Add mdio_support and lp_advertising fields to ethtool_cmd. Set these
in mdio45_ethtool_gset{,_npage}().

This implements the ETHTOOL_SPAUSEPARAM operation for MDIO (clause 45)
PHYs with auto-negotiation MMDs.
