65ed76010d hugetlbfs: lockdep annotate root inode properly
This fixes the below reported false lockdep warning.  e096d0c7e2
("lockdep: Add helper function for dir vs file i_mutex annotation") added
a similar annotation for every other inode in hugetlbfs but missed the
root inode because it was allocated by a separate function.

For HugeTLB fs we allow taking i_mutex in mmap.  HugeTLB fs doesn't
support file write and its file read callback is modified in a05b0855fd
("hugetlbfs: avoid taking i_mutex from hugetlbfs_read()") to not take
i_mutex.  Hence for HugeTLB fs with regular files we really don't take
i_mutex with mmap_sem held.

61065a30af fs/buffer.c: remove BUG() in possible but rare condition
While stressing the kernel with with failing allocations today, I hit the
following chain of events:

alloc_page_buffers():

	bh = alloc_buffer_head(GFP_NOFS);
	if (!bh)
		goto no_grow; <= path taken

grow_dev_page():
        bh = alloc_page_buffers(page, size, 0);
        if (!bh)
                goto failed;  <= taken, consequence of the above

and then the failed path BUG()s the kernel.

The failure is inserted a litte bit artificially, but even then, I see no
reason why it should be deemed impossible in a real box.

Even though this is not a condition that we expect to see around every
time, failed allocations are expected to be handled, and BUG() sounds just
too much.  As a matter of fact, grow_dev_page() can return NULL just fine
in other circumstances, so I propose we just remove it, then.

13d518074a epoll: clear the tfile_check_list on -ELOOP
An epoll_ctl(,EPOLL_CTL_ADD,,) operation can return '-ELOOP' to prevent
circular epoll dependencies from being created.  However, in that case we
do not properly clear the 'tfile_check_list'.  Thus, add a call to
clear_tfile_check_list() for the -ELOOP case.

4306629e1c GFS2: Remove unused argument from gfs2_internal_read
gfs2_internal_read accepts an unused ra_state argument, left over from
when we did readahead on the rindex. Since there are currently no plans
to add back this readahead, this patch removes the ra_state parameter
and updates the functions which call gfs2_internal_read accordingly.

c50b91c4bd GFS2: Remove bd_list_tr
This is another clean up in the logging code. This per-transaction
list was largely unused. Its main function was to ensure that the
number of buffers in a transaction was correct, however that counter
was only used to check the number of buffers in the bd_list_tr, plus
an assert at the end of each transaction. With the assert now changed
to use the calculated buffer counts, we can remove both bd_list_tr and
its associated counter.

This should make the code easier to understand as well as shrinking
a couple of structures.

dad30e9031 GFS2: Remove duplicate log code
The main part of this patch merges the two functions used to
write metadata and data buffers to the log. Most of the code
is common between the two functions, so this provides a nice
clean up, and makes the code more readable.

The gfs2_get_log_desc() function is also extended to take two more
arguments, and thus avoid having to set the length and data1
fields of this strucuture as a separate operation.

e8c92ed769 GFS2: Clean up log write code path
Prior to this patch, we have two ways of sending i/o to the log.
One of those is used when we need to allocate both the data
to be written itself and also a buffer head to submit it. This
is done via sb_getblk and friends. This is used mostly for writing
log headers.

The other method is used when writing blocks which have some
in-place counterpart. This is the case for all the metadata
blocks which are journalled, and when journaled data is in use,
for unescaped journalled data blocks.

This patch replaces both of those two methods, and about half
a dozen separate i/o submission points with a single i/o
submission function. We also go direct to bio rather than
using buffer heads, since this allows us to build i/o
requests of the maximum size for the block device in
question. It also reduces the memory required for flushing
the log, which can be very useful in low memory situations.

2f7ee358e5 GFS2: Use variable rather than qa to determine if unstuff necessary
In the future, the qadata structure will be eliminated and merged
back in with the block reservation structure, after we extend the
lifespan of that. This patch is a step forward in eliminating the
qadata structure. It adds a variable to the do_grow function to
determine when unstuffing is necessary, and has been done.

9598d25ed9 GFS2: Change variable blk to biblk
In the resource group code, we have no less than three different
kinds of block references: block relative to the file system (u64),
block relative to the rgrp (u32), and block relative to the bitmap.
This is a small step to making the code more readable; it renames
variable blk to biblk to solidify in my mind that it's relative to
the bitmap and nothing else.

4c569a72c3 GFS2: Instruct DLM to avoid queue convert slowdown
This patch instructs DLM to prevent an "in place" conversion, where the
lock just stays on the granted queue, and instead forces the conversion to
the back of the convert queue. This is done on upward conversions only.
    
This is useful in cases where, for example, a lock is frequently needed in
PR on one node, but another node needs it temporarily in EX to update it.
This may happen, for example, when the rindex is being updated by gfs2_grow.
The gfs2_grow needs to have the lock in EX, but the other nodes need to
re-read it to retrieve the updates. The glock is already granted in PR on
the non-growing nodes, so this prevents them from continually re-granting
the lock in PR, and forces the EX from gfs2_grow to go through.

99aa784667 jbd2: use GFP_NOFS for blkdev_issue_flush
flush request is issued in transaction commit code path, so looks using
GFP_KERNEL to allocate memory for flush request bio falls into the classic
deadlock issue.  I saw btrfs and dm get it right, but ext4, xfs and md are
using GFP.

4a17fd5229 sock: Introduce named constants for sk_reuse
Name them in a "backward compatible" manner, i.e. reuse or not
are still 1 and 0 respectively. The reuse value of 2 means that
the socket with it will forcibly reuse everyone else's port.

936af1576e aio: don't bother with unmapping when aio_free_ring() is coming from exit_aio()
... since exit_mmap() is coming and it will munmap() everything anyway.
In all other cases aio_free_ring() has ctx->mm == current->mm; moreover,
all other callers of vm_munmap() have mm == current->mm, so this will
allow us to get rid of mm argument of vm_munmap().

95b72eb0bd NFSv4: Ensure we do not reuse open owner names
The NFSv4 spec is ambiguous about whether or not it is permissible
to reuse open owner names, so play it safe. This patch adds a timestamp
to the state_owner structure, and combines that with the IDA based
uniquifier.

6be5ceb02e VM: add "vm_mmap()" helper function
This continues the theme started with vm_brk() and vm_munmap():
vm_mmap() does the same thing as do_mmap(), but additionally does the
required VM locking.

This uninlines (and rewrites it to be clearer) do_mmap(), which sadly
duplicates it in mm/mmap.c and mm/nommu.c.  But that way we don't have
to export our internal do_mmap_pgoff() function.

Some day we hopefully don't have to export do_mmap() either, if all
modular users can become the simpler vm_mmap() instead.  We're actually
very close to that already, with the notable exception of the (broken)
use in i810, and a couple of stragglers in binfmt_elf.

98a2139f4f nfs: Enclose hostname in brackets when needed in nfs_do_root_mount
When hostname contains colon (e.g. when it is an IPv6 address) it needs
to be enclosed in brackets to make parsing of NFS device string possible.
Fix nfs_do_root_mount() to enclose hostname properly when needed. NFS code
actually does not need this as it does not parse the string passed by
nfs_do_root_mount() but the device string is exposed to userspace in
/proc/mounts.

3af9d8f227 cifs: fix offset handling in cifs_iovec_write
In the recent update of the cifs_iovec_write code to use async writes,
the handling of the file position was broken. That patch added a local
"offset" variable to handle the offset, and then only updated the
original "*poffset" before exiting.

Unfortunately, it copied off the original offset from the beginning,
instead of doing so after generic_write_checks had been called. Fix
this by moving the initialization of "offset" after that in the
function.

25cd999e1a Btrfs: fix that check_int_data mount option was ignored
The bitfield member mount_opt was too small by one bit to hold the mount
option that enabled to include data extents in the integrity checker.
Since the same issue happened when the BTRFS_MOUNT_PANIC_ON_FATAL_ERROR
option was added (git rebase silently merges so that the increase of the
size of the bitfield member is lost), the bit limit was removed entirely.

b9688bb845 btrfs: don't return EINTR
It is basically a good thing if we are interruptible when waiting for
free space, but the generality in which it is implemented currently
leads to system calls being interruptible that are not documented this
way. For example git can't handle interrupted unlink(), leading to
corrupt repos under space pressure.
Instead we raise the bar to only be interruptible by SIGKILL.

5cf1ab5613 Btrfs: always store the mirror we read the eb from
A user reported a panic where we were trying to fix a bad mirror but the
mirror number we were giving was 0, which is invalid.  This is because we
don't do the transid verification until after the read, so as far as the
read code is concerned the read was a success.  So instead store the mirror
we read from so that if there is some failure post read we know which mirror
to try next and which mirror needs to be fixed if we find a good copy of the

4735fb2828 Btrfs: Make free_ipath() deal gracefully with NULL pointers
Make free_ipath() behave like most other freeing functions in the
kernel and gracefully do nothing when passed a NULL pointer.

Besides this making the bahaviour consistent with functions such as
kfree(), vfree(), btrfs_free_path() etc etc, it also fixes a real NULL
deref issue in fs/btrfs/ioctl.c::btrfs_ioctl_ino_to_path(). In that
function we have this code:

        ipath = init_ipath(size, root, path);
        if (IS_ERR(ipath)) {
                ret = PTR_ERR(ipath);
                ipath = NULL;

        }

        btrfs_free_path(path);

If we ever take the true branch of that 'if' statement we'll end up
passing a NULL pointer to free_ipath() which will subsequently
dereference it and we'll go "Boom" :-(
This patch will avoid that.

This way we can better utilize multiple devices.

259e5e6c75 Add PR_{GET,SET}_NO_NEW_PRIVS to prevent execve from granting privs
With this change, calling
  prctl(PR_SET_NO_NEW_PRIVS, 1, 0, 0, 0)
disables privilege granting operations at execve-time.  For example, a
process will not be able to execute a setuid binary to change their uid
or gid if this bit is set.  The same is true for file capabilities.

Additionally, LSM_UNSAFE_NO_NEW_PRIVS is defined to ensure that
LSMs respect the requested behavior.

To determine if the NO_NEW_PRIVS bit is set, a task may call
  prctl(PR_GET_NO_NEW_PRIVS, 0, 0, 0, 0);
It returns 1 if set and 0 if it is not set. If any of the arguments are
non-zero, it will return -1 and set errno to -EINVAL.
(PR_SET_NO_NEW_PRIVS behaves similarly.)

This functionality is desired for the proposed seccomp filter patch
series.  By using PR_SET_NO_NEW_PRIVS, it allows a task to modify the
system call behavior for itself and its child tasks without being
able to impact the behavior of a more privileged task.

Another potential use is making certain privileged operations
unprivileged.  For example, chroot may be considered "safe" if it cannot
affect privileged tasks.

Note, this patch causes execve to fail when PR_SET_NO_NEW_PRIVS is
set and AppArmor is in use.  It is fixed in a subsequent patch.

bfa890a3cd Fix number parsing in cifs_parse_mount_options
The function kstrtoul() used to parse number strings in the mount
option parser is set to expect a base 10 number . This treats the octal
numbers passed for mount options such as file_mode as base10 numbers
leading to incorrect behavior.

Change the 'base' argument passed to kstrtoul from 10 to 0 to
allow it to auto-detect the base of the number passed.

d53ba47484 Btrfs: use commit root when loading free space cache
A user reported that booting his box up with btrfs root on 3.4 was way
slower than on 3.3 because I removed the ideal caching code.  It turns out
that we don't load the free space cache if we're in a commit for deadlock
reasons, but since we're reading the cache and it hasn't changed yet we are
safe reading the inode and free space item from the commit root, so do that
and remove all of the deadlock checks so we don't unnecessarily skip loading
the free space cache.  The user reported this fixed the slowness.  Thanks,

c6664b42c4 Btrfs: remove lock assert from get_restripe_target()
This fixes a regression introduced by fc67c450.  spin_is_locked() always
returns 0 on UP kernels, which caused assert in get_restripe_target() to
be fired on every call from btrfs_reduce_alloc_profile() on UP systems.
Remove it completely for now, it's not clear if it's going to be needed
in future.

9dc4e6c4d1 nfsd: don't fail unchecked creates of non-special files
Allow a v3 unchecked open of a non-regular file succeed as if it were a
lookup; typically a client in such a case will want to fall back on a
local open, so succeeding and giving it the filehandle is more useful
than failing with nfserr_exist, which makes it appear that nothing at
all exists by that name.

Similarly for v4, on an open-create, return the same errors we would on
an attempt to open a non-regular file, instead of returning
nfserr_exist.

This fixes a problem found doing a v4 open of a symlink with
O_RDONLY|O_CREAT, which resulted in the current client returning EEXIST.

child is added have been observed.

1/ scsi is performing async scans and the device is removed prior to the

ca9248d833 GFS2: Allow caching of rindex glock
This patch allows caching of the rindex glock. We were previously
setting the GL_NOCACHE bit when the glock was released. That forced
the rindex inode to be invalidated, which caused us to re-read
rindex at the next access. However, it caused the glock to be
unnecessarily bounced around the cluster. This patch allows
the glock to remain cached, but it still causes the rindex to be
re-read once it has been written to by gfs2_grow.

Ben and I have tested single-node gfs2_grow cases and I've tested
clustered gfs2_grow cases on my four-node cluster.

640946f203 dentry leak in simple_fill_super() failure exit
d_genocide() does _not_ evict dentries; it just removes extra ref
pinning each of those.  Normally it's followed by shrinking the
tree (it's done just before generic_shutdown_super() by kill_litter_super()),
but in case of simple_fill_super() nothing of that kind will follow.
Just do shrink_dcache_parent() manually.

1a48e2ac03 userns: Replace the hard to write inode_userns with inode_capable.
This represents a change in strategy of how to handle user namespaces.
Instead of tagging everything explicitly with a user namespace and bulking
up all of the comparisons of uids and gids in the kernel,  all uids and gids
in use will have a mapping to a flat kuid and kgid spaces respectively.  This
allows much more of the existing logic to be preserved and in general
allows for faster code.

In this new and improved world we allow someone to utiliize capabilities
over an inode if the inodes owner mapps into the capabilities holders user
namespace and the user has capabilities in their user namespace.  Which
is simple and efficient.

Moving the fs uid comparisons to be comparisons in a flat kuid space
follows in later patches, something that is only significant if you
are using user namespaces.

f68e556e23 Make the "word-at-a-time" helper functions more commonly usable
I have a new optimized x86 "strncpy_from_user()" that will use these
same helper functions for all the same reasons the name lookup code uses
them.  This is preparation for that.

This moves them into an architecture-specific header file.  It's
architecture-specific for two reasons:

 - some of the functions are likely to want architecture-specific
   implementations.  Even if the current code happens to be "generic" in
   the sense that it should work on any little-endian machine, it's
   likely that the "multiply by a big constant and shift" implementation
   is less than optimal for an architecture that has a guaranteed fast
   bit count instruction, for example.

 - I expect that if architectures like sparc want to start playing
   around with this, we'll need to abstract out a few more details (in
   particular the actual unaligned accesses).  So we're likely to have
   more architecture-specific stuff if non-x86 architectures start using
   this.

   (and if it turns out that non-x86 architectures don't start using
   this, then having it in an architecture-specific header is still the
   right thing to do, of course)

    Otherwise what goes out of TCP is spaghetti and doesn't batch
    effectively into GSO/TSO clusters.

12) Once we put a SKB into either the netlink receiver's queue or a
    socket error queue, it can be consumed and freed up, therefore we
    cannot touch it after queueing it like that.

13) PPP has this annoying behavior in that for every transmit call it
    immediately stops the TX queue, then calls down into the next layer
    to transmit the PPP frame.

    But if that next layer can take it immediately, it just un-stops the
    TX queue right before returning from the transmit method.

    Besides being useless work, it makes several facilities unusable, in
    particular things like the equalizers.  Well behaved devices should
    only stop the TX queue when they really are full, and in PPP's case
    when it gets backlogged to the downstream device.

35f9c09fe9 tcp: tcp_sendpages() should call tcp_push() once
commit 2f53384424 (tcp: allow splice() to build full TSO packets) added
a regression for splice() calls using SPLICE_F_MORE.

We need to call tcp_flush() at the end of the last page processed in
tcp_sendpages(), or else transmits can be deferred and future sends
stall.

Add a new internal flag, MSG_SENDPAGE_NOTLAST, acting like MSG_MORE, but
with different semantic.

For all sendpage() providers, its a transparent change. Only
sock_sendpage() and tcp_sendpages() can differentiate the two different
flags provided by pipe_to_sendpage()

234e340582 simple_open: automatically convert to simple_open()
Many users of debugfs copy the implementation of default_open() when
they want to support a custom read/write function op.  This leads to a
proliferation of the default_open() implementation across the entire
tree.

Now that the common implementation has been consolidated into libfs we
can replace all the users of this function with simple_open().

7563ec4c21 hugetlbfs: remove unregister_filesystem() when initializing module
It was introduced by d1d5e05ffd ("hugetlbfs: return error code when
initializing module") but as Al pointed out, is a bad idea.

Quoted comments from Al:
 "Note that unregister_filesystem() in module init is *always* wrong;
  it's not an issue here (it's done too early to care about and
  realistically the box is not going anywhere - it'll panic when attempt
  to exec /sbin/init fails, if not earlier), but it's a damn bad
  example.

  Consider a normal fs module.  Somebody loads it and in parallel with
  that we get a mount attempt on that fs type.  It comes between
  register and failure exits that causes unregister; at that point we
  are screwed since grabbing a reference to module as done by mount is
  enough to prevent exit, but not to prevent the failure of init.  As
  the result, module will get freed when init fails, mounted fs of that
  type be damned."

So remove it.

This allocation can be as large as 64k.  As David points out, "falling
back to vmalloc here is much better solution than failing to retreive
the attribute - it will work no matter how fragmented memory gets.  That
means we don't get incomplete backups occurring after days or months of
uptime and successful backups".

99663be772 proc: fix mount -t proc -o AAA
The proc_parse_options() call from proc_mount() runs only once at boot
time.  So on any later mount attempt, any mount options are ignored
because ->s_root is already initialized.

As a consequence, "mount -o <options>" will ignore the options.  The
only way to change mount options is "mount -o remount,<options>".

To fix this, parse the mount options unconditionally.

We can deadlock if we have a write oplock and two processes
use the same file handle. In this case the first process can't
unlock its lock if the second process blocked on the lock in the
same time.

Fix it by using posix_lock_file rather than posix_lock_file_wait
under cinode->lock_mutex. If we request a blocking lock and
posix_lock_file indicates that there is another lock that prevents
us, wait untill that lock is released and restart our call.

6308191f6f tracing, sched, vfs: Fix 'old_pid' usage in trace_sched_process_exec()
1. TRACE_EVENT(sched_process_exec) forgets to actually use the
   old pid argument, it sets ->old_pid = p->pid.

2. search_binary_handler() uses the wrong pid number. tracepoint
   needs the global pid_t from the root namespace, while old_pid
   is the virtual pid number as it seen by the tracer/parent.

With this patch we have two pid_t's in search_binary_handler(),
not really nice. Perhaps we should switch to "struct pid*", but
in this case it would be better to cleanup the current code
first and move the "depth == 0" code outside.

 "We've merged in the error handling patches from SuSE.  These are
  already shipping in the sles kernel, and they give btrfs the ability
  to abort transactions and go readonly on errors.  It involves a lot of
  churn as they clarify BUG_ONs, and remove the ones we now properly
  deal with.

  Josef reworked the way our metadata interacts with the page cache.
  page->private now points to the btrfs extent_buffer object, which
  makes everything faster.  He changed it so we write an whole extent
  buffer at a time instead of allowing individual pages to go down,,
  which will be important for the raid5/6 code (for the 3.5 merge
  window ;)

  Josef also made us more aggressive about dropping pages for metadata
  blocks that were freed due to COW.  Overall, our metadata caching is
  much faster now.

  We've integrated my patch for metadata bigger than the page size.
  This allows metadata blocks up to 64KB in size.  In practice 16K and
  32K seem to work best.  For workloads with lots of metadata, this cuts
  down the size of the extent allocation tree dramatically and fragments
  much less.

  Scrub was updated to support the larger block sizes, which ended up
  being a fairly large change (thanks Stefan Behrens).

  We also have an assortment of fixes and updates, especially to the
  balancing code (Ilya Dryomov), the back ref walker (Jan Schmidt) and
  the defragging code (Liu Bo)."

Fixed up trivial conflicts in fs/btrfs/scrub.c that were just due to
removal of the second argument to k[un]map_atomic() in commit
7ac687d9e0.

cb85a6ed67 proc: stats: Use arch_idle_time for idle and iowait times if available
Git commit a25cac5198 "proc: Consider NO_HZ when printing idle and
iowait times" changes the code for /proc/stat to use get_cpu_idle_time_us
and get_cpu_iowait_time_us if the system is running with nohz enabled.
For architectures which define arch_idle_time (currently s390 only)
this is a change for the worse. The result of arch_idle_time is supposed
to be the exact sleep time of the target cpu and should be used instead
of the value kept by the scheduler.

6268b325c3 Revert "ext4: don't release page refs in ext4_end_bio()"
This reverts commit b43d17f319.

Dave Jones reports that it causes lockups on his laptop, and his debug
output showed a lot of processes hung waiting for page_writeback (or
more commonly - processes hung waiting for a lock that was held during
that writeback wait).

The page_writeback hint made Ted suggest that Dave look at this commit,
and Dave verified that reverting it makes his problems go away.

Ted says:
 "That commit fixes a race which is seen when you write into fallocated
  (and hence uninitialized) disk blocks under *very* heavy memory
  pressure.  Furthermore, although theoretically it could trigger under
  normal direct I/O writes, it only seems to trigger if you are issuing
  a huge number of AIO writes, such that a just-written page can get
  evicted from memory, and then read back into memory, before the
  workqueue has a chance to update the extent tree.

  This race has been around for a little over a year, and no one noticed
  until two months ago; it only happens under fairly exotic conditions,
  and in fact even after trying very hard to create a simple repro under
  lab conditions, we could only reproduce the problem and confirm the
  fix on production servers running MySQL on very fast PCIe-attached
  flash devices.

  Given that Dave was able to hit this problem pretty quickly, if we
  confirm that this commit is at fault, the only reasonable thing to do
  is to revert it IMO."

So does option "-o autodefrag,compress".

Reasons:
When the cleaner kthread gets to fetch inodes from the defrag tree and defrag
them, it will dirty pages and submit them, this will comes to another DATA COW
where the processing inode will be inserted to the defrag tree again.

This patch sets a rule for COW code, i.e. insert an inode when we're really
going to make some defragments.

fixes the deadlock on page, but it also introduces another bug.

A page may have been truncated after unlock & lock.
So we need to find it again to get the right one.

And since we've held i_mutex lock, inode size remains unchanged and
we can drop isize overflow checks.

The bug is from running xfstests 209 with autodefrag.

The race is as follows:

     invalidate pagecache
                                run_defrag
                                  readpage(old data)
                                  set page dirty (old data)
     dio(new data, rewrite)
     invalidate pagecache (*)
     endio

t2(autodefrag) will get old data into pagecache via readpage and set
pagecache dirty.  Meanwhile, invalidate pagecache(*) will fail due to
dirty flags in pages.  So the old data may be flushed into disk by
flush thread, which will lead to data loss.

And so does the case of user defragment progs.

The patch fixes this race by holding i_mutex when we readpage and set page dirty.

15d1ff8111 Btrfs: fix deadlock during allocating chunks
This deadlock comes from xfstests 251.

We'll hold the chunk_mutex throughout the whole of a chunk allocation.
But if we find that we've used up system chunk space, we need to allocate a
new system chunk, but this will lead to a recursion of chunk allocation and end
up with a deadlock on chunk_mutex.
So instead we need to allocate the system chunk first if we find we're in ENOSPC.

797a9d797f nfsd: only register cld pipe notifier when CONFIG_NFSD_V4 is enabled
Otherwise, we get a warning or error similar to this when building with
CONFIG_NFSD_V4 disabled:

    ERROR: "nfsd4_cld_block" [fs/nfsd/nfsd.ko] undefined!

Fix this by wrapping the calls to rpc_pipefs_notifier_register and
..._unregister in another function and providing no-op replacements
when CONFIG_NFSD_V4 is disabled.

7ca4be45a0 Btrfs: don't use crc items bigger than 4KB
With the big metadata blocks, we can have crc items
that are much bigger than a page.  There are a few
places that we try to kmalloc memory to hold the
items during a split.

Items bigger than 4KB don't really have a huge benefit
in efficiency, but they do trigger larger order allocations.
This commits changes the csums to make sure they stay under
4KB.  This is not a format change, just a #define to limit
huge items.

3c4bb26b21 Btrfs: flush out and clean up any block device pages during mount
Btrfs puts the filesystem metadata into its own address space, and
somehow the block device address space isn't getting onto disk properly
before a mount.  The end result is that a loop of mkfs and mounting the
filesystem will sometimes find stale or incorrect data.

This commit should fix it by sprinkling fdatawrites and invalidate_bdev
calls around.  This is a short term measure to make sure it is fixed.
The block devices really should be flushed and cleaned up higher in the
stack.

42be35d039 fs: only send IPI to invalidate LRU BH when needed
In several code paths, such as when unmounting a file system (but not
only) we send an IPI to ask each cpu to invalidate its local LRU BHs.

For multi-cores systems that have many cpus that may not have any LRU BH
because they are idle or because they have not performed any file system
accesses since last invalidation (e.g.  CPU crunching on high perfomance
computing nodes that write results to shared memory or only using
filesystems that do not use the bh layer.) This can lead to loss of
performance each time someone switches the KVM (the virtual keyboard and
screen type, not the hypervisor) if it has a USB storage stuck in.

This patch attempts to only send an IPI to cpus that have LRU BH.

        Move memory barriers here.  This already done for MIPS and Alpha.

    (2) asm/switch_to.h

        Move switch_to() and related stuff here.

    (3) asm/exec.h

        Move arch_align_stack() here.  Other process execution related bits
        could perhaps go here from asm/processor.h.

    (4) asm/cmpxchg.h

        Move xchg() and cmpxchg() here as they're full word atomic ops and
        frequently used by atomic_xchg() and atomic_cmpxchg().

    (5) asm/bug.h

        Move die() and related bits.

b18dafc86b vfs: fix d_ancestor() case in d_materialize_unique
In d_materialise_unique() there are 3 subcases to the 'aliased dentry'
case; in two subcases the inode i_lock is properly released but this
does not occur in the -ELOOP subcase.

This seems to have been introduced by commit 1836750115 ("fix loop
checks in d_materialise_unique()").

[ Added a comment, and moved the unlock to where we generate the -ELOOP,
  which seems to be more natural.

  You probably can't actually trigger this without a buggy network file
  server - d_materialize_unique() is for finding aliases on non-local
  filesystems, and the d_ancestor() case is for a hardlinked directory
  loop.

  But we should be robust in the case of such buggy servers anyway. ]

If we find that "cup" is NULL in this case, then we obviously don't
want to dereference it. What we really want to print in this case
