In order to keep a sub-device from promiscuously attaching to the
pvrusb2 driver, the i2c adapter's class must be cleared. This change
clears that class by default.

This implements a temporary mechanism to "untrack" an i2c module from
the old i2c layer. The v4l2-subdev related code in the driver will
use this to remove a sub-device from the old i2c layer. In the end,
once the old i2c layer is removed, this will also eventually go away.

This is the first step in the effort to move the pvrusb2 driver over
to using the v4l2-subdev framework. This commit involves mainly
splitting apart pvrusb2-i2c-core - part of it is the driver's I2C
adapter driver and the rest is the old i2c module handling logic. The
i2c module handling junk is moved out to pvrusb2-i2c-track and various
header references are correspondingly updated. Yes, this patch has a
huge pile of checkpatch complaints, but I'm NOT going to fix any of
it. Why? First, I'm moving a large chunk of existing code and I'm
not going to spend time adjusting it to match someone's idea of coding
style. Second, in the end I expect all that moved code to go away by
the time the rework is done so wasting time on it now to adhere to the
standard is in the end a large waste of time.

There are several Kconfig items using CUSTOMIZE. Yet, most use the English
writing CUSTOMISE. This generates lots of trouble, because people sometimes type
the Kbuild item different.
Let's standardise every occurrence using the same syntax.
The changes were generated by this small shell script:

Patch removes err() macros from few usb devices.
It places pr_err in pvrusb2-v4l2.c, dev_err in dabusb and in usbvision
drivers. Beside placing dev_err, patch defines new s2255_dev_err macro
with S2255_DRIVER_NAME in s2255 module.

The name of the pvrusb2 module is not likely to ever change, and there
are plenty of other places where the name is directly coded, so there
is little utility in using a macro to infer the module name here. In
addition, using that macro complicates other uses of the driver
involving older kernels where this macro works differently. Yes I
know for many places we don't have to worry about that. But my
alternative is that I have to build special logic in the pvrusb2
standalone driver to special-case what is otherwise costmetic and that
is just plain nuts for something as trivial as this, especially since
this change does not at all have any compile time or run time impact
on the driver. I'm just removing a nicety that didn't have a lot of
value here to begin with.

It appears that various v4l-dvb drivers are changing to require
explicit initialization before use. This change to the pvrusb2 driver
implements an automatic issuance of VIDIOC_INT_INIT when a module is
bound to the driver, thus conforming to the new behavior.

Previous v4l-dvb changeset id 4cc8ed11e2e0 changed the pvrusb2-hdw
internal API regarding i2c chip debug register access. However that
change failed to also update the corresponding function comment
describing the API. As driver maintained I never saw a request for an
ack on that change; there probably should have been one especially
since the manner in which this API operates was changed - its
interface is now entangled with a v4l specific struct and I would have
preferred to keep this API clear of moving-target v4l-isms such as
this one if at all possible which is why I had done it the way I did
before. But whatever. This commit at least fixes the comment issue.

Implement a new internal function to create a string device
identifier. This ID stays with the specific device, making it useful
to user space to identify specific devices. We use the serial number
if available; otherwise we give up and just spit out a unit/instance ID.

This patch (as1161) changes the interface to
usb_lock_device_for_reset(). The existing interface is apparently not
very clear, judging from the fact that several of its callers don't
use it correctly. The new interface always returns 0 for success and
it always requires the caller to unlock the device afterward.
The new routine will not return immediately if it is called while the
driver's probe method is running. Instead it will wait until the
probe is over and the device has been unlocked. This shouldn't cause
any problems; I don't know of any cases where drivers call
usb_lock_device_for_reset() during probe.

Since the i2c driver ID will be removed in the near future we have to
modify the v4l2 debugging API to use the driver name instead of driver ID.
Note that this API is not used in applications other than v4l2-dbg.cpp
as it is for debugging and testing only.
Should anyone use the old VIDIOC_G_CHIP_IDENT, then this will be logged
with a warning that it is deprecated and will be removed in 2.6.30.

Since internal to v4l2 the ioctl prototype is the same regardless of it
being called through .ioctl or .unlocked_ioctl, we need to convert it all
to the long return type of unlocked_ioctl.

Introduce a struct v4l2_file_operations for v4l2 drivers.
Remove the unnecessary inode argument.
Move compat32 handling (and llseek) into the v4l2-dev core: this is now
handled in the v4l2 core and no longer in the drivers themselves.
Note that this changeset reverts an earlier patch that changed the return
type of__video_ioctl2 from int to long. This change will be reinstated
later in a much improved version.

This patch is part of a larger patch series which will remove
the "char bus_id[20]" name string from struct device. The device
name is managed in the kobject anyway, and without any size
limitation, and just needlessly copied into "struct device".
To set and read the device name dev_name(dev) and dev_set_name(dev)
must be used. If your code uses static kobjects, which it shouldn't
do, "const char *init_name" can be used to statically provide the
name the registered device should have. At registration time, the
init_name field is cleared, to enforce the use of dev_name(dev) to
access the device name at a later time.
We need to get rid of all occurrences of bus_id in the entire tree
to be able to enable the new interface. Please apply this patch,
and possibly convert any remaining remaining occurrences of bus_id.
We want to submit a patch to -next, which will remove bus_id from
"struct device", to find the remaining pieces to convert, and finally
switch over to the new api, which will remove the 20 bytes array
and does no longer have a size limitation.

When the tuner modules were moved to common/tuners, a separate customize
option were added for tuners. However, the automatic selection of the
tuners were still using the older option.
This causes that the automatic selection to fail, if DVB_FE_CUSTOMISE is
selected. Also, since those tuners are now under MEDIA_TUNER_CUSTOMIZE
menu, if you unset MEDIA_TUNER_CUSTOMIZE, you can't manually select the
tuners.
This patch fixes this error by replacing DVB_FE_CUSTOMISE by
MEDIA_TUNER_CUSTOMIZE on all places were a tuner is selected.

The kernel number of a v4l2 node (e.g. videoX, radioX or vbiX) is now
independent of the minor number. So instead of using the minor field
of the video_device struct one has to use the num field: this always
contains the kernel number of the device node.
I forgot about this when I did the v4l2 core change, so this patch
converts all drivers that use it in one go. Luckily the change is
trivial.

Fix deadlock problem in 2.6.27 caused by new USB core behavior in
response to a USB device reset request. With older kernels, the USB
device reset was "in line"; the reset simply took place and the driver
retained its association with the hardware. However now this reset
triggers a disconnect, and worse still the disconnect callback happens
in the context of the caller who asked for the device reset. This
results in an attempt by the pvrusb2 driver to recursively take a
mutex it already has, which deadlocks the driver's worker thread.
(Even if the disconnect callback were to happen on a different thread
we'd still have problems however - because while the driver should
survive and correctly disconnect / reconnect, it will then trigger
another device reset during the repeated initialization, which will
then cause another disconect, etc, forever.) The fix here is simply
to not attempt the device reset (it was of marginal value anyway).

Disable "virtual" IR receiver on for 24xxx devices that have an
internal IR blaster. In that case there's another another IR
receiver present and to leave the virtual receiver available
just causes confusion. This means that 24xxx users will no
longer see a phantom IR chip.

This builds upon the previous pvrusb2 change to more formally
implement full cropping support. This enables access from the
driver's V4L interface, and enables access to full capabilities from
sysfs as well. Note that this is only effective when in analog mode.
It also will only work when the underlying digitizer's driver (saa7115
or cx25840 depending on the hardware) also implements the appropriate
functions.

The driver includes an internal table specifying additional
information on a per device-type basis. This works great until
somebody tries to run-time associate another USB ID with the driver.
This change should hopefully allow the driver to fail gracefully under
such a circumstance.

The earlier change from Hans Verkuil that pushed the BKL from
video_open() down into the drivers should be unneeded for the pvrusb2
driver. This driver's implementation for open already protects its
internal structures through other means, thus the BKL is not required.
This change reverses Hans' previous change, for the pvrusb2 driver.
It probably would have been a good idea for Hans to previously have
asked for my ack before committing his change.

The BKL is now moved from the video_open function in v4l2-dev.c to the
various drivers. It seems about a third of the drivers already has a
lock of some sort protecting the open(), another third uses
video_exclusive_open (yuck!) and the last third required adding the
BKL in their open function.

The device 2040:2950 is a really old variant of the PVR USB2 hardware.
I have just learned of its existence. For the purposes of the pvrusb2
driver, it is functionally identical to the well known 29xxx series
(2040:2900). Amazing that this went undetected for 3+ years.

The Zilog IR chip on HVR-1900 devices is held in reset when the device
initializes. We have to bring this chip out of reset before LIRC has
any chance of operating the chip. So do it.

On 32-bit architectures PAGE_ALIGN() truncates 64-bit values to the 32-bit
boundary. For example:
u64 val = PAGE_ALIGN(size);
always returns a value < 4GB even if size is greater than 4GB.
The problem resides in PAGE_MASK definition (from include/asm-x86/page.h for
example):

The "~" is performed on a 32-bit value, so everything in "and" with
PAGE_MASK greater than 4GB will be truncated to the 32-bit boundary.
Using the ALIGN() macro seems to be the right way, because it uses
typeof(addr) for the mask.
Also move the PAGE_ALIGN() definitions out of include/asm-*/page.h in
include/linux/mm.h.

Back in the early days of the pvrusb2 driver, the kernel class
mechanism in use for the sysfs interface had no means to pass
per-attribute information to the show / store functions. This forced
me to implement a horrible ugly thunking mechanism (i.e. infer the
missing data through the use of dedicated cookie cutter bounce
functions). However now we're using a better mechanism which also
passes enough additional information to the show / store functions
that we no longer need the hack. So eliminate all the crap. Yay!

The driver enforces a "quiet period" on the encoder in certain
situations before attempting to operate it. This seems to help avoid
video encoding errors / corruption. The quiet period was 50msec, but
through experimentation it has been observed to improve further if the
interval is increased to 100msec.

The pvrusb2 driver initially sets the tuner to known broadcast frequencies
in the Chicago area, to ease driver testing for the maintainer.
This patch keeps those default frequencies, but allows them to be altered
via modprobe option. This allows the same ease and convenience for testing
multiple pvrusb2 devices one after another under other conditions and areas.
For instance, the default initial frequency, 175.25 MHz, might not
necessarily be valid on all cable television networks, but usually will be a
valid NTSC broadcast channel.

1) FW_LOADER is defined as:
config FW_LOADER
tristate "Userspace firmware loading support"
depends on HOTPLUG
2) several V4L/DVB driver just selects it;
3) select is not smart enough to auto-select HOTPLUG, if select FW_LOADER.
So, All drivers that select FW_LOADER should also depend on HOTPLUG.
An easier solution (for the end-user perspective) would be to "select HOTPLUG".
However, live is not simple. This would cause recursive dependency issues like
this one:

VIDEO_TUNER is responsible for compilation of tuners.ko module. This were the
previous behaviour before the creation of MEDIA_TUNER.
Before this patch, tuner.ko were created even for drivers that don't need a
tuner (like webcam drivers).

This was a build option in the past, to avoid conflicts with the cxusb module
for digital televsion support. Now that dtv mode support has been merged into
pvrusb2, the OnAir devices are fully supported by this single module. This no
longer should be a build option.

There were several issues in the past, caused by the hybrid tuner design, since
now, the same tuner can be used by drivers/media/dvb and drivers/media/video.
Kconfig items were rearranged, to split V4L/DVB core from their drivers.

Apparently the kernel developers no longer consider it proper
etiquette to use __FUNCTION__; everyone must instead use __func__
(even though it breaks with older compilers). And worse still, actual
effort is being expended to sweep this change throughout the kernel
source tree. Don't these people have better things to do? So...
Completely clean out all use of __FUNCTION__ from the pvrusb2 driver
(it was just in the sysfs interface). I'm not going to use __func__
either. So there.

The pvrusb2 driver was getting had by this scenario:
1. Task A calls kthread_stop() for task B.
2. Before exiting, then Task B calls kthread_stop() for task C.
The problem is, kthread_stop() wants to allocate an internal resource
to itself (i.e. acquire a lock), which won't be released until
kthread_stop() returns. But kthread_stop() won't return until task B
is dead. But task B won't die until it finishes its call to
kthread_stop() for task C, and that will block waiting on the resource
already allocated inside task A. Deadlock.
With the pvrusb2 driver, task A is the caller to pvr_exit(), task B is
the control thread run inside of pvrusb2-context.c, and task C is any
worker thread run inside of pvrusb2-hdw.c.
This problem got introduced by the previous threading setup change,
which was itself an attempt to fix a module tear-down race (which it
actually did fix). The lesson here is that a task being waited on as
part of a kthread_stop() simply cannot be allow to also issue a
kthread_stop() - or we make sure not to issue the enclosing
kthread_stop() until we know that the inner kthread_stop() has
completed first. The solution for the pvrusb2 driver is some hackish
code which changes the main control thread tear down into a two step
process. This then makes it possible to delay issuing the
kthread_stop() on the control thread until after we know that
everything has been torn down first. (And yes, we really need that
kthread_stop() because it's the only way to safely guarantee that a
module-referencing kernel thread has safely returned back out of the
module before we finally remove the module.)

If a disconnect happens before initialization is completed, the
pvrusb2 driver can accidentally touch dangling pointers. The whole
initialization function must be protected by the big_lock, and once
inside that lock, the initialization function should abort if it is
discovered that a disconnect has already taken place.

The pvrusb2 driver - for basically forever - was not enforcing a
proper module tear-down. Kernel threads are used inside the driver
and all must be gone before the module can be safely removed. This
changeset reimplements a chunk of pvrusb2-context.c to enforce this
correctly. Unfortunately this is not a simple fix. The new
implementation also cuts back on kernel thread usage; instead of there
being 1 control thread per instance now it's just 1 control thread
shared by all instances. (By dropping to a single thread then the
module exit function can block on its shutdown and the thread itself
can monitor and cleanly shut down all of the other instances first.)

Implement timed measurement of encoder operation for the first time it
is run. This allows the driver to note when the encoder has been run
successfully for at least 1/4 second. On top of that implement
various bits to ensure that the encoder has been run once before
digital streaming for OnAir devices. This is done via several core
state machine tweaks.

Some tuners seem to not work in digital mode unless the encoder is
healthy. Implement a device attribute to represent this flag and
modify the core state machines to enforce this requirement.

If the device fails to stream, the feed thread will block forever
waiting for buffers. But while in this state it was not looking for
an exit condition from the driver DVB interface. This caused the
thread to jam. Implement a new stop flag (which will be set
appropriately) to tell the thread to stop.

When the DVB interface is not compiled, the pvr2_dvb_props struct is
not available - so it really should be ifdef'ed out as well. This
didn't cause an error because in this context its usage was as an
opaque pointer. But it really shouldn't be present at all if DVB is
not enabled.

The commands to start / stop USB streaming for an analog device are
fairly standard, owing to the fact that all supported devices
apparently started from the same common reference design. However
with digital mode, the commands seem to vary by vendor. This change
makes that variance more explicit. It also cleans up a related
problem for OnAir devices which prevented digital mode from working at
all.

Numerous places in the driver need to issue simple commands to the FX2
microcontroller (e.g. only 1 or 2 bytes, no reply needed). Previously
each place that did this, had to take lock, set up a central buffer,
and call the function to perform the handshake. This change puts
these steps into a single spot. This also has the effect of removing
the need to mess with the control lock from numerous places in the
code.
