The registration function shouldn't initialize the mutex or
list head. The fcoe SW transport should initialize itself
before registering.

1) There were a few functions with a strange layout, i.e. all
arguments on the second line, when not necessary.
Where ever possible I moved the return value to the same line
as the function name. However, when the line was too long

mailing list was that '**/' "is consistently unconventional. Not
wrong, just odd." The Documentation/kernel-doc-nano-HOWTO.txt

If we've just created an interface and the an rport is
logging in we may have a request on the wire (say PRLI).
If we destroy the interface, we'll go through each rport
on the disc->rports list and set each rport's state to NONE.
Then the lport will reset the EM. The EM reset will send a
CLOSED event to the prli_resp() handler which will notice
that the state != PRLI. In this case it frees the frame
pointer, decrements the refcount and unlocks the rport.
The problem is that there isn't a frame in this case. It's
just a pointer with an embedded error code. The free causes
an Oops.
This patch moves the error checking to be before the state
checking.

When a sequence cannot be delivered to the target, the local
port will schedule retries, While this process is in progress,
if we destroy the FCoE interface, the fcoe_sw_destroy routine is
entered, and the fc_exch_mgr_free(lp->emp) is called. Thus
if fc_exch_alloc() is called when retrying the sequence,
the mempool_alloc() will fail to allocate the exchange because
the mempool of the exchange manager has already been released.
This patch is to cancel any pending retry work of the local
port before we start to destroy the interface.
Also, when resetting the local port, we should also stop the
scheduled pending retries.

The fc_fcp_complete_locked detected data underrun in this case and set
the FC_DATA_UNDRUN but that was ignored by fc_io_compl for all cases
including read underrun.
Added code to not to ignore FC_DATA_UNDRUN for read IO and instead
suggested scsi-ml to retry cmd to recover from lost data frame.
Not sure if it is okay to ignore FC_DATA_UNDRUN for other case, so let
code as is for other cases but removed or-ing with zero valued fsp->cdb_status
for those cases.

This allows any rport ELS to retry on LS_RJT.
The rport error handling would only retry on resource allocation failures
and exchange timeouts. I have a target that will occasionally reject PLOGI
when we do a quick LOGO/PLOGI. When a critical ELS was rejected, libfc would
fail silently leaving the rport in a dead state.
The retry count and delay are managed by fc_rport_error_retry. If the retry
count is exceeded fc_rport_error will be called. When retrying is not the
correct course of action, fc_rport_error can be called directly.

The fcoe_xmit could call fc_pause in case the pending skb queue len is larger
than FCOE_MAX_QUEUE_DEPTH, the fc_pause was trying to grab lport->lp_muex to
change lport->link_status and that had these issues :-
1. The fcoe_xmit was getting called with bh disabled, thus causing
"BUG: scheduling while atomic" when grabbing lport->lp_muex with bh disabled.
2. fc_linkup and fc_linkdown function calls lport_enter function with
lport->lp_mutex held and these enter function in turn calls fcoe_xmit to send
lport related FC frame, e.g. fc_linkup => fc_lport_enter_flogi to send flogi
req. In this case grabbing the same lport->lp_mutex again in fc_puase from
fcoe_xmit would cause deadlock.
The lport->lp_mutex was used for setting FC_PAUSE in fcoe_xmit path but
FC_PAUSE bit was not used anywhere beside just setting and clear this
bit in lport->link_status, instead used a separate field qfull in fc_lport
to eliminate need for lport->lp_mutex to track pending queue full condition
and in turn avoid above described two locking issues.
Also added check for lp->qfull in fc_fcp_lport_queue_ready to trigger
SCSI_MLQUEUE_HOST_BUSY when lp->qfull is set to prevent more scsi-ml cmds
while lp->qfull is set.
This patch eliminated FC_LINK_UP and FC_PAUSE and instead used dedicated
fields in fc_lport for this, this simplified all related conditional
code.
Also removed fc_pause and fc_unpause functions and instead used newly added
lport->qfull directly in fcoe.

When a rport goes away, libFC does a plogi which will reset exchanges
at the rport. Clean exchanges at our end, both in transport and libFC.
If transport hooks into exch_mgr_reset, it will call back into
fc_exch_mgr_reset() to clean up libFC exchanges.

fc_exch_mgr structure is private to fc_exch.c. To export exch_mgr_reset to
transport, transport needs access to the exch manager. Change
exch_mgr_reset to use lport param which is the shared structure between
libFC and transport.
Alternatively, fc_exch_mgr definition can be moved to libfc.h so that lport
can be accessed from mp*.

We currently try to spin up drives connected to standby and unavailable
ports. This will never succeed and wastes a lot of time. Fail quickly
if the sense data reports the port is in standby or unavailable state.

Commit f27bac2761 which converted sd to
use ida instead of idr incorrectly removed sd_index_lock around id
allocation and free. idr/ida do have internal locks but they protect
their free object lists not the allocation itself. The caller is
responsible for that. This missing synchronization led to the same id
being assigned to multiple devices leading to oops.

Need to make sure the outgoing pdu can fit into a single skb. When
calulating the max. outgoing pdu payload size, take into consideration
of
- data can be held in the skb's fragment list, assume 512 bytes per

To ensure smooth operations amongst the FCoE and NIC side
components of the ISP81xx chip, the FCoE driver (qla2xxx) must
ensure the 10gb NIC driver (qlge) does not timeout waiting for
IDC (Inter-Driver Communication) acknowledgments. The
acknowledgment requirements are trivial -- a simple mirroring of
incoming mailbox registers during the AEN to a process-context
capable mailbox command.

Correct response-queue-0 processing by instructing the firmware
to run with interrupt-handshaking disabled, similarly to what is
now done for all non-0 response queues. Since all
response-queues now run in the same mode, the driver no longer
needs the hot-path 'is-disabled-HCCR' test.

Why we see panic at iscsi_eh_cmd_timed_out()
The mpp virtual HBA doesn’t have a specific scsi_transport. Therefore,
the
blank_transport_template will be assigned to the virtual host of the MPP
virtual HBA by SCSI middle level. Please note that the statement C has

It is very easy to understand why we get panic in the
iscsi_eh_cmd_timed_out().
A scsi_cmnd from a no-iscsi device definitely can not resolve out a
session and
session->lock. The panic can be happed anywhere during the differencing.

During cancel testing it has been shown that 15 seconds is not
nearly long enough for the VIOS to respond to a cancel under
loaded situations. Increasing this timeout to 60 seconds allows
time for the VIOS to cancel the outstanding commands and prevents
us from escalating to a full host reset, which can take much longer.

The ibmvfc driver has a bug in its SCN handling. If it receives
an ELS event such asn an N-Port SCN event or an unsolicited PLOGI,
or any other SCN event which causes ibmvfc_reinit_host to be called,
it is possible that we will call fc_remote_port_add for a target
that already has an rport added, which can result in duplicate
rports getting created for the same targets. Fix this by calling
fc_remote_port_rolechg in this scenario instead to report any possible
role change that may have occurred.

Currently the ibmvfc driver sets the IBMVFC_CLASS_3_ERR flag
in the VFC Frame if both the adapter and the device claim support
for Class 3. However, this bit actually refers to Class 3 Error
Recovery, which is currently not supported by the VIOS. Setting this
bit can cause lots of command timeout responses from the VIOS resulting
in general instability. Fix this by never setting this bit.

The device number denoting a generic SCSI devices (sg) in a blktrace
trace is broken; major and minor are always 0. It looks like
sdp->device->sdev_gendev.devt is not initialized properly.
The fix below uses other data to make up a valid device number,
similar to the way an sg device number is generated for sysfs output.

We were running i/o and performing a bunch of hba resets in a loop.
This forces a lot of target removes and then rescans. Since the
resets are occuring during scan it's causing the scan i/o to timeout,
invoking error recovery, etc. We end up getting some nasty crashing
in scsi_scan.c due to references to old sdevs that are failing
but had some lingering references that kept them around.
Fix by setting device state to SDEV_DEL if the LLD's slave_alloc
fails.

The ibmvscsi client driver is not unmapping the SCSI command after
encountering a DMA mapping error while trying to map an indirect
scattergather list for the event pool. This leads to a leak of DMA
entitlement that could result in the device failing future DMA operations
in a CMO environment.

Currently, netlink_broadcast() reports errors to the caller if no
messages at all were delivered:
1) If, at least, one message has been delivered correctly, returns 0.
2) Otherwise, if no messages at all were delivered due to skb_clone()
failure, return -ENOBUFS.
3) Otherwise, if there are no listeners, return -ESRCH.
With this patch, the caller knows if the delivery of any of the
messages to the listeners have failed:
1) If it fails to deliver any message (for whatever reason), return
-ENOBUFS.
2) Otherwise, if all messages were delivered OK, returns 0.
3) Otherwise, if no listeners, return -ESRCH.
In the current ctnetlink code and in Netfilter in general, we can add
reliable logging and connection tracking event delivery by dropping the
packets whose events were not successfully delivered over Netlink. Of
course, this option would be settable via /proc as this approach reduces
performance (in terms of filtered connections per seconds by a stateful
firewall) but providing reliable logging and event delivery (for
conntrackd) in return.
This patch also changes some clients of netlink_broadcast() that
may report ENOBUFS errors via printk. This error handling is not
of any help. Instead, the userspace daemons that are listening to
those netlink messages should resync themselves with the kernel-side
if they hit ENOBUFS.
BTW, netlink_broadcast() clients include those that call
cn_netlink_send(), nlmsg_multicast() and genlmsg_multicast() since they
internally call netlink_broadcast() and return its error value.

There is currently a DMA mapping leak that can occur in the ibmvfc
driver if we fail to allocate a scatterlist. Fix this by unmapping
the scatterlist in the failure path. Additionally, only log an error
for a scatterlist allocation failure if the log level is greater
than the default, since this can occur when running Active Memory
Sharing and this is not considered an error.

Commit f78badb1ae ([SCSI] fc
transport: pre-emptively terminate i/o upon dev_loss_tmo timeout)
changed the callback semantics of dev_loss_tmo and
terminate_rport_io such that repeated calls could be made. This
could result in the the driver using stale (NULLed-out, in
dev_loss_tmo) data from the rport. Correct this by addint a
simple check to ensure a valid fcport is attached.

When clearing the flash device's SR, the comment is incorrect...
clearing the SR is 2 steps:
1. the SR protect bit is 1, so the first write zero clears only
that bit,
2. the SR protect bit is now 0, so the next write zero clears the
remaining bits.
The sector erase debug print more correctly identifies that the erase failed.

Pre-ISP81XX parts (including ISP24xx and ISP25xx) could contain a
firmware image within a segment of flash, driver would fallback
to loading this firmware if the request-firmware interface failed
(userspace .bin file). Moving forward, all ISP81XX parts will
ship with a suggested-to-be-used firmware image within flash
which all driver should first attempt to load. If the flash
firmware load fails, the driver will then fallback to loading
firmware via the request-firmware interface (ql8100_fw.bin).

Original code would incorrectly bypass serialization if the DPC
thread were performing a big-hammer operation (ISP abort). This
short circuit, though rare, would subsequently stomp on a
secondary thread's mailbox command execution. Found during
ISP81XX testing.

qla4xxx does not check the I_T nexus values correctly
so it ends up creating one session to the target. If
a portal should disappear or they should be reported
in different order the driver will think it is already
logged in when it could now be speaking to a different
target portal or accessing it through a different
initiator port (iscsi initiator port is not tied to
hardware and is just the initiator name plus isid
so you could end up with multiple ports through one
host).
This patch has the driver check the iscsi scsi port
values when matching sessions (we do not check
the initiator name because that is static). It results
in a portal from each target portal group getting
logged into instead of just one per target. In the future
the firmware should hopefully send us notification of other
sessions that are created to other portals within the
same tpgt and the sessions should have different isids.

I am not sure what happened. It looks like we have always leaked
the q->queue that is allocated from the kfifo_init call. nab finally
noticed that we were leaking and this patch fixes it by adding a
kfree call to iscsi_pool_free. kfifo_free is not used per kfifo_init's
instructions to use kfree.

This patch makes cxgb3 invoke the GRO hooks instead of LRO. As
GRO has a compatible external interface to LRO this is a very
straightforward replacement.
I've kept the ioctl controls for per-queue LRO switches. However,
we should not encourage anyone to use these.
Because of that, I've also kept the skb construction code in
cxgb3. Hopefully we can phase out those per-queue switches
and then kill this too.

__scsi_device_lookup_by_target() will always return
the first sdev with a matching LUN, regardless of
the state. However, when this sdev is in SDEV_DEL
scsi_device_lookup_by_target() will ignore this
device and so any valid device on the list after
the deleted device will never be found.
So we have to modify __scsi_device_lookup_by_target()
to skip any device in SDEV_DEL.

Because scsi_release_buffers() is called before commands that go
through the ACTION_RETRY and ACTION_DELAYED_RETRY legs are requeued.
However, they're not re-prepared, so nothing ever reallocates the
buffer resources to them. Fix this by releasing the buffers only if
we're not going to go down these legs (but scsi_release_buffers() on
all legs including two in scsi_end_request(); this latter needs a
special version __scsi_release_buffers() because the final one can be
called after the request has been freed, so the bidi test in
scsi_release_buffers(), which touches the request has to be skipped).

This patch makes part of the scsi probe (which is mostly device spin up and the
partition scan) asynchronous. Only the part that runs after getting the device
number allocated is asynchronous, ensuring that device numbering remains stable.

When we reworked the transport for the rport lifetimes, in cases where the
rport was reused as a container for tgt id bindings, we inadvertantly
removed the callback to the driver indicating that dev_loss_tmo had fired.
This patch restores that functionality.

At some point since 2.6.22, the aha152x_cs driver stopped working and
started erring on load with the following messages:
kernel: pcmcia: request for exclusive IRQ could not be fulfilled.
kernel: pcmcia: the driver needs updating to supported shared IRQ lines.

DIF does not work with 6-byte commands so we previously ignored those
commands when preparing a request. However, DIX does not need
RDPROTECT/WRPROTECT to be set and 6-byte commands are consequently
perfectly valid in host-only mode.
This patch fixes a problem where we would set the wrong DIX operation
when issuing commands to a legacy disk.

broke DIX error handling. Also, we are now using EILSEQ to indicate
integrity errors to the upper layers (as opposed to regular EIO
failures). This allows filesystems to inspect buffers and decide
whether to retry the I/O. Update scsi_io_completion() accordingly.

[SCSI] simplify scsi_io_completion()
because the simplification uses scsi_queue_insert(). The problem with
this function is that it expects to be called from the completion path
while the command is still outstanding, so it decrements the device
and host busy counts to do the requeue. The problem is that
scsi_io_completion() is a path executed well after these counts have
*already* been decremented, leading to a double decrement if the
command goes down any error path leading to ACTION_DELAYED_RETRY.
The fix is to allow a private function __scsi_queue_insert() with a
flag to say whether the busy counters should be decremented. This is
made static to scsi_lib.c to discourage other use.

I increased the delay step by step until loading of mvsas
reliably detected the drive 200 times in sequence. A much better
approach would be to monitor the hardware for some flag which
indicates that port detection has finished, but I do not have any
hardware documentation.

Make enlarge_buffer() retry allocation if the previously chosen page
order was too small. Really limit the page order to 6. Return error if

st_scsi_execute is a helper function to perform SCSI commands
involving data transfer between user and kernel space (st_read and
st_write).
It's the future plan to combine this with st_scsi_kern_execute helper
function.

This patch simiplifies the fragment buffer management a bit, all the
buffers in the fragment list become the same size. This is necessary
to use the block layer API (sg driver was modified in the same way)
since the block layer API takes the same size page frames instead of
scatter gatter.

The commit 818827669d (block: make
blk_rq_map_user take a NULL user-space buffer) extended
blk_rq_map_user to accept a NULL user-space buffer with a READ
command. It was necessary to convert sg to use the block layer mapping
API.
This patch extends blk_rq_map_user again for a WRITE command. It is
necessary to convert st and osst drivers to use the block layer
apping API.

This fixes bio_copy_user_iov to properly handle the partial mappings
with struct rq_map_data (which only sg uses for now but st and osst
will shortly). It adds the offset member to struct rq_map_data and
changes blk_rq_map_user to update it so that bio_copy_user_iov can add
an appropriate page frame via bio_add_pc_page().

In a previous patch to fix an issue with error recovery,
the behavior of the max_requests module paramater was also
changed. If, for some reason, max_requests is set to one by
the user, we will end up with a negative number for can_queue.
Fix this by making max_requests not include the two event structs
needed to do error recovery.

The remove() callback in platform drivers should return int in
accordance to the definition of the platform_driver structure.
However, the SGI-specific WD93 SCSI controller driver defines
the callback as a void function, which causes the following
compilation warning:
drivers/scsi/sgiwd93.c:314: warning: initialization from
incompatible pointer type
This patch fixes the warning by changing the return type of
the remove() callback to what the core driver code requires.

If a link down event is received, outstanding commands may get
returned to the ibmvfc driver with a "transaction cancelled implicit"
response. This is currently translated to DID_ABORT, which does
not get retried by SCSI core, but rather passes the failure up
the stack. This can result in I/O errors at the filesystem level.
Fix up this response a well as a few other error responses.

As planed, this removes ide-scsi.
The 2.6 kernel supports direct writing to ide CD drives, which
eliminates the need for ide-scsi. ide-scsi has been unmaintained and
marked as deprecated.

fcoe selects libfc and requires SCSI and PCI (the SCSI requirement is
implicitly covered by an enclosing if). Fix them both up so they
cannot be configured in an invalid state: make LIBFC select
SCSI_FC_ATTRS and make FCOE depend on PCI and select LIBFC.

cxgb3i requires the cxgb3 net driver, so it selects it. However,
cxgb3 has dependencies which the select cannot see. Fix this by
separating out the cxgb3 dependencies into a separate hidden config
option (CONFIG_CHELSIO_T3_DEPENDS) and make both cxgb3 and cxgb3i
depend on it.

Now that (almost) all host drivers have been fixed not to abuse ide_lock
and core code usage of ide_lock has been sanitized we may safely replace
ide_lock by per-hwgroup locks.
This patch is partially based on earlier patch from Ravikiran G Thirumalai.
While at it:
- don't use deprecated HWIF() and HWGROUP() macros
- update locking documentation in ide.h

While doing various error injection testing, such as cable
pulls and target moves, some issues were observed in handling
these events. This patch improves the way these events are handled
by increasing the delay waiting for the fabric to settle and also
changes the behavior of Link Up to break the CRQ to ensure everything
gets cleaned up properly on the VIOS.

Pre-emptively terminate i/o on the rport if dev_loss_tmo has fired.
The desire is to terminate everything, so that the i/o is cleaned up
prior to the sdev's being unblocked, thus any outstanding timeouts/aborts
are avoided.
Also, we do this early enough such that the rport's port_id field is
still valid. FCOE libFC code needs this info to find the i/o's to
terminate.

Following changes have been made.
1. qla_hw_data structure holds an array for request queue pointers,
and an array for response queue pointers.
2. The base request and response queues are created by default.
3. Additional request and response queues are created at the time of vport
creation. If queue resources are exhausted during vport creation, newly
created vports use the default queue.
4. Requests are sent to the request queue that the vport was assigned
in the beginning.
5. Responses are completed on the response queue with which the request queue
is associated with.

Encapsulation protocol for running Fibre Channel over Ethernet interfaces.
Creates virtual Fibre Channel host adapters using libfc.
This layer is the LLD to the scsi-ml. It allocates the Scsi_Host, utilizes
libfc for Fibre Channel protocol processing and interacts with netdev to
send/receive Ethernet packets.

libFC is composed of 4 blocks supported by an exchange manager
and a framing library. The upper 4 layers are fc_lport, fc_disc,
fc_rport and fc_fcp. A LLD that uses libfc could choose to
either use libfc's block, or using the transport template
defined in libfc.h, override one or more blocks with its own
implementation.
The EM (Exchange Manager) manages exhcanges/sequences for all
commands- ELS, CT and FCP.
The framing library frames ELS and CT commands.
The fc_lport block manages the library's representation of the
host's FC enabled ports.
The fc_disc block manages discovery of targets as well as
handling changes that occur in the FC fabric (via. RSCN events).
The fc_rport block manages the library's representation of other
entities in the FC fabric. Currently the library uses this block
for targets, its peer when in point-to-point mode and the
directory server, but can be extended for other entities if
needed.
The fc_fcp block interacts with the scsi-ml and handles all
I/O.

The ibmvscsi driver currently has a bug in it which can result
in it using up all its event structs for commands. If something
results in all those commands timing out, we won't have any resources
left to send aborts or resets. This results in escalating to a host reset
in order to recover, which is a bit heavy handed. This fixes it
by reducing can_queue by two in order to have resources to do EH.
It also changes the max_requests module parameter so that it is not
writable at runtime, since the code really does not handle it changing
at runtime.

st_scsi_kern_execute is a helper function to perform SCSI commands
synchronously. It supports data transfer with a liner in-kernel buffer
(not scatter gather). st_scsi_kern_execute internally uses
scsi_execute().
The majority of st_do_scsi can be replaced with
st_scsi_kern_execute. This is a preparation for rewriting st_do_scsi
to remove obsolete scsi_execute_async().

Per the recent discussions at the Linux Plumbers Conference, when
enabling MSI or MSI-X, generate a test interrupt to verify the
interrupt routing is working properly. If the test interrupt fails,
fall back to MSI first, and if that fails as well, to INTx. If the
interrupt test fails with INTx, log an error and fail the PCI probe.
Also changed the use of spin_(lock|unlock) to the _irq(save|restore)
variants in the interrupt handlers because with multi-message MSI-X,
both interrupt handlers can now run in parallel.

The phba hbq_get address was setup before the HBA reported support for
Interrupt Enable Block (INB). This caused loopback tests to fail
after all HBQs had been used once. Moved setting of hba_get until
after config port mailbox command completes.

This patch changes scsi_probe_lun() to retry INQUIRY if the device has
not actually sent back any INQUIRY data,
This enables the Thecus N2050 storage device to work better. The
firmware on that device starts up strangely; it sends no data in
response to the initial INQUIRY, and it sends the INQUIRY information
in response to the followup REQUEST SENSE. But after that it works
better, so retrying the INQUIRY is enough to get it going.

Adds a delay prior to retrying a failed NPIV login. This fixes
a scenario if the backing fibre channel adapter is getting reset
due to an EEH event, NPIV login will fail. Currently, ibmvfc
retries three times very quickly, resets the CRQ and tries one
more time. If the adapter is getting reset due to EEH, this isn't
enough time. This adds a delay prior to retrying a failed NPIV
login and also increments the number of retries.

cxgb3i does not offload the processing of the header,
but it will always process the padding. This patch
adds a padding offload flag to detect when the LLD
supports this.
The patch also modifies the header processing so that
we do not try to read/bypass the header dugest in the
skb. cxgb3i will not include it with the header like
with other offload cards.

bnx2i and cxgb3i need to encode LLD info in the itt so that
the firmware/hardware can process the pdu. This patch allows
the LLDs to encode info in the task->hdr->itt that they
setup in the alloc_pdu callout (any resources that are allocated
can be freed with the pdu in the cleanup_task callout). If
the LLD encodes info in the itt they should implement a
parse_pdu_itt callout. If parse_pdu_itt is not implemented
libiscsi will do the right thing for the LLD.

This modifies the login buffer allocation to use __get_free_pages.
It will allow drivers that want to send this data with zero copy
operations to easily line things up on page boundaries.

cxgb3i offloads data transfers. It does not offload the entire scsi/iscsi
procssing like qla4xxx and it does not offload the iscsi sequence
processing like how bnx2i does. cxgb3i relies on iscsi_tcp for the
seqeunce handling so this changes how we transfer unsolicitied data by
adding a common r2t struct and helpers.

cxgb3i is unlike qla4xxx and bnx2i in that it does not offload entire
scsi commands or iscsi sequences. Instead it only offloads the transfer
of a ISCSI DATA_IN pdu's data, the digests and padding. This patch fixes up the
iscsi tcp recv path so that it exports its skb recv processing so
cxgb3i and other drivers can call them. All they have to do is pass
the function the skb with the hdr or data pdu header and this function
will do the rest.

Since we're trying to eliminate struct scsi_device timeout, the tape
driver has to be updated to use the block queue timeout instead. The
tape use of scsi_device timeout looks to be self consistent, so I don't
think this necessarily fixes any bug, but it has to be done to allow me
to remove the timeout parameter from struct scsi_device.

On the Quadra 900 and 950 there are two ESP chips sharing one IRQ. Because
the shared IRQ is edge-triggered, we must make sure that an IRQ transition
from one chip doesn't go unnoticed when the shared IRQ is already active
due to the other. This patch prevents interrupts getting lost so that both
SCSI busses may be used simultaneously.

Fix asm constraints and arguments so as not to transfer an odd byte when
there may be more words to transfer. The bug would probably also cause
exceptions sometimes by transferring one too many bytes.

This patch (as1142b) consolidates a lot of repetitious code in
scsi_io_completion(). It also fixes a few comments. Most
importantly, however, it clearly distinguishes among the three sorts
of retries that can be done when a command fails to complete:
Unprepare the request and resubmit it, so that a new
command will be created for it.
Requeue the request directly so that it will be retried
immediately using the same command.
Requeue the request so that it will be retried following
a short delay.
Complete the remainder of the request with an I/O error.

1. For several error conditions, we would now print the sense twice
in slightly different ways, so unify the location of sense
printing.
2. I added more descriptions to actual failure conditions for
better debugging
3. according to spec, ABORTED_COMMAND is supposed to be retried
(except on DIF failure). Our old behaviour of erroring it looks
to be a bug.
4. I'd prefer not to default initialise the action variable because
that ensures that every leg of the error handler has an
associated action and the compiler will warn if someone later
accidentally misses one or removes one.

The virtual fibre channel stack can return a failure response for a command
indicating the port login has been invalidated without sending the client
an async event. Add code to handle this response and initiate a PLOGI.

The CRQs used by the ibmvfc driver are read and written by both
the client and the server. Therefore, we need to mark them volatile
so that we do not cache their contents when handling an interrupt.
This fixes a problem which can surface as occasional command timeouts.
No commands were actually timing out, but due to accessing cached data
for the CRQ in the interrupt handler, the interrupt was not processing
all command completions as it should.

Following changes have been made to the qla2xxx FC driver in
preparation for the multi- queue and future SR IOV hardware.
1. scsi_qla_host structure has been changed to contain scsi host
specific data only.
2. A new structure, qla_hw_data is created to contain HBA specific
hardware data.
3. Request and response IO specific data strucures are created.
4. The global list of fcports for the hba is not maintained anymore,
instead a fcport list is construted on per scsi_qla_host.

This patch improves handling of TASK ABORTED status by Linux SCSI
mid-layer. Currently, command returned with this status considered
failed and returned to upper layers. It leads to additional error
recovery load on file systems and block layer, which sometimes can
cause undesired side effects, like I/O errors and file systems
corruptions. See http://lkml.org/lkml/2008/11/1/38, for instance.
From other side, TASK ABORTED status is returned by SCSI target if the
corresponding command was aborted by another initiator and the target
has TAS bit set in the control mode page. So, in the majority of cases
commands with TASK ABORTED status should be simply retried. In other
cases, maybe_retry path will not retry if no retries are allowed.

When the mode select sent to the controller fails with the retryable
error, it is better to retry the mode_select from the hardware handler
itself, instead of propagating the failure to dm-multipath.

If the ibmvfc driver is in discovery attempting to log into a target
and it encounters an error, the command may get retried one or more
times, depending on the error received. If the retries are
unsuccessful such that the discovery thread gives up on discovery to
that target, the target ends up in a state where, if SCSI core had
previously known about the device, the host will get unblocked but the
host will not be logged into the target, causing any commands sent to
the target to fail. This patch fixes this so that if this occurs, the
target is deleted such that the normal dev_loss processing can occur
instead.

Due to an ambiguity in the VIOS VFC interface specification,
abort/cancel handling is not done correctly and can result in double
completion of commands. In order to cancel all outstanding commands to
a device, a cancel must be sent, followed by an abort task set. After
the responses are received for these commands, there may still be
commands outstanding, in the process of getting flushed back, in which
case, we need to wait for them. This patch removes the assumption that
if the abort and the cancel both complete successfully that the device
queue has been flushed and waits for all the responses to come back.

If either a "transport fault" or a "general transport" error is received
and no other error information is available, the command is improperly
returned as successful. Fix this to return DID_ERROR in this case.

We need to check the address that pci_alloc_consistent() returns since
it might fail.
When pci_alloc_consistent() fails, some IOMMUs set the dma_handle
argument to zero. So we can't use fibptr->hw_fib_pa directly here.

Use the newly introduced pci_ioremap_bar() function in drivers/scsi.
pci_ioremap_bar() just takes a pci device and a bar number, with the goal
of making it really hard to get wrong, while also having a central place

It's called under that lock everywhere else and it does alter the
request state, so it should be.
This one occurance in scsi_requeue_command() could open a window where
req->special is set to NULL while the requests is going through either
timeout or completion processing leading to NULL pointer derefs of the
sort complained of in bugzillas 12020 and 12195.

The test-unit-ready portion of this patch was causing boots to fail on
my test machine (as in http://lkml.org/lkml/2008/12/5/161). With this
patch in place, the system is booting reliably.
Mike Anderson found the same problem in the hp_hw_start_stop code,
and I applied the same solution in cdrom_read_cdda_bpc.

The hardware supports transfers up to a page boundary per buffer.
Currently, we work around that in the DMA code by splitting each
buffer up as we run through the scatterlist. Avoid this by telling
the block layers about the hardware restriction.
Eventually, this will allow us to phase out the splitting code,
but not until the old IDE layer allows us to control the value it
gives to blk_queue_segment_boundary().

Calling crq_queue_create could lead to the creation of a rport. We
need to set up everything before creating a rport. This moves
crq_queue_create to the end of initialization to avoid a race which
causes an oops if lost.

Update FMODE_NDELAY before each ioctl call so that we can kill the
magic FMODE_NDELAY_NOW. It would be even better to do this directly
in setfl(), but for that we'd need to have FMODE_NDELAY for all files,
not just block special files.

This regression was added in 2.6.27, when the mtask and ctask were
merged into the the common task struct. The patch applies to
scsi-rc-fixes, but also applies to 2.6.27 with some offsets.
The problem is that __iscsi_conn_send_pdu assumes that userspace was
not sending nops with the format it is checking for in the "if" below.
It turns out that older userspace tools are. This patch moves the
setting of the internal ping_task tracker (it tracks libiscsi current
outstanding nop) to iscsi_send_nopout which is only used by kernel callers.

scsi_eh_try_stu() was still using the timeout parameter in the device
which is now not set (i.e. zero filled) meaning that it waited no time
at all for the start unit command to complete (leading the routine to
conclude failure every time). This lead to a 2.6.27 regression:

Where firewire devices that were non spec compliant wouldn't spin up.
Fix this by using the block queue timeout value instead.

The code
if (shost->dma_channel != NO_ISA_DMA)
free_dma(shost->dma_channel);
in there is triggerable only if we have CONFIG_ISA (we only set ->dma_channel to
something other than NO_ISA_DMA under #ifdef CONFIG_ISA). OTOH, free_dma() is
not guaranteed to be there in absense of CONFIG_ISA. IOW, driver runs into
undefined symbols on PCI-but-not-ISA configs (e.g. on frv) and it's a false
positive.
Fix: put the entire if () under #ifdef CONFIG_ISA; behaviour doesn't change and
dependency on free_dma() disappears for !CONFIG_ISA.

dpt_i2o.c::adpt_i2o_to_scsi() reads the value at (reply+5) which
should contain the length in bytes of the transferred data. This
would be correct if reply was a u32 *. However it is a void * here,
so we need to read the value at (reply+20) instead.
The value at (reply+5) is usually 0xff0000, which is apparently
'large enough' and didn't cause any trouble until 2.6.27 where
commit 427e59f09f

Drivers want to be able to return DID_TRANSPORT_DISRUPTED and
have it do the right thing for commands like tape and passthrouh
as far as retries go. The LLDs previously used DID_BUS_BUSY or DID_ERROR
which followed the cmd->retries limit, but DID_TRANSPORT_DISRUPTED
was skipping that check so it could have caused a problem with tape
commands.
This patch has DID_TRANSPORT_DISRUPTED check the cmd->retries/cmd->allowed.

driver was incorrectly returning a SUCCESS status if the driver's
request to the firmware to abort a command failed. By doing so,
the mid-layer believed, incorrectly, that the command has
completed and has been returned (ultimately clearing
scsi_cmnd.request_buffer) yet the driver still has the command.
What should correctly happen is a mid-layer escalation
(device-reset, etc.) of recovery during which the driver will
eventually return the outstanding commands to the mid-layer.

As it is, all instances of ->release() for files that have ->fasync()
need to remember to evict file from fasync lists; forgetting that
creates a hole and we actually have a bunch that *does* forget.
So let's keep our lives simple - let __fput() check FASYNC in
file->f_flags and call ->fasync() there if it's been set. And lose that
crap in ->release() instances - leaving it there is still valid, but we
don't have to bother anymore.

The iscsi_ibft.c changes are almost certainly a bugfix as the
pointer 'ip' is a u8 *, so they never print the last 8 bytes
of the IPv6 address, and the eight bytes they do print have
a zero byte with them in each 16-bit word.
Other than that, this should cause no difference in functionality.

When computing the full size of the device, we need to cast
sdkp->capacity before shifting, since in some configurations sector_t
can be a 32-bit number.

This patch (as1138) removes from sd.c some old code for switching from
10-byte commands to 6-byte commands. This code is redundant -- the
switching for READ and WRITE is already handled in
scsi_io_completion() and the switching for MODE SENSE is already
handled in scsi_mode_sense(). (There is no comparable switch for MODE
SELECT, but I doubt one is needed.)
Furthermore the other handlers do a better job; they check for
appropriate ASC and ASCQ values before blindly switching the size.
The code in sd.c is known to cause problems with some devices by
switching when it shouldn't.

interrupts inside sg_copy_buffer), no need to disable interrupts
before calling scsi_sg_copy_from_buffer. So we can simplify
twa_scsiop_execute_scsi_complete() a bit, which disables interrupts
just for scsi_sg_copy_from_buffer.

The current handling of NO_SENSE check condition is the same as
RECOVERED_ERROR, and assumes that in both cases, the I/O was fully
transferred.
We have seen cases of arrays returning with NO_SENSE (no error), but
the I/O was not completely transferred, thus residual set. Thus,
rather than return good_bytes as the entire transfer, set good_bytes
to 0, so that the midlayer then applies the residual in calculating
the transfer, and for sd, will fail the I/O and fall into a retry
path.

This patch implements q->lld_busy_fn() for scsi mid layer to export
its busy state for request stacking drivers.
For efficiency, no lock is taken to check the busy state of
shost/starget/sdev, since the returned value is not guaranteed and
may be changed after request stacking drivers call the function,
regardless of taking lock or not.
When scsi can't dispatch I/Os anymore and needs to kill I/Os
(e.g. !sdev), scsi needs to return 'not busy'.
Otherwise, request stacking drivers may hold requests forever.

Take the Diag/Activity-LED of the HBA out of diagnostic mode on PM
resume after successful PM resume from standby and HBA restart, it
remained always on before. If something fails before complete
recovery, it should remain on, since it is a diagnostics LED, reason
for the used higher layer for the clear.

The problem is essentially caused by us eating UNIT_ATTENTION
conditions in scsi_test_unit_ready(). Fix by updating the ->changed
flag when this happens if the media is removable.

To keep the size of changesets sane we split the switch by drivers;
to keep the damn thing bisectable we do the following:
1) rename the affected methods, add ones with correct
prototypes, make (few) callers handle both. That's this changeset.
2) for each driver convert to new methods. *ALL* drivers
are converted in this series.
3) kill the old (renamed) methods.
Note that it _is_ a flagday; all in-tree drivers are converted and by the
end of this series no trace of old methods remain. The only reason why
we do that this way is to keep the damn thing bisectable and allow per-driver
debugging if anything goes wrong.

This merges branches irq/genirq, irq/sparseirq-v4, timers/hpet-percpu
and x86/uv.
The sparseirq branch is just preliminary groundwork: no sparse IRQs are
actually implemented by this tree anymore - just the new APIs are added
while keeping the old way intact as well (the new APIs map 1:1 to
irq_desc[]). The 'real' sparse IRQ support will then be a relatively
small patch ontop of this - with a v2.6.29 merge target.

The driver flag dynids.use_driver_data is almost consistently not set,
and causes more problems than it solves. It was initially intended as a
flag to indicate whether a driver's usage of driver_data had been
carefully inspected and was ready for values from userspace. That audit
was never done, so most drivers just get a 0 for driver_data when new
IDs are added from userspace via sysfs. So remove the flag, allowing
drivers to see the data directly (a followon patch validates the passed
driver_data value against what the drivers expect).

Tejun's commit 7b595756ec made sysfs
attribute->owner unnecessary. But the field was left in the structure to
ease the merge. It's been over a year since that change and it is now
time to start killing attribute->owner along with its users - one arch at
a time!
This patch is attempt #1 to get rid of attribute->owner only for
CONFIG_X86_64 or CONFIG_X86_32 . We will deal with other arches later on
as and when possible - avr32 will be the next since that is something I
can test. Compile (make allyesconfig / make allmodconfig / custom config)
and boot tested.
akpm: the idea is that we put the declaration of sttribute.owner inside
`#ifndef CONFIG_X86'. But that proved to be too ambitious for now because
new usages kept on turning up in subsystem trees.

We must be using the bh spin locking functions in
iscsi_eh_device_reset becuase the session lock interacts with
a thread and softirq.
This patch also fixes up a bogus comment and check in fail_command,
because no one drops the lock (bnx2i did but it is not going
upstream yet and there were other refcount changes for that).

I had this in my patchset to add target reset support, but
it got dropped due to patching conflicts. This initial patch
just renames the function and users. We are actually just
dropping the session, and so this does not have anything to do
with the host exactly. It does for software iscsi because
we allocate a host per session, but for cxgb3i this makes no
sense.

If the driver knows when hardware is removed like with cxgb3i,
bnx2i, qla4xxx and iser then we will want to remove the sessions/devices
that are bound to that device before removing the host.
cxgb3i and in the future bnx2i will remove the host and that will
remove all the sessions on the hba. iser can call iscsi_kill_session
when it gets an event that indicates that a hca is removed.
And when qla4xxx is hooked in to the lib (it is only hooked into
the class right now) it can call iscsi remove host like the
partial offload card drivers.

iscsi_tcp was updating the exp_statsn (exp_statsn acknowledges
status and tells the target it is ok to let the resources for
a iscsi pdu to be reused) before it got all the data for pdu read
into OS buffers. Data corruption was occuring if something happens
to a packet and the network layer requests a retransmit, and the
initiator has told the target about the udpated exp_statsn ack,
then the target may be sending data from a buffer it has reused
for a new iscsi pdu. This fixes the problem by having the LLD
(iscsi_tcp in this case) just handle the transferring of data, and
has libiscsi handle the processing of status (libiscsi completion
processing is done after LLD data transfers are complete).

Now that we no longer use protection_type as trigger for preparing
protected CDBs we can remove the places that set it to zero. This
allows userland to see which protection type the device is formatted
with regardless of whether the HBA supports DIF or not.

There's a target reset bug.
This loop:
for (id = 0; id <= shost->max_id; id++) {
Never terminates if shost->max_id is set to ~0, like aic94xx does.
It's also pretty inefficient since you mostly have compact target
numbers, but the max_id can be very high. The best way would be to
sort the recovery list by target id and skip them if they're equal,
but even a worst case O(N^2) traversal is probably OK here, so fix it
by finding the next highest target number (assuming n+1) and
terminating when there isn't one.

We do not need to set REQ_NOMERGE because when the module calls
blk_execute_rq -> blk_execute_rq_nowait, blk_execute_rq_nowait sets
it for us. This brings all the modules in sync for those bits.

This checks the errors the scsi-ml determined were retryable
and returns if we should fast fail it based on the request
fail fast flags.
Without the patch, drivers like lpfc, qla2xxx and fcoe would return
DID_ERROR for what it determines is a temporary communication problem.
There is no loss of connectivity at that time and the driver thinks
that it would be fast to retry at the driver level. SCSI-ml will however
sees fast fail on the request and DID_ERROR and will fast fail the io.
This will then cause dm-multipath to fail the path and possibley switch
target controllers when we should be retrying at the scsi layer.
We also were fast failing device errors to dm multiapth when
unless the scsi_dh modules think otherwis we want to retry at
the scsi layer because multipath can only retry the IO like scsi
should have done. multipath is a little dumber though because it
does not what the error was for and assumes that it should fail
the paths.

Multipath is best at handling transport errors. If it gets a device
error then there is not much the multipath layer can do. It will just
access the same device but from a different path.
This patch breaks up failfast into device, transport and driver errors.
The multipath layers (md and dm mutlipath) only ask the lower levels to
fast fail transport errors. The user of failfast, read ahead, will ask
to fast fail on all errors.
Note that blk_noretry_request will return true if any failfast bit
is set. This allows drivers that do not support the multipath failfast
bits to continue to fail on any failfast error like before. Drivers
like scsi that are able to fail fast specific errors can check
for the specific fail fast type. In the next patch I will convert
scsi.

This has qla2xxx use the new transport error values instead of
DID_BUS_BUSY. I am not sure if all the errors
in qla_isr.c I changed are transport related. We end up blocking/deleting
the rport for all of them so it is better to use the new transport error since
the fc classs will decide when to fail the IO.
With this patch if I pull a cable then IO that had reached
the driver, will be failed with DID_TRANSPORT_DISRUPTED (not including
tape). The fc class will then fail the IO when the fast io fail tmo
has fired, and the driver will flush any other commands running.

Currently, if there is a transport problem the iscsi drivers will return
outstanding commands (commands being exeucted by the driver/fw/hw) with
DID_BUS_BUSY and block the session so no new commands can be queued.
Commands that are caught between the failure handling and blocking are
failed with DID_IMM_RETRY or one of the scsi ml queuecommand return values.
When the recovery_timeout fires, the iscsi drivers then fail IO with
DID_NO_CONNECT.
For fcp, some drivers will fail some outstanding IO (disk but possibly not
tape) with DID_BUS_BUSY or DID_ERROR or some other value that causes a retry
and hits the scsi_error.c failfast check, block the rport, and commands
caught in the race are failed with DID_IMM_RETRY. Other drivers, may
hold onto all IO and wait for the terminate_rport_io or dev_loss_tmo_callbk
to be called.
The following patches attempt to unify what upper layers will see drivers
like multipath can make a good guess. This relies on drivers being
hooked into their transport class.
This first patch just defines two new host byte errors so drivers can
return the same value for when a rport/session is blocked and for
when the fast_io_fail_tmo fires.
The idea is that if the LLD/class detects a problem and is going to block
a rport/session, then if the LLD wants or must return the command to scsi-ml,
then it can return it with DID_TRANSPORT_DISRUPTED. This will requeue
the IO into the same scsi queue it came from, until the fast io fail timer
fires and the class decides what to do.
When using multipath and the fast_io_fail_tmo fires then the class
can fail commands with DID_TRANSPORT_FAILFAST or drivers can use
DID_TRANSPORT_FAILFAST in their terminate_rport_io callbacks or
the equivlent in iscsi if we ever implement more advanced recovery methods.
A LLD, like lpfc, could continue to return DID_ERROR and then it will hit
the normal failfast path, so drivers do not have fully be ported to
work better. The point of the patches is that upper layers will
not see a failure that could be recovered from while the rport/session is
blocked until fast_io_fail_tmo/recovery_timeout fires.

When we block a rport and the driver implements the terminate
callback we will fail IO that was running quickly. However
IO that was in the scsi_device/block queue sits there until
the dev_loss_tmo fires, and this can make it look like IO is
lost because new IO will get executed but that IO stuck in
the blocked queue sits there for some time longer.
With this patch when the fast io fail tmo fires, we will
fail the blocked IO and any new IO. This patch also allows
all drivers to partially support the fast io fail tmo. If the
terminate io callback is not implemented, we will still fail blocked
IO and any new IO, so multipath can handle that.
This patch also allows the fc and iscsi classes to implement the
same behavior. The timers are just unfornately named differently.
This patch also fixes the problem where drivers were unblocking
the target in their terminate callback, which was needed for
rport removal, but for fast io fail timeout it would cause
IO to bounce arround the scsi/block layer and the LLD queuecommand.
And it for drivers that could have IO stuck but did not have
a terminate callback the unblock calls in the class will fix
them.

If the fcport is not online then we do not want to block IO to all ports on
the host. We just want to stop IO on port not online, so we should be using
the SCSI_MLQUEUE_TARGET_BUSY return value.
For the case where we race with the rport memset initialization
we do not want the queuecommand to be called again so we can just use
SCSI_MLQUEUE_TARGET_BUSY for this.

When qla4xxx begins recovery and the iscsi class is firing up to handle
it, we need to retrn SCSI_MLQUEUE_TARGET_BUSY from the driver instead
of host busy, because the session recovery only affects the one target.

SCSI-ml manages the queueing limits for the device and host, but
does not do so at the target level. However something something similar
can come in userful when a driver is transitioning a transport object to
the the blocked state, becuase at that time we do not want to queue
io and we do not want the queuecommand to be called again.
The patch adds code similar to the exisiting SCSI_ML_*BUSY handlers.
You can now return SCSI_MLQUEUE_TARGET_BUSY when we hit
a transport level queueing issue like the hw cannot allocate some
resource at the iscsi session/connection level, or the target has temporarily
closed or shrunk the queueing window, or if we are transitioning
to the blocked state.
bnx2i, when they rework their firmware according to netdev
developers requests, will also need to be able to limit queueing at this
level. bnx2i will hook into libiscsi, but will allocate a scsi host per
netdevice/hba, so unlike pure software iscsi/iser which is allocating
a host per session, it cannot set the scsi_host->can_queue and return
SCSI_MLQUEUE_HOST_BUSY to reflect queueing limits on the transport.
The iscsi class/driver can also set a scsi_target->can_queue value which
reflects the max commands the driver/class can support. For iscsi this
reflects the number of commands we can support for each session due to
session/connection hw limits, driver limits, and to also reflect the
session/targets's queueing window.

Use a special request for serialisation purposes and get rid of the
awkward ide_spin_wait_hwgroup(). This also involves converting the
ide_devset structure so it can be shared by the /proc and the ioctl code.

Make drive->id an unnamed union so id can be accessed either by using
'u16 *id' or 'struct hd_driveid *driveid'. Then convert all existing
drive->id users accordingly (using 'u16 *id' when possible).
This is an intermediate step to make ide 'struct hd_driveid'-free.
While at it:
- Add missing KERN_CONTs in it821x.c.
- Use ATA_ID_WORDS and ATA_ID_*_LEN defines.
- Remove unnecessary checks for drive->id.
- s/drive_table/table/ in ide_in_drive_list().

__sg_start_req() was used temporarily to call blk_get_request() during
converting sg to use the block layer.
Now sg always calls blk_get_request() so we can move blk_get_request()
to sg_start_req(). We don't need __sg_start_req anymore.

sg had lots of the own functions for the direct IO but now sg uses the
block layer functions for it. There are only five lines for the direct
IO. SG_ALLOW_DIO_CODE define was used to compile out the direct IO
code but we don't need the define. If someone wants to remove the
direct IO code, he can do easily without the define.

old sg_rq_end_io() was used to wrap sg_cmd_done during converting sg
to use the block layer (in order to cover the difference
scsi_execute_async and blk_execute_rq_nowait). Now we don't need it so
let's remove it.

Right now SCSI and others do their own command timeout handling.
Move those bits to the block layer.
Instead of having a timer per command, we try to be a bit more clever
and simply have one per-queue. This avoids the overhead of having to
tear down and setup a timer for each command, so it will result in a lot
less timer fiddling.

Modify the SCSI disk driver to call the revalidate_disk()
wrapper. This allows us to do some housekeeping such as accounting for
a disk being resized online. The wrapper will call
sd_revalidate_disk() at the appropriate time.

With the older SG interface, we don't know a user-space address to
trasfer data when executing a SCSI command. So we can't pass a
user-space address to blk_rq_map_user.
This patch fixes sg to pass a NULL user-space address to
blk_rq_map_user so that it just sets up a request and bios with page
frames propely without data transfer.

This patch introduces struct rq_map_data to enable bio_copy_use_iov()
use reserved pages.
Currently, bio_copy_user_iov allocates bounce pages but
drivers/scsi/sg.c wants to allocate pages by itself and use
them. struct rq_map_data can be used to pass allocated pages to
bio_copy_user_iov.
The current users of bio_copy_user_iov simply passes NULL (they don't
want to use pre-allocated pages).

Currently, blk_rq_map_user and blk_rq_map_user_iov always do
GFP_KERNEL allocation.
This adds gfp_mask argument to blk_rq_map_user and blk_rq_map_user_iov
so sg can use it (sg always does GFP_ATOMIC allocation).

Now that disk and partition handlings are mostly unified, it's easy to
allow disk to have extended device number. This patch makes
add_disk() use extended device number if disk->minors is zero. Both
sd and ide-disk are updated to use this.
* sd_format_disk_name() is implemented which can generically determine
the drive name. This removes disk number restriction stemming from
limited device names.
* If sd index goes over SD_MAX_DISKS (which can be increased now BTW),
sd simply doesn't initialize minors letting block layer choose
extended device number.
* If CONFIG_DEBUG_EXT_DEVT is set, both sd and ide-disk always set
minors to 0 and use extended device numbers.

With previous changes, it's meaningless to limit the number of
partitions. Replace @ext_minors with GENHD_FL_EXT_DEVT such that
setting the flag allows the disk to have maximum number of allowed
partitions (only limited by the number of entries in parsed_partitions
as determined by MAX_PART constant).
This kills not-too-pretty alloc_disk_ext[_node]() functions and makes
@minors parameter to alloc_disk[_node]() unnecessary. The parameter
is left alone to avoid disturbing the users.

Extended devt introduces non-contiguos device numbers. This patch
implements a debug option which forces most devt allocations to be
from the extended area and spreads them out. This is enabled by
default if DEBUG_KERNEL is set and achieves...
1. Detects code paths in kernel or userland which expect predetermined
consecutive device numbers.
2. When something goes wrong, avoid corruption as adding to the minor
of earlier partition won't lead to the wrong but valid device.

Update sd and ide-disk such that they can take advantage of extended
minors.
ide-disk already has 64 minors per device and currently doesn't use
extended minors although after this patch it can be turned on by
simply tweaking constants.
sd only had 16 minors per device causing problems on certain peculiar
configurations. This patch lifts the restriction and enables it to
use upto 64 minors.

* Implement disk_devt() and part_devt() and use them to directly
access devt instead of computing it from ->major and ->first_minor.
Note that all references to ->major and ->first_minor outside of
block layer is used to determine devt of the disk (the part0) and as
->major and ->first_minor will continue to represent devt for the
disk, converting these users aren't strictly necessary. However,
convert them for consistency.
* Implement disk_max_parts() to avoid directly deferencing
genhd->minors.
* Update bdget_disk() such that it doesn't assume consecutive minor
space.
* Move devt computation from register_disk() to add_disk() and make it
the only one (all other usages use the initially determined value).
These changes clean up the code and will help disk->part dereference
fix and extended block device numbers.

The Flash Layout Table (FLT) present on many recent HBAs encodes
flash usage information, organizes data stored into separate
regions and presents the information uniformly to the driver.
Use this information rather than using specific hard-coded values
based on ISP type.

Currently Firmware does not have counters for input megabytes and
output megabytes, therefore driver counts these values depending
on the status of the scsi command and direction of the command.
The values are exported in the FC_HOST path.

Due to patch building error on my side, we are still passing DID_BUS_BUSY
for commands that are running, when we want to return whatever the caller
of fail_all_commands wanted. This replaces the hardcoded error code with
the value that is passed in.

There's already a fc_vport_termintate() call exported by
the transport. This patch adds a symmetric call to the API to allow
an NPIV-capable LLD to instantiate vports sans user intervention.

Brian King <brking@linux.vnet.ibm.com> reported that fibre channel
devices can oops during scanning if their ports block (because the
device goes from CREATED -> BLOCK -> RUNNING rather than CREATED ->
BLOCK -> CREATED).
Fix this by adding a new state: CREATED_BLOCK which can only transition
back to CREATED and disallow the CREATED -> BLOCK transition. Now both
the created and blocked states that the mid-layer recognises can include
CREATED_BLOCK.

This patch adds scsi netlink recieve and event support for transport
and scsi LLDD's. It is a reimplementation of the patch posted last
week by David Somayajulu.

There are a few things done differently:
- Transport support is included
- Event delivery is included
- The vendor message is now its own unique message type, considered
part of the generic "SCSI Transport".
- LLDD entry points are now registered rather than included in the
scsi_host_template.
Background: When I started to implement the event handler via template,
I had to either: muck up scsi_add_host and scsi_remove_host; or have
the event handler search all possible shosts. Neither was acceptable.
Moving to a registration solves this, and also limits the scope of
the changes to something that could be backported to a distro without
breaking an already-released-distro kabi. However, I admit it isn't
as elegant, as the passing of the LLDD host template in the
registration and the complexity around dynamic add/remove shows.
- The receive path was augmented to require a unique identifier for
the LLDD before the message was allowed to be handed off to the
driver. Given how quickly very fatal errors occur if there's msg
mismatches (which I saw in testing my own tools :), I believe this
to be a very good thing. The id plays off the vendor id scheme already
introduced for the vendor unique event messages used by FC.
Additionally, the id use as the basis of the registration/deregistration.
- Send assist functions, for both the transport and LLDDs are included.

This patch cleans up the behavior of scsi_host_lookup().
The original implementation attempted to use the dual role of
either returning a pointer value, or a negative error code.
User's needed to use IS_ERR() to check the result. Additionally,
the IS_ERR() macro never checks for when a NULL pointer was
returned, so a NULL pointer actually passes with a success case.
Note: scsi_host_get(), used by scsi_host_lookup(), can return
a NULL pointer.
Talk about a mudhole for the unitiated to step into....
This patch converts scsi_host_lookup() to return either NULL
or a valid pointer. The consumers were updated for the change.

Logically, SCR access ops should take @link; however, there was no
compelling reason to convert all SCR access ops when adding @link
abstraction as there's one-to-one mapping between a port and a non-PMP
link. However, that assumption won't hold anymore with the scheduled
addition of slave link.
Make SCR access ops per-link.

The current sg list traversal logic for the continuation entries
doesn't advance the list pointer once all seven slots are used, so the
next continuation entry (if there is one) wrongly begins again at the
start of the sg list.
Fix by advancing the sg pointer after the for_each_sg().

Sometimes, particularly for USB devices with the last sector bug,
requests get completed in chunks. There's a bug in this in that if
one of the chunks gets an error, we complete that chunk with an error
but never move on to the remaining ones, leading to the request
hanging (because it's not fully completed).
Fix this by completing all remaining chunks if an error is encountered.

We still have life time issues with the sysfs command filter kobject,
so disable it for 2.6.27 release. We can revisit this and make it work
properly for 2.6.28, for 2.6.27 release it's too risky.

For IBM z series certain LUNs can no longer be accessed.
This is because kernel version 2.6.19 a check was introduced not to
create a generic SCSI device for devices that return PQ=1 and
PDT=0x1f. For WLUNs (see SAM-3, p. 41ff) generic SCSI devices should
be created unconditionally without looking at the PQ bit, so add a
check for WLUNs in with this test.

This patch remove blk_register_filter and blk_unregister_filter in
gendisk, and adds them to sd.c, sr.c. and ide-cd.c
The commit abf5439370 moved cmdfilter
from gendisk to request_queue. It turned out that in some subsystems
multiple gendisks share a single request_queue. So we get:

This thing was completely pointless.
Just find the OF device in the parent of drivers that want to program
this device, and map the DMA regs inside such drivers too.
This also moves the dummy claim_dma_lock() and release_dma_lock()
implementation to floppy_32.h, which makes it handle this issue
just like floppy_64.h does.

sg allowed any command for TYPE_SCANNER. The cmd_filter patchset
doesn't. We can't change sg's permission since it might break the
existing software.

cmd_filter works only for the block layer SG_IO with SCSI block
devices. It breaks scsi/sg.c, bsg, and the block layer SG_IO with SCSI
character devices (such as st). We hit a kernel crash with them.
The problem is that cmd_filter code accesses to gendisk (having struct
blk_scsi_cmd_filter) via inode->i_bdev->bd_disk. It works for only
SCSI block device files. With character device files, inode->i_bdev
leads you to struct cdev. inode->i_bdev->bd_disk->blk_scsi_cmd_filter
isn't safe.
SCSI ULDs don't expose gendisk; they keep it private. bsg needs to be
independent on any protocols. We shouldn't change ULDs to expose their
gendisk.
This patch moves struct blk_scsi_cmd_filter from gendisk to
request_queue, a common object, which eveyone can access to.
The user interface doesn't change; users can change the filters via
/sys/block/. gendisk has a pointer to request_queue so the cmd_filter
code accesses to struct blk_scsi_cmd_filter.

If the client virtual fibre channel adapter is already logged into the
server and does an NPIV Login again, the async queue, which is used for
reporting Link Up/Link Down type of events, does not get reset on the
server side. Fix up the client driver so that we also do not reset it.
This fixes a problem of lost async events following relogins.

If an ELS is received while the virtual fibre channel adapter is going
through its discovery, a flag is set which causes discovery to get
re-driven. However, the hosts's state does not get set back to
IBMVFC_INITIALIZING and scsi_block_requests does not get called again,
which can result in queuecommand ops getting sent during
discovery. This should not occur and may cause problems. One example
is that we may no longer be logged into the target we send the command
to, resulting in a failure which should not have occurred.

This fixes a hang on module removal. The module removal code was setting
the hosts's state to IBMVFC_HOST_OFFLINE before tearing down the kernel
thread, but, due to a bug in ibmvfc_wait_while_resetting, was not waiting
for the kernel thread's offlining work to be done prior to destroying
the kernel thread, which left the scsi host in a blocked state which we
never got out of.

When running ibmvscsi in a shared memory partition, it must provide
a default value for the amount of DMA resources it will need in order to
perform reasonably well. This was being calculated in sectors rather than
bytes, as it should. This patch fixes this.

/sys/bus/pci/drivers/megaraid_sas/dbg_lvl defaults to being
world-writable, which seems bad (letting any user affect kernel driver
behavior and logging level).
This turns off group and user write permissions, so that on typical
production systems only root can write to it.

During internal testing, we've seen issues (hangs) with the
'deferred' vport tear-down-processing typically accompanied with
the fc_remove_host() call. This is due to the current
implementation's back-end vport handling being performed by the
physical-HA's DPC thread where premature shutdown could lead to
latent vport requests without a processor.
This should also address a problem reported by Gal Rosen
(http://marc.info/?l=linux-scsi&m=121731664417358&w=2) where the
driver would attempt to awaken a previously torn-down DPC thread
from interrupt context by implicitly calling wake_up_process()
rather than the driver's qla2xxx_wake_dpc() helper. Rather, than
reshuffle the remove_one() device-removal code, during unload,
depend on the driver's timer to wake-up the DPC process, by
limiting wake-ups based on an 'unloading' flag.
