driver will try to release all the resources it had allocated
for that interface including rports. But, it seems that it does not
release the reference held for the name server rport in that process
resulting into a memory leak. This patch fixes that memory leak.

If the port takes a while to transition we could exhaust
the retries when using DID_TRANSPORT_DISRUPTED. For this
case we do not want to use any of the cmd's
retries, because if the command was running then when
it got failed the retry counter was already incremented.
And if this is the first time we are seeing the command,
(it got queued because it slipped through during the race)
then it should not have its retries incremented. The
fc class will decide the correct handling later.

The fc_block_scsi_eh() might return with status FAST_IO_FAIL
indicating I/O has been terminated due to fast_io_fail timeout.
In this case the rport is still blocked, so any error recovery
will be failing on this port. Hence we need to check if the
return value from fc_block_scsi_eh() is something other than 0,
in which case it should just return with that status.

Based on reading of the FCP2/4 specification, the driver cannot
expect the storage device to send FCP_RSP information. Instead,
the driver should interpret the data which is present in the frame
to base decisions on the success or failure of the system.

By not clearing the 'login needed' nor 'login outstanding' states
for an fcport after a big-hammer (adapter reset), the driver may
not properly perform a PLOGI/PRLI sequence (and lose visibility
to the rport) during a follow-on SNS scan.

The additional increment of dsd_seg was causing the compiler to throw an array
out of bounds warning. This patch moves to a direct assignment of
cmd_pkt->fcp_data_dseg_len so that the compiler doesn't generate an array out
bounds warning.

The ep_disconnect function could be freeing the ep
while beiscsi_conn_get_param is running. This has
the driver use the get ep param callback instead
of the get conn param to fix this.

bnx2i has some checks to try and make sure the ep
is not destroyed while the addr/port is getting
read. However, if after this check:
if (!(bnx2i_conn && bnx2i_conn->ep && bnx2i_conn->ep->hba))
goto out;
bnx2i_conn->ep is cleared by ep_disconnect then we will
oops.
This patches fixes the problem by having the driver
use the get_ep_param callback instead of get_conn_param.

For drivers using the ep callbacks the addr and port
are attached to the endpoint instead of the conn.
This adds a callout to the iscsi_transport to get
ep values. It also adds locking around the get
param call to make sure that ep_disconnect does
not free the LLD's ep interconnect structs from
under us (the ep has a refcount so it will not
go away but the LLD may have structs from other
subsystems that are not allocated in the ep so
we need to protect them from getting freed).

This has iscsi_tcp use the iscsi_conn_get_addr_param
libiscsi function. It also drops the use of the libiscsi
session portal buffers, so they can be removed in
the next patches. Instead of copying the values
at bind time we get them during get() time. If we are
not connected userspace will now get -ENOTCONN,
so it knows that connection is disconnected instead
of a possible stale value.

When iscsid restarts it does not know the connection's
endpoint, so it is getting leaked. This fixes the problem
by having the iscsi class force a disconnect before a
new connection is bound.

For scenarios where the shost is not being passed to bnx2i for the
iSCSI offload connection request, the code would consult the routing
table to select the CNIC device.
This code path will erroneously error out if the corresponding L2
interface's MTU has been setup to > 1500.

Allow CNIC to go through the proper cleanup procedure for an endpoint
which failed to connect. Proper cleanup is necessary for the chip
to reset back to the initial state for the offloaded endpoint.

RRQ Implementation fixes
- Added checks to prevent a call to findnode_did in clr_active_rrq
- Added the del_sync_timer call for the rrq_tmr to the stop_hba_timers routine.
- Added a check in __lpfc_set_active_rrq for the driver unloading to prevent
adding an rrq when the driver is being removed.
- Add code to scsi_iocb_cmpl to check for the remote stop and add the rrq.
- Added the same check to els retry.
- Added code to compare the source did in the els rrq to the vports did and
chose the right exchange ID.
- Initialize the start_cmd pointer to indicate when we have looped through
all of the scsi buffers.
- Remove the need for the lock around the clearing of the active bit in the
rrq.
- Added code to clean the els and fcp xri aborted list and remove the all of
the RRQs for a deleted vport.

Controller will transfer only 32-bits on completion if it
knows we are only using 32-bit tags. Also, some newer controllers
apparently (and erroneously) require that we only use 32-bit tags,
and that we inform the controller of this.

It's not enough to simple avoid putting the board into performant
mode, as we have to set up the interrupts differently, etc. When
I originally tested this module parameter, I tested it incorrectly
without realizing it, and the driver was running in performant mode
the whole time unbeknownst to me.

In a multiple configuration change scenario a remove notification can be
followed by an immediate add notification for the same device, which
will cause the device to be removed but never added back. This patch
fixes the problem by ensuring that in such situations the device will be
added back.

If the target device gets lost, this fix is needed, as it causes
negative unintended responses on basic I/O tests. If the target device
gets lost, the upstream qla2xxx driver returns
SCSI_MLQUEUE_TARGET_BUSY which causes an immediate retry without drop
in the number of allowed retries. This semantic change, as a result of
removing FC_DEVICE_LOST check is reasonable, as it only extends a
short transitional period, until the transport is called to notify
that the rport as lost (fc_remote_port_delete()). Once transport
notification is done, fc_remote_port_chkready() check will take over.

The conversion is quite complex given that the libata new error
handler has to be hooked into the current libsas timeout and error
handling. The way this is done is to process all the failed commands
via libsas first, but if they have no underlying sas task (and they're
on a sata device) assume they are destined for the libata error
handler and send them accordingly.
Finally, activate the port recovery of the libata error handler for
each port known to the host. This is somewhat suboptimal, since that
port may not need recovering, but given the current architecture of
the libata error handler, it's the only way; and the spurious
activation is harmless.

During one of our testing, we noticed that mode select command sent
from the host did not have the lun_table updated.
Problem is root caused to the way lun table is updated. Lun table
update was done after the call to blk_rq_map_kern is made. This was
causing problem because kernel uses bounce buffer(bio_copy_kern) if
the address is not aligned. The command buffer updated after the
call(blk_rq_map_kern) was not going on the wire. Moved the code to
update the lun_table before the call to fix the problem.

If I create a scsi_debug device that is larger than 4GB, the multiplication of
(block * scsi_debug_sector_size) can produce a 64-bit value. Unfortunately,
the compiler sees two 32-bit quantities and performs a 32-bit multiplication,
thus truncating the bits above 2^32. This causes the wrong memory location to
be read or written. Change block and rest to be unsigned long long.

To facilitate LLDDs to reuse the code, skb queue related functions are moved to
libfcoe, so that both fcoe and bnx2fc drivers can use them. The common structures
fcoe_port, fcoe_percpu_s are moved to libfcoe. fcoe_port will now have an
opaque pointer that points to corresponding driver's interface structure.
Also, fcoe_start_io and fcoe_fc_crc are moved to libfcoe.
As part of this change, fixed fcoe_start_io to return ENOMEM if
skb_clone fails.

fcoe uses the system_wq to destroy ports and the work items need to be
flushed before the driver is unloaded. As the work items free the
containing data structure, they can't be flushed directly. The
workqueue should be flushed instead.
Also, the destruction works can be chained - ie. destruction of a port
may lead to destruction of another port where the work item for the
former queues the work for the latter. Currently, the depth of chain
can be at most two and fcoe_exit() makes sure everything is complete
by calling flush_scheduled_work() twice.
With commit c8efcc25 (workqueue: allow chained queueing during
destruction), destroy_workqueue() can take care of chained works on
workqueue destruction. Add and use fcoe_wq instead. Simply
destroying fcoe_wq on driver unload takes care of flushing.

Problem:
From initaitor machine, when queried role of target (other end of connection),
it is "initiator", hence SCSI-ml doesn't send any LUN Inquiry commands.
Fix:
If there is a registered target for FC_TYPE_FCP, extend lport's params
(capability) to be target as well, By default lport params are
INITIATOR only. Having this fix, caused initiator to send SCSI LUN
inquiry command to target.

Remove the existing sysfs entry points of the fcoe.ko module parameters that
are used to create/destroy/enable/disable an FCoE instance, rather, use the
newly added fcoe transport code to attach itself as an FCoE transport provider
when fcoe.ko gets loaded. There is no functionality change on the logic of
fcoe interacts with upper libfc and lower netdev. The fcoe transport only acts
as thin layer to provide a unified interface for all fcoe transport providers
so all FCoE instances on any network interfaces from all vendors can be
managed through the same Open-FCoE.org's user space tool package, which also
has full DCB support.

module. Move the module information to fcoe_transport, and it will have

Add the new fcoe_transport.c file that implements basic fcoe transport
interface. Eventually, the sysfs entries to create/destroy/enable/disable
an FCoE instance will be coming to the fcoe transport layer, who does a
look-up to find the corresponding transport provide and pass the corresponding
action over to the identified provider.
The fcoe.ko will become the default fcoe transport provider that can support
FCoE on any given netdev interfaces, as the Open-FCoE.org's default software
FCoE HBA solution. Any vendor specific FCoE HBA driver that is built on top
of Open-FCoE's kernel stack of libfc & libfcoe as well as the user land tool
of fcoe-utils can easily plug-in and start running FCoE on their network
interfaces. The fcoe.ko will be converted to act as the default provider if
no vendor specific transport provider is found, as it is always added to the
very end of the list of attached transports.

Problem:
In case of exchange responder case, EMA selection was defaulted to the
last EMA from EMA list (lport.ema_list). If exchange ID is selected
from offload pool and not setup DDP, resulting into incorrect
selection of EMA, and eventually dropping the packet because unable to
find exchange.
Fix:
Enhanced the exchange ID selection (depending upon request type and
exchange responder) Made necessary enhancement in EMA selection
algorithm.

When sending an outgoing PRLI as an initiator, get the parameters
from registered providers so that they all get a chance to decide
on roles.
The passive provider is called last, and could override the
initiator role.

When an SCST provider is registered, it needs to know what
local ports are available for configuration as targets.
Add a notifier chain that is invoked when any local port
that is added or deleted.
Maintain a global list of local ports and add an
interator function that calls a given function for
every existing local port. This is used when first
loading a provider.

For multi-sequence exchanges, this allows the target driver
to add a response handler for handling subsequent sequences,
and exchange manager resets.

Allow FC-4 provider modules to hook into libfc, mostly for targets.
This should allow any FC-4 module to handle PRLI requests and maintain
process-association states.
Each provider registers its ops with libfc and then will be called for
any incoming PRLI for that FC-4 type on any instance. The provider
can decide whether to handle that particular instance using any method
it likes, such as ACLs or other configuration information.
A count is kept of the number of successful PRLIs from the remote port.
Providers are called back with an implicit PRLO when the remote port
is about to be deleted or has been reset.
fc_lport_recv_req() now sends incoming FC-4 requests to FC-4 providers,
and there is a built-in provider always registered for handling
incoming ELS requests.
The call to provider recv() routines uses rcu_read_lock()
so that providers aren't removed during the call. That lock is very
cheap and shouldn't affect any performance on ELS requests.
Providers can rely on the RCU lock to protect a session lookup as well.
