In some situations PAV alias devices on LPAR are not accessible.
The initialization procedure required to enable access to PAV alias
devices has to be performed per storage server subsystem and not
only once per storage server.

The request queue flush function of the dasd driver has to dequeue
the requests first and then call the end request function. Otherwise
a kernel bug in ll_rw_block.c might get triggered.

Remove system device class for xpram. It creates the directory hierarchy
under /sys/devices/system/xpram/xpram0. The xpram0 directory is empty and
it is always created while xpram1 and following devices are always missing,
independent if the devices exist or not. Since the xpram devices are
listed in /proc/partitions and /sys/block/ as slram<x> the system device
class for xpram is meaningless.

I/O on a CCW device may stall if a channel path to that device is
logicaly varied off/on. A user I/O interrupt can get misinterpreted
as interrupt for an internal path verification operation due to a
missing check and is therefore never reported to the device driver.
Correct check for pending interruptions before starting path
verification.

In special conditions where a subchannel rejects the HALT I/O-
instruction with a busy indication (cc 2), I/O may stall.
I/O request termination logic retries HALT I/O indefinitely
because it expects HALT I/O to alter the subchannel status which
is not true when cc 2 is returned.
In case of a busy indication, try CLEAR I/O instruction immediately.

1. Multipath devices for which SetPGID is not supported are not handled well.
Use NOP ccws for path verification (sans path grouping) when SetPGID is not
supported.
2. Check for PGIDs already set with SensePGID on _all_ paths (not just the
first one) and try to find a common one. Moan if no common PGID can be
found (and use NOP verification). If no PGIDs have been set, use the css
global PGID (as before). (Rationale: SetPGID will get a command reject if
the PGID it tries to set does not match the already set PGID.)
3. Immediately before reboot, issue RESET CHANNEL PATH (rcp) on all chpids. This
will remove the old PGIDs. rcp will generate solicited CRWs which can be
savely ignored by the machine check handler (all other actions create
unsolicited CRWs).

The module parameters for xpram are not or in a wrong way parsed.
The xpram module uses the module_param_array directive with an int
parameter which causes the kernel to automatically parse the passed
numbers. This will cause errors if arguments are omitted or cause
wrong results if arguments have size qualifiers.
Use module_param_array with charp and parse the arguments later.

Add a reg_mutex to prevent unregistering a subchannel before it has been
registered. Since 2.6.17, we've seen oopses in kslowcrw when a device is
found to be not operational during sense id when doing initial device
recognition; it is not clear yet why that particular problem was not (yet)
observed with earlier kernels...

This patch adds the wrapper function skb_is_gso which can be used instead
of directly testing skb_shinfo(skb)->gso_size. This makes things a little
nicer and allows us to change the primary key for indicating whether an skb
is GSO (if we ever want to do that).

Fixes for several channel measurement facility bugs:
* Blocks copied from the hardware might not be consistent. Solve this
by moving the copying into idle state and repeating the copying.
* avg_sample_interval changed with every read, even though no new block
was available. Solve this by storing a timestamp when the last new
block was received.
* Several locking issues.
* Measurements were not reenabled after a disconnected device became
available again.
* Remove #defines for ioctls that were never implemented.

After setting a path to a dasd offline at the SE, I/O hangs on that
dasd for 5 minutes, then continues.
I/O for which an interrupt will not be reported after the channel
path has been disabled was not terminated by the common I/O layer,
causing the dasd MIH to hit after 5 minutes.
Be more aggressive in terminating I/O after setting a channel path
offline. Also make sure to generate a fake irb if the device
driver issues an I/O request after being notified of the killed
I/O and clear residual information from the irb before trying to
start the delayed verification.

The ccw dump function dasd_eckd_dump_ccw_range can crash because
it does not take care about the IDAL flag in the ccw.
Check for IDALs flag set in CCW and follow the indirect list to
print the data that is refered by the ccw.

The initial i/o to a 3270 device is done using the static module variables
raw3270_init_data and raw3270_init_request. If the 3270 device driver is
built as a module and gets loaded above 2GB, the initial i/o will fail
because these variables will get addresses > 2GB. To make it work the
two variables are moved to struct raw3270 and the data structure is
allocated with GFP_DMA.

Work around the problem that a device cannot be unregistered from
driver_for_each_device() because of klist node refcounting: Get device
after device owned by the driver to be unregistered with driver_find_device()
and then unregister it. This works because driver_get_device() gets us out of
the region of the elevated klist node refcount. driver_find_device() will
always get the next device in the list after the found one has been
unregistered.

Trying to set a DASD root device online can fail under some circumstances
with the message "Read configuration data returned error -5". The cause
is that read configuration data incorrectly aborts with -EIO when it
encounters a temporary busy condition at a storage server.
Perform retry when encountering temporary busy conditions.

Having separate fields in sk_buff for TSO/UFO (tso_size/ufo_size) is not
going to scale if we add any more segmentation methods (e.g., DCCP). So
let's merge them.
They were used to tell the protocol of a packet. This function has been
subsumed by the new gso_type field. This is essentially a set of netdev
feature bits (shifted by 16 bits) that are required to process a specific
skb. As such it's easy to tell whether a given device can process a GSO
skb: you just have to and the gso_type field and the netdev's features
field.
I've made gso_type a conjunction. The idea is that you have a base type
(e.g., SKB_GSO_TCPV4) that can be modified further to support new features.
For example, if we add a hardware TSO type that supports ECN, they would
declare NETIF_F_TSO | NETIF_F_TSO_ECN. All TSO packets with CWR set would
have a gso_type of SKB_GSO_TCPV4 | SKB_GSO_TCPV4_ECN while all other TSO
packets would be SKB_GSO_TCPV4. This means that only the CWR packets need
to be emulated in software.

The path grouping can fail due to non-unique pathgroup-IDs. The source for
the CPU-ID part of the ID was incorrectly specified on 64 bit systems.
Additionally, the length of the ID was too large due to incorrect data packing
declaration. Fix CPU-ID lowcore address and add missing packing declaration.

If zfcp's port erp fails we now call fc_remote_port_delete. This helps
to avoid offlined scsi devices if scsi commands time out due to path
failures. When an adapter erp fails we call fc_remote_port_delete for
all ports on that adapter.

Several problems occured with lcs device driver:
- device not operational anymore after cable pull/plug-in.
- unpredictable results occured, e.g. kernel panic
using cards of type QD8F.
- STOPLAN and delete multicast address command
were not proper recognized by OSA card under heavy network workload.
- channel/device error checks missing in interrupt handler.
To fix all problems at once recovery of lcs devices has been improved.
missing error checks in lcs interrupt handler has been added.
Once a hardware problem occurs lcs will recover the device now properly.

- fix fake_ll during initial device bringup. fake_ll was
not active after first start of the device.
Problem only occured when qeth was built without IPV6 support.
- avoid skb usage after invocation of qeth_flush_buffers,
because skb might already be freed.
- remove yet another useless netif_wake_queue in
qeth_softsetup_ipv6 since this function is only called
when device is going online. In this case card->state will
never be in state UP. So let the net_device queue down .

While debugging why our LCS emulator is having some problems I noticed the
following weirdness in drivers/s390/net/lcs.c routine lcs_irq. The `if'
statement is always true since SCHN_STAT_PCI is defined as 0x80.

As pointed out by Paulo Marques <pmarques@grupopie.com> MAX_IPD_TIME is by
a factor of ten too small. Since this means that we allow ten times more
IPDs in the intended time frame this could result in a cpu check stop of a
physical cpu.

Generate new sysfs-attribute 'uid' that contains an device specific unique
identifier. This can be used to identity multiple ALIASES of the same
physical device (PAV). In addition the sysfs-attributes 'vendor' (containing
the manufacturer of the device) and 'alias' (identify alias or base device) is
added. This is first part of PAV support in LPAR (also valid on zVM).

In case of an instruction processing damage (IPD) machine check in kernel mode
the resulting action is always to stop the kernel. This is not necessarily
the best solution since a retry of the failing instruction might succeed. Add
logic to retry the instruction if no more than 30 instruction processing
damage checks occured in the last 5 minutes.

With CONFIG_SLAB_DEBUG=y networking over qeth doesn't work. The problem is
that the qib structure embedded in the qeth_irq structure needs an alignment
of 256 but kmalloc only guarantees an alignment of 8. When using SLAB
debugging the alignment of qeth_irq is not sufficient for the embedded qib
structure which causes all users of qdio (qeth and zfcp) to stop working.
Allocate qeth_irq structure with __get_free_page. That wastes a small amount
of memory (~2500 bytes) per online adapter.

The dasd state machine is not designed to enable an unformatted device, since
'unformatted' is a final state. The BIODASDENABLE ioctl calls
dasd_enable_device() which never returns if the device is in this special
state. Return -EPERM in dasd_increase_state for unformatted devices to make
dasd_enable_device terminate. Note: To get such an unformatted device online
it has to be re-analyzed. This means that the device needs to be disabled
prior to re-enablement.

Avoid memory allocation with GFP_KERNEL in qdio_establish/qdio_shutdown. Use
memory pool instead. (Otherwise this can lead to an I/O stall where qdio
waits for a free page and zfcp waits for end of error recovery in low memory
situations.)

Fix a race condition in the I/O termination logic. The race can cause I/O to
a dasd device to fail with no retry left after turning one channel path to the
device off and on multiple times.

tty support code will be removed from the ctc network device driver.
Today we have a couple of alternatives which are performing much
better. The second thing is that ctc should be a network
device driver only.
We should not mix tty and networking here.
This first patch will remove the tty code from ctcmain.c .
It also removes the build entry from the Makefile as well as TTY
definitions from ctcmain.h.
The second patch will remove two files, ctctty.c and ctctty.h.

The dasd driver sometimes print the misleading message "Can't offline dasd
device with open count = 0". The reason why it can't offline the device in
this case is that the device is still in the startup phase. Print a more
meaningful message.

Debugging events in cio_trace/hex_ascii are truncated for some trace entries.
Increase trace event size to 16 bytes to cover longer text events, make
CIO_HEX_EVENT an inline function that loops to cover bigger hex events.

cio_ignore_proc_init() returns 1 in case of success and 0 in case of failure.
The caller tests for != 0, so better return 0 in case of success and -ENOENT
in case of failure.

This patch changes several mempool users, all of which are basically just

Undetected edge case for CRT messages to CEX2A caused length to be too short,
thus truncating the message. The solution was to check a different variable
which actually determines which key type is being used.
Increment version number in z90main.c to correct level of 1.3.3, fix copyright
year and add comment about bitlength limit of CEX2A.

If a tape device is assigned to another host, the interrupt for the assign
operation comes back with deferred condition code 1. Under some conditions
this can lead to an endless loop of retries. Check if the current request is
still in IO in deferred condition code handling and prevent retries when the
request has already been cancelled.

When a request is aborted because of a signal, we currently stop the request
via csh, but we do not wait for the interrupt of csh in any case. We free the
request structure and therefore when the interrupt for the csh operation is
presented, the request object is no longer valid and an invalid callback
pointer is used.
To fix this wait until the interrupt for csh arrives and until
wait_event_interruptible() does not return -ERESTARTSYS.

If a deferred CC happens there will be lots of messages, because the retry is
done immediatly in the interrupt handler which can be too fast. To avoid this
requeue the request and schedule the queue to be processed.

The DASD extended error reporting is a facility that allows to get detailed
information about certain problems in the DASD I/O. This information can be
used to implement fail-over applications that can recover these problems.

dasd_cmd just implements three ioctls which are wrappers around functionality
in the core kernel or other modules. When merging those into dasd_mod they
just add 22 lines of code which is far less than the amount of code removed in
the last two patches, and which doesn't spill into another 4k pages when build
modular, while removing a 128lines module.

Add an ->ioctl method to the dasd_discipline structure. This allows to apply
the same kind of cleanups the last patch applied to dasd_ioctl.c to
dasd_eckd.c (the only dasd discipline with special ioctls) aswell.
Again lots of code removed. During auditing the ioctls I found two fishy
return value propagations from copy_{from,to}_user, maintainers please check
those, I've marked them with XXX comments.

Handle ioctls implemented in dasd_ioctl through the normal switch statement
that most drivers use instead of the awkward dasd_ioctl_no_register routine.
This avoids searching a linear list on every call to dasd_ioctl(), and allows
to give the various ioctl implementation functions sane prototypes, aswell as
moving the check for bdev->bd_disk->private_data from the individual functions
to dasd_ioctl. (I think it can't actually every be NULL, but let's keep that
for later)

When cio waits for the interrupt for a basic sense, interrupts for hsch() or
csch() issued in the meantime are wrongly counted as interrupts for the basic
sense and the accumulated irb is passed to the device driver. In
ccw_device_w4sense(), check for clear or halt function in the irb and pass the

(2) Avoid scsi_device allocation during registration of an remote port.

(3) Fix queuecommand behaviour when an zfcp unit is blocked.
(Call scsi_done with DID_NO_CONNECT instead of returning
SCSI_MLQUEUE_DEVICE_BUSY otherwise we might end up waiting
for completion in blk_execute_rq for ever.)

DASD allows to open a device as soon as gendisk is registered, which means the
device is a fake device (capacity=0) and we do know nothing about blocksize
and partitions at that point of time. In case the device is opened by
someone, the bdev and inode creation is done with the fake device info and the
following partition detection code is just using the wrong data.
To avoid this modify the DASD state machine to make sure that the open is
rejected until the device analysis is either finished or an unformatted device
was detected.

It fixes a bug in zfcp which provokes a race
in scsi_scan.c. Finally this can lead to an Oops like:
kernel BUG at fs/sysfs/symlink.c:87!
Correctly set this_id for the host. Otherwise we provoke
a race between scsi_target_reap_work and concurrent
scsi_add_device.

When using the dasd diag discipline, the base discipline module (eckd or fba)
can be unloaded, even though the dasd driver requires both discipline modules
(base and diag) to work correctly.
Implement reference counting for both base and diag discipline modules in
order to fix this.

Using FCP devices with V=V support, the input queue stalled when CCQ 97 had
been returned in qdio_do_eqbs. When this happen we have to reissue the eqbs
instruction.
Another bug was when V=V was enabled we checked if hardware has SIGA-sync
support. If not we returned with 0 from tiqdio_is_inbound_q_done. Thus qdio
lost initiative on FCP devices and input queue stalled. Running devices in
V=V there is no SIGA-sync support but nevertheless we have to process
tiqdio_is_inbound_q_done either.

with ccw_device_disband_done() (which leaves the device in offline state)

When the sclp interface takes very long to serve a request, the sclp core
driver will report a failed request to the sclp tty driver even though the
request is still being processed by the sclp interface. Eventually the sclp
interface completes the request and updates some fields in the request buffer
which leads to a corrupted tty pages list. The next time function
sclp_tty_write_room is called, the corrupted list will be traversed, resulting
in an oops.
To avoid this remove the busy retry limit and increase retry intervals.

The sleep_on function clears a running cqr without waiting for the related
interrupt. This can lead to a panic at the time the interrupt is processed
because the related memory might already be freed. Wait for clear-interrupt
and de-queue cqr prior to return.

The open_count is increased for every opener, that includes the blkdev_get in
dasd_scan_partitions. This tampers the open_count in BIODASDINFO. Hide the
internal open from user-space.

The API and code have been through various bits of initial review by
serial driver people but they definitely need to live somewhere for a
while so the unconverted drivers can get knocked into shape, existing
drivers that have been updated can be better tuned and bugs whacked out.
This replaces the tty flip buffers with kmalloc objects in rings. In the
normal situation for an IRQ driven serial port at typical speeds the
behaviour is pretty much the same, two buffers end up allocated and the
kernel cycles between them as before.
When there are delays or at high speed we now behave far better as the
buffer pool can grow a bit rather than lose characters. This also means
that we can operate at higher speeds reliably.
For drivers that receive characters in blocks (DMA based, USB and
especially virtualisation) the layer allows a lot of driver specific
code that works around the tty layer with private secondary queues to be
removed. The IBM folks need this sort of layer, the smart serial port
people do, the virtualisers do (because a virtualised tty typically
operates at infinite speed rather than emulating 9600 baud).
Finally many drivers had invalid and unsafe attempts to avoid buffer
overflows by directly invoking tty methods extracted out of the innards
of work queue structs. These are no longer needed and all go away. That
fixes various random hangs with serial ports on overflow.
The other change in here is to optimise the receive_room path that is
used by some callers. It turns out that only one ldisc uses receive room
except asa constant and it updates it far far less than the value is
read. We thus make it a variable not a function call.
I expect the code to contain bugs due to the size alone but I'll be
watching and squashing them and feeding out new patches as it goes.
Because the buffers now dynamically expand you should only run out of
buffering when the kernel runs out of memory for real. That means a lot of
the horrible hacks high performance drivers used to do just aren't needed any
more.

tty_insert_flip_char is an old API and continues to work as before, as does
tty_flip_buffer_push() [this is why many drivers dont need modification]. It
does now also return the number of chars inserted
There are also
tty_buffer_request_room(tty, len)
which asks for a buffer block of the length requested and returns the space
found. This improves efficiency with hardware that knows how much to
transfer.

At the moment tty buffers are attached directly to the tty. This is causing a
lot of the problems related to tty layer locking, also problems at high speed
and also with bursty data (such as occurs in virtualised environments)
I'm working on ripping out the flip buffers and replacing them with a pool of
dynamically allocated buffers. This allows both for old style "byte I/O"
devices and also helps virtualisation and smart devices where large blocks of
data suddenely materialise and need storing.
So far so good. Lots of drivers reference tty->flip.*. Several of them also
call directly and unsafely into function pointers it provides. This will all
break. Most drivers can use tty_insert_flip_char which can be kept as an API
but others need more.
At the moment I've added the following interfaces, if people think more will
be needed now is a good time to say
int tty_buffer_request_room(tty, size)
Try and ensure at least size bytes are available, returns actual room (may be
zero). At the moment it just uses the flipbuf space but that will change.
Repeated calls without characters being added are not cumulative. (ie if you
call it with 1, 1, 1, and then 4 you'll have four characters of space. The
other functions will also try and grow buffers in future but this will be a
more efficient way when you know block sizes.
int tty_insert_flip_char(tty, ch, flag)
As before insert a character if there is room. Now returns 1 for success, 0
for failure.
int tty_insert_flip_string(tty, str, len)
Insert a block of non error characters. Returns the number inserted.
int tty_prepare_flip_string(tty, strptr, len)
Adjust the buffer to allow len characters to be added. Returns a buffer
pointer in strptr and the length available. This allows for hardware that
needs to use functions like insl or mencpy_fromio.

Add a compat_ioctl method to the dasd driver so the last entries in
arch/s390/kernel/compat_ioctl.c can go away. Unlike the previous attempt this
one does not replace the ioctl method with an unlocked_ioctl method so that
the ioctl_by_bdev calls in s390 partition code continue to work.

HDIO_GETGEO is implemented in most block drivers, and all of them have to
duplicate the code to copy the structure to userspace, as well as getting
the start sector. This patch moves that to common code [1] and adds a
->getgeo method to fill out the raw kernel hd_geometry structure. For many
drivers this means ->ioctl can go away now.
[1] the s390 block drivers are odd in this respect. xpram sets ->start
to 4 always which seems more than odd, and the dasd driver shifts
the start offset around, probably because of it's non-standard
sector size.

Sanitize some s390 Kconfig options. We have ARCH_S390, ARCH_S390X,
ARCH_S390_31, 64BIT, S390_SUPPORT and COMPAT. Replace these 6 options by
S390, 64BIT and COMPAT.

When the sclp_cpi module is loaded on a system which does not support the
required SCLP call (e.g. on z/VM), ENOSUPP is returned to user space. The
correct return value is EOPNOTSUPP.

Add support for multiple subchannel sets. Works with arbitrary devices in
subchannel set 1 and is transparent to device drivers. Although currently
only two subchannel sets are available, this will work with the architectured
maximum number of subchannel sets as well.
