mspec_mmap was setting VM_LOCKED (without adjusting locked_vm): don't do
that, it serves no purpose in 2.6, other than to mess up the locked_vm
accounting - mspec's pages won't get reclaimed anyway. Thanks to Dmitry
Monakhov for raising the issue.

The only pseudo-legitimate MIPS user of genrtc was a systems that doesn't
have an RTC in hardware at all. At this point faking one is a little
pointless ...
sysfs is now completely out of driver/module lifetime game. After
deletion, a sysfs node doesn't access anything outside sysfs proper,
so there's no reason to hold onto the attribute owners. Note that
often the wrong modules were accounted for as owners leading to
accessing removed modules.
This patch kills now unnecessary attribute->owner. Note that with
this change, userland holding a sysfs node does not prevent the
backing module from being unloaded.

Instead of all drivers reading pci config space to get the revision
ID, they can now use the pci_device->revision member.
This exposes some issues where drivers where reading a word or a dword
for the revision number, and adding useless error-handling around the
read. Some drivers even just read it for no purpose of all.
In devices where the revision ID is being copied over and used in what
appears to be the equivalent of hotpath, I have left the copy code
and the cached copy as not to influence the driver's performance.
Compile tested with make all{yes,mod}config on x86_64 and i386.

Most of the AGP changes recently have been done in lock-step with
DRM updates, so it's probably easier to have airlied pushing
AGP changes at the same time he does DRM updates.

Cautionary tale to others: Never do this, when computers
don't boot, people assume you're responsible even if 15
other subsystems initialised after yours. :-) ]

Recently my console UTF-8 patch went mainline. Here is an additional patch
that fixes two nasty issues and improves a third one, namely:
1. My patch changed the behavior if a glyph is not found in the Unicode
mapping table. Previously for Unicode values less than 256 or 512 the
kernel tried to display the glyph from that position of the glyph table,
which could lead to a different accented letter being displayed. I
removed this fallback possibility and changed it to display the
replacement symbol.
As Behdad pointed out, some fonts (e.g. sun12x22 from the kbd package)
lack Unicode mapping information, hence all you get is lots of question
marks. Though theoretically it's actually a user-space bug (the font
should be fixed), Behdad and I both believe that it'd be good to work
around in the kernel by re-introducing the fallback solution for ASCII
characters only. This sounds a quite reasonable decision, since all fonts
ship the ASCII characters in the first 128 positions. This way users
won't be surprised by lots of question marks just because s/he issued a
not-so-perfectly parameterized setfont command. As this fallback is only
re-introduced for code points below 128, you still won't see an accented
letter replaced by another, but at least you'll always get the English
letters right.
2. My patch introduced "question mark with inverted color attributes" as a
last resort fallback glyph. Though it perfectly works on VGA console, on
framebuffer you may end up with question marks that are highlighed but
shouldn't be, and normal characters that are accidentally highlighed.
This is caused by missing FLUSHes when changing the color attribute.
3. I've updated the table of double-width character based on Markus's
updated version. Only ten new code poings (one interval) is added.

After i915 chip, GMCH has no AGP port. Origin bridge driver in device
table will try to access illegal regs like APBASE, APSIZE, etc. This
may cause problem.
So mark them as NULL in the table, we won't load if no IGD got detect
and bridge has no AGP port.

Folding is done to minimize the theoretical possibility of systematic
weakness in the particular bits of the SHA1 hash output. The result of
this bug is that 16 out of 80 bits are un-folded. Without a major new
vulnerability being found in SHA1, this is harmless, but still worth
fixing.

The bug is caused by switching to hung_up_tty_fops in do_tty_hangup. An
ioctl call can be waiting on BLK after testing for existence of the locked
ioctl handler in the normal tty fops, but before calling the locked ioctl
handler. If a hangup occurs at that point, the locked ioctl fop is NULL
and an oops occurs.
(akpm: we can remove my debugging code from do_ioctl() now, but it'll be OK to
do that for 2.6.23)

Commit 9b01bd5b28 introduced a
compat_ioctl handler for RADEON_SETPARAM, the sole purpose of which was
to handle the fact that on i386, alignof(uint64_t)==4.
Unfortunately, this handler was installed for _all_ 64-bit
architectures, instead of only x86_64 and ia64. And thus it breaks
32-bit compatibility on every other arch, where 64-bit integers are
aligned to 8 bytes in 32-bit mode just the same as in 64-bit mode.
Arnd has a cunning plan to use 'compat_u64' with appropriate alignment
attributes according to the 32-bit ABI, but for now let's just make the
compat_radeon_cp_setparam routine entirely disappear on 64-bit machines
whose 32-bit compat support isn't for i386. It would be a no-op with
compat_u64 anyway.

kill_pgrp(task_pgrp(current)) sends the signal to the current's thread
group, but can choose any sub-thread as a target for signal_wake_up().
This means that job_control() and tty_check_change() may return

I have made a tool to parse the kernel that does not pre-process the
source. That means that my parser tries to parse all the code, including
code in the #else branch or code that is not often compiled because the
driver is not very used (or not used at all). So, my parser sometimes
reports parse error not originally detected by gcc. Here is my (first)
patch.

First thing mm.h does is including sched.h solely for can_do_mlock() inline
function which has "current" dereference inside. By dealing with can_do_mlock()
mm.h can be detached from sched.h which is good. See below, why.
This patch
a) removes unconditional inclusion of sched.h from mm.h
b) makes can_do_mlock() normal function in mm/mlock.c
c) exports can_do_mlock() to not break compilation
d) adds sched.h inclusions back to files that were getting it indirectly.
e) adds less bloated headers to some files (asm/signal.h, jiffies.h) that were
getting them indirectly
Net result is:
a) mm.h users would get less code to open, read, preprocess, parse, ... if
they don't need sched.h
b) sched.h stops being dependency for significant number of files:
on x86_64 allmodconfig touching sched.h results in recompile of 4083 files,
after patch it's only 3744 (-8.3%).

Andi Kleen is unhappy with the changes, and they really do not seem
worth it. IPMI could use DIE_NMI_IPI instead of the new callback, even
though that ends up having its own set of problems too, mainly because
the IPMI code cannot really know the NMI was from IPMI or not.
Manually fix up conflicts in arch/x86_64/kernel/traps.c and
drivers/char/ipmi/ipmi_watchdog.c.

When trying to load the ipmi_si module on a powerpc with no BMC (baseboard
management controller) the driver failes to load correctly, but doesn't
unregister itself from of_platform. So, on a second modprobe the kernel
crashes. This patch adds the missing unregister call.

Flush the tty flip buffer when the line discipline input queue is flushed,
including the user call tcflush(TCIFLUSH/TCIOFLUSH). This prevents
unexpected stale data after a user application calls tcflush().

I'm using a custom BIOS to configure the northbridge GART at address
0x80000000, size 2G. Linux complains:

The ACPI/PM base I/O address which is the base
for the TCO registers is defined as bits [15:7]
(highest bit is 31, lowest is 0)
The code however only reads bits [14:7]. So
base_address &= 0x00007f80;
needs to be:
base_address &= 0x0000ff80;
This patch fixes this.

Fix trivial comment conflict in kernel/relay.c.
Fix tty_set_ldisc in tty_io.c so that tty->receive_room is only cleared if
actually changing line disciplines.
Without this fix a problem occurs when requesting the line discipline to
change to the same line discipline. In this case tty->receive_room is
cleared but ldisc->open() is not called to set tty->receive_room back to a
sane value. The result is that tty->receive_room is stuck at 0 preventing
the tty flip buffer from passing receive data to the line discipline.
For example: a switch from N_TTY to N_TTY followed by a select() call for
read input results in data never being received because tty->receive_room
is stuck at zero.
A switch from N_TTY to N_TTY followed by a read() call works because the
read() call itself sets tty->receive_room correctly (but select does not).
Previously (< 2.6.18) this was not a problem because the tty flip buffer
pushed data to the line discipline without regard for tty->receive room.

Create a variable, default_utf8, that defines the system-wide default UTF-8
setting. This variable can be altered via sysfs. If the variable is properly
set, this should mimimize breakage of UTF-8 encoded consoles when doing a
reset or echo -e '\033c' and of newly opened/allocated consoles.
This is based from patches by Jan Engelhardt and Paul LeoNerd Evans.

flush_scheduled_work() can sleep, and we're calling it under spinlock.
AFAICS, moving flush_scheduled_work before spin_lock() should not cause any
problems.
Reason being - The only thing that can race against tpm_release is tpm_open
(tpm_release is called when last reference to the file is closed and only
thing that can happen after that is tpm_open??) and tpm_open acquires
driver_lock and more over it bails out with EBUSY if chip->num_opens is
greater than 0.
I also moved chip->num_pending-- to after deleting timer and setting data
pending as it looks more correct for the paranoid although it probably doesn't
matter as it is guarded by driver_lock. None the less this change should not
cause problems.
While I was at it I noticed a missing NULL check in tpm_register_hardware
which is fixed with this patch as well.

The UTF-8 part of the vt driver suffers from the following issues which are
addressed in my patch:
1) If there's no glyph found for a particular valid UTF-8 character, we try
to display U+FFFD. However if this one is not found either, here's what
the current kernel does:
- First, if the Unicode value is less than the number of glyphs, use the
glyph directly from that position of the glyph table. While it may be a
good idea in the 8-bit world, it has absolutely no sense with Unicode
in mind. For example, if a Latin-2 font is loaded and an application
prints U+00FB ("u with circumflex", not present in Latin-2) then as a
fallback solution the glyph from the 0xFB position of the Latin-2
fontset (which is an "u with double accent" - a different character) is
displayed.
- Second, if this fallback fails too, a simple ASCII question mark is
printed, which is visually undistinguishable from a real question mark.
I changed the code to skip the first step (except if in non-UTF-8 mode),
and changed the second step to print the question mark with inverse color
attributes, so it is visually clear that it's not a real question mark,
and resembles more to the common glyph of U+FFFD.
2) The UTF-8 decoder is buggy in many ways:
- Lone continuation bytes (section 3.1 of Markus Kuhn's UTF-8 stress
test) are not caught, they are displayed as some "random" (taken
directly form the font table, see above) glyphs instead the replacement
character.
- Incomplete sequences (sections 3.2 and 3.3 of the stress test) emit no
replacement character, but rather cause the subsequent valid character
to be displayed more times(!).
- The decoder is not safe: overlong sequences are not caught currently,
they are displayed as if these were valid representations. This may
even have security impacts.
- The decoder does not handle D800..DFFF and FFFE..FFFF specially, it
just emits these code points and lets it be looked up in the glyph
table. Since these are invalid code points, I replace them by U+FFFD
and hence give no chance for them to be looked up in the glyph table.
(Assuming no font ships glyphs for these code points, this change is
not visible to the users since the glyph shown will be the same.)
With my fixes to the decoder it now behaves exactly as Markus Kuhn's
stress test recommends.
3) It has no concept of double-width (CJK) characters. It's way beyond the
scope of my patch to try to display them, but at least I think it's
important for the cursor to jump two positions when printing such
characters, since this is what applications (such as text editors)
expect. Currently the cursor only jumps one position, and hence
applications suffer from displaying and refreshing problems, and editing
some English letters that are preceded by some CJK characters in the same
line is a nightmare. With my patch an additional space is inserted after
the CJK character has been printed (which usually means a replacement
symbol of course). (If U+FFFD isn't availble and hence an inverse
question mark is displayed in the first cell, I keep the inverted state
for the space in the 2nd column so it's quite easy to see that they are
tied together.)
4) There is a small built-in table of zero-width spaces that are not to be
printed but silently skipped. U+200A is included there, but it's not a
zero-width character, so I remove it from there.

This patch fixes two things in module_init.
- fix register_chrdev() error check
Currently dtlk doesn't check register_chrdev() failure correctly.
register_chrdev() returns a errno on failure.
- check probe failure
dtlk ignores probe failure and allows the module loading without
such device. I got "Trying to free nonexistent resource" message
by release_region() when unloading module without device.

Update some of the layered parport_driver code to use parport->dev:
- i2c-parport (parent of i2c_adapter)
- spi_butterfly (parent of spi_master, allowing cruft removal)
- lp (creating class_device)
- ppdev (parent of parportN device)
- tipar (creating class_device)
There are still drivers that should be updated, like some of the input
drivers; but they won't be any worse off than they are today.

This patch moves the die notifier handling to common code. Previous
various architectures had exactly the same code for it. Note that the new
code is compiled unconditionally, this should be understood as an appel to
the other architecture maintainer to implement support for it aswell (aka
sprinkling a notify_die or two in the proper place)
arm had a notifiy_die that did something totally different, I renamed it to
arm_notify_die as part of the patch and made it static to the file it's
declared and used at. avr32 used to pass slightly less information through
this interface and I brought it into line with the other architectures.

While researching the tty layer pid leaks I found a weird case in selinux when
we drop a controlling tty because of inadequate permissions we don't do the
normal hangup processing. Which is a problem if it happens the session leader
has exec'd something that can no longer access the tty.
We already have code in the kernel to handle this case in the form of the
TIOCNOTTY ioctl. So this patch factors out a helper function that is the
essence of that ioctl and calls it from the selinux code.
This removes the inconsistency in handling dropping of a controlling tty and
who knows it might even make some part of user space happy because it received
a SIGHUP it was expecting.
In addition since this removes the last user of proc_set_tty outside of
tty_io.c proc_set_tty is made static and removed from tty.h

This patch should contain no functional changes.
At some point I got confused and thought put_pid could not be called while a
spin lock was held. While it may be nice to avoid that to reduce lock hold
times put_pid can be safely called while we hold a spin lock.
This patch removes all of the complications from the code introduced by my
misunderstanding, making the code a little more readable.

The console subsystem already has an idea of a boot console, using the
CON_BOOT flag. The implementation has some flaws though. The major
problem is that presence of a boot console makes register_console() ignore
any other console devices (unless explicitly specified on the kernel
command line).
This patch fixes the console selection code to *not* consider a boot
console a full-featured one, so the first non-boot console registering will
become the default console instead. This way the unregister call for the
boot console in the register_console() function actually triggers and the
handover from the boot console to the real console device works smoothly.
Added a printk for the handover, so you know which console device the
output goes to when the boot console stops printing messages.
The disable_early_printk() call is obsolete with that patch, explicitly
disabling the early console isn't needed any more as it works automagically
with that patch.
I've walked through the tree, dropped all disable_early_printk() instances
found below arch/ and tagged the consoles with CON_BOOT if needed. The
code is tested on x86, sh (thanks to Paul) and mips (thanks to Ralf).
Changes to last version: Rediffed against -rc3, adapted to mips cleanups by
Ralf, fixed "udbg-immortal" cmd line arg on powerpc.

At least on x86_64 the present cyclades.h is broken due to the wrong size
of uclong. This affects, of course, both the kernel and the user-level
utilities. The symptom is that cyzload refuses to load the firmware. I
also managed to freeze the machine when unloading the module.
The patch below fixes this in an architecture-independent way. I have
tested it with 2.6.19 and the driver works fine again with a Cyclades-Z on
an Athlon 64 X2.

Replace call_smp_function with stop_machine_run in the Intel RNG driver.
CPU A has done read_lock(&lock)
CPU B has done write_lock_irq(&lock) and is waiting for A to release the lock.
A third CPU calls call_smp_function and issues the IPI. CPU A takes CPU
C's IPI. CPU B is waiting with interrupts disabled and does not see the
IPI. CPU C is stuck waiting for CPU B to respond to the IPI.
Deadlock.
The solution is to use stop_machine_run instead of call_smp_function
(call_smp_function should not be called in situations where the CPUs may be
suspended).

Convert over to the new NMI handling for getting IPMI watchdog timeouts via an
NMI. This add config options to know if there is the ability to receive NMIs
and if it has an NMI post processing call. Then it modifies the IPMI watchdog
to take advantage of this so that it can know if an NMI comes in.
It also adds testing that the IPMI NMI watchdog works.

The IPMI driver used enable_irq and disable_irq when it got into situations
where it couldn't allocate memory; it did this to avoid having the interrupt
just lock the machine when it couldn't get memory to perform the transaction
to disable the interrupt.
This patch modifies the driver to not use disable_irq and enable_irq. It
instead sends the messages to the BMC to perform this operation. It also
makes sure interrupts are cleanly disabled when the interface is shut down and
cleans up some shutdown things that are no longer necessary.

Add support for of_platform_driver to the ipmi_si module. When loading the
module, the driver will be registered to of_platform. The driver will be
probed for all devices with the type ipmi. It's supporting devices with
compatible settings ipmi-kcs, ipmi-smic and ipmi-bt. Only ipmi-kcs could be
tested.

There can still be other contexts that may use page flipping later on, so do
just unilaterally 'clean it up', which could lead to the wrong page being
displayed, e.g. when running 3D apps with a GLX compositing manager such as
compiz using page flipping.

radeon: make PCI GART aperture size variable, but making table size variable
This is precursor to getting a TTM backend for this stuff, and also
allows the PCI table to be allocated at fb 0
radeon: add support for reverse engineered xpress200m
The IGPGART setup code was traced using mmio-trace on fglrx by myself
and Phillip Ezolt <phillipezolt@gmail.com> on dri-devel.
This code doesn't let the 3D driver work properly as the card has no
vertex shader support.

If device->num is zero we attempt to kmalloc() zero bytes. When SLUB is
enabled this returns a null pointer and take that as an allocation failure
and fail the device register. Check for no devices and avoid the
allocation.

The Berkshire USB-PC Watchdog driver uses a semaphore as mutex. Use the mutex
API instead of the (binary) semaphore.

I noticed that many source files include <linux/pci.h> while they do
not appear to need it. Here is an attempt to clean it all up.
In order to find all possibly affected files, I searched for all
files including <linux/pci.h> but without any other occurence of "pci"
or "PCI". I removed the include statement from all of these, then I
compiled an allmodconfig kernel on both i386 and x86_64 and fixed the
false positives manually.
My tests covered 66% of the affected files, so there could be false
positives remaining. Untested files are:

Under CONFIG_DISCONTIGMEM, assuming that a !pfn_valid() implies all
subsequent pfn-s are also invalid is wrong. Thus replace this by

Get the IO resources list in sony-laptop in the same order as listed
in sonypi and make sonypi check if one of those is already busy.
The sonypi check can be disabled by a module parameter in case the user
thinks we are plainly wrong (check_ioport=0).

For some vendor/id pairs the kernel will autoload both the sis-agp and the
amd64_agp modules as the sis-agp module will load for all sis devices.
This collision causes the bug reported in:
http://bugzilla.novell.com/show_bug.cgi?id=248665
As currently sis_probe does its own matching, requesting the whole range
gains nothing. The clean fix seems to me to leave the matching to the core
and advertise only the devices actually supported. This patch does so.

In order to get more randomness for secure_tcpv6_sequence_number(),
secure_tcp_sequence_number(), secure_dccp_sequence_number() functions,
we can use the high resolution time services, providing nanosec
resolution.
I've also done two kmalloc()/kzalloc() conversions.

Add a back-off mechanism to hvc_console's polling logic. This change
drops the timers/second ratio from ~90 to ~1/2 while the console is
idle.
This change is most noticeable when watching /proc/timer_stats output.
This only affects when the hvc_console is running in poll mode, i.e.
power4 and cell systems.
I've tested on Power4, Michael Ellerman has both contributed to the
patch and tested on cell.

While digging through my MAP_FIXED changes, I found that rather obvious
bug in /dev/mem mmap implementation for nommu archs. get_unmapped_area()
is expected to return an address, not a pfn.

On G965, I810_PGETBL_CTL is a mmio offset, but we wrongly take it
as pci config space offset in detecting GTT size. This one line patch

On a multiprocessor machine the VT_WAITACTIVE ioctl call may return 0 if
fg_console has already been updated in redraw_screen() but the console
switch itself hasn't been completed. Fix this by checking fg_console in
vt_waitactive() with the console sem held.

Commit d720bc4b8f partially removed a
private implementation of baud speed decoding. However it doesn't seem
to be complete: after the speed is decoded, it is still being used as an
index to a local speed table (array overrun, no doubt).
This was found by Graham Murray who noticed it caused a 2.6.19 regression

It seems that some watchdog drivers are doing following mistake:
rv = misc_register();
if (rv < 0)
return rv;
rv = request_region();
if (rv < 0) {
misc_deregister();
return rv;
}
But, right after misc_register() returns, misc device can be opened and
ioctls interacting with hardware issued, and driver can do outb() to
port it doesn't own yet, because request_region() is still pending.

Bugzilla Bug #9457
Add refcounting of user waiters to the DRM hardware lock, so that we can use
DRM_LOCK_CONT flag more conservatively.
Also add a kernel waiter refcount that if nonzero transfers the lock for the
kernel context when it is released. This is useful when waiting for idle and can be used for very simple fence object driver implementations for the new memory manager

and I have been able to very by inspection they are possible.
When converting tty_io.c to store pids as struct pid pointers instead
of pid_t values it appears I overlooked two places where we stop using
the pid value. The very obvious one is in do_tty_hangup, and the one
the less obvious one in __proc_set_tty.
When looking into the code __proc_set_tty only has pids that need to
be put because of failures of other parts of the code to properly
perform hangup processing. Fixing the leak here in __proc_set_tty
is easy and obviously correct so I am doing that first.
Fixing the places that should be performing hangup processing is much
less obviously correct. So those I'm aiming those patches at -mm.
for now, so the can age a while before they are merged.

When the console is in VT_AUTO+KD_GRAPHICS mode, switching to the
SUSPEND_CONSOLE fails, resulting in vt_waitactive() waiting indefinitely or
until the task is interrupted. This patch tests if a console switch can
occur in set_console() and returns early if a console switch is not
possible.

The overflows could lead to the AGP aperture overlapping the framebuffer are in the card's address space when the latter is located at the very end of th 32 bit address space, which would result in a freeze on X server startup,
probably because the card read commands from the framebuffer instead of from AGP.

When calling write() with a buffer larger than 512 bytes, the
driver's write buffer overflows, allowing to overwrite the EIP and
execute arbitrary code with kernel privileges.
In read(), there exists a similar problem, but coming from the device.
A malicous or buggy device sending more than 512 bytes can overflow
of the driver's read buffer, with the same effects as above.

ipmi_si_intf tries to access default ports, if no device could be found
elsewhere. On PPC we have a function to check, if these legacy IO ports
are accessible. This patch adds a check for these ports on PPC. This
patch fixes a breakage of IPMI module on PPC machines without a BMC.

This patch fixes a possible race that leads to double freeing an idr index.
When the master begin to close, release_dev() is called and then
pty_close() is called:

This is done without helding any locks other than BKL. Inside pty_close(),
being a master close, the devpts entry will be removed:

But devpts_pty_kill() will call get_node() that may sleep while waiting for
&devpts_root->d_inode->i_sem. When this happens and the slave is being
opened, tty_open() just found the driver and index:

This part of the code is already protected under tty_mute. The problem is
that the slave close already got an index. Then init_dev() is called and
blocks waiting for the same &devpts_root->d_inode->i_sem.
When the master close resumes, it removes the devpts entry, and the
relation between idr index and the tty is gone. The master then sleeps
waiting for the tty_mutex on release_dev().
Slave open resumes and found no tty for that index. As result, a NULL tty
is returned and init_dev() doesn't flow to fast_track:

The result of this, is that a new tty will be created and init_dev() returns
sucessfull. After returning, tty_mutex is dropped and master close may resume.
Master close finds it's the only use and both sides are closing, then releases
the tty and the index. At this point, the idr index is free, but slave still
has it.
Slave open then calls pty_open() and finds that tty->link->count is 0,
because there's no master and returns error. Then tty_open() calls
release_dev() which executes without any warning, as it was a case of last
slave close when the master is already closed (master->count == 0,
slave->count == 1). The tty is then released with the already released idr
index.
This normally would only issue a warning on idr_remove() but in case of a
customer's critical application, it's never too simple:
thread1: opens master, gets index X
thread1: begin closing master
thread2: begin opening slave with index X
thread1: finishes closing master, index X released
thread3: opens master, gets index X, just released
thread2: fails opening slave, releases index X <----
thread4: opens master, gets index X, init_dev() then find an already in use
and healthy tty and fails
If no more indexes are released, ptmx_open() will keep failing, as the
first free index available is X, and it will make init_dev() fail because
you're trying to "reopen a master" which isn't valid.
The patch notices when this race happens and make init_dev() fail
imediately. The init_dev() function is called with tty_mutex held, so it's
safe to continue with tty till the end of function because release_dev()

Fix conflict in include/linux/sched.h due to kill_proc_info() being made

The line :
hp->Mode &= !RIO_PCI_INT_ENABLE;
is obviously wrong as RIO_PCI_INT_ENABLE=0x04 and is used as a bitmask
2 lines before. Getting no IRQ would not disable RIO_PCI_INT_ENABLE
but rather RIO_PCI_BOOT_FROM_RAM which equals 0x01.
Obvious fix is to change ! for ~.

The semantic effect of insert_at_head is that it would allow new registered
sysctl entries to override existing sysctl entries of the same name. Which is
pain for caching and the proc interface never implemented.
I have done an audit and discovered that none of the current users of
register_sysctl care as (excpet for directories) they do not register
duplicate sysctl entries.
So this patch simply removes the support for overriding existing entries in
the sys_sysctl interface since no one uses it or cares and it makes future
enhancments harder.

After Al Viro (finally) succeeded in removing the sched.h #include in module.h
recently, it makes sense again to remove other superfluous sched.h includes.
There are quite a lot of files which include it but don't actually need
anything defined in there. Presumably these includes were once needed for
macros that used to live in sched.h, but moved to other header files in the
course of cleaning it up.
To ease the pain, this time I did not fiddle with any header files and only
removed #includes from .c-files, which tend to cause less trouble.
Compile tested against 2.6.20-rc2 and 2.6.20-rc2-mm2 (with offsets) on alpha,
arm, i386, ia64, mips, powerpc, and x86_64 with allnoconfig, defconfig,
allmodconfig, and allyesconfig as well as a few randconfigs on x86_64 and all
configs in arch/arm/configs on arm. I also checked that no new warnings were
introduced by the patch (actually, some warnings are removed that were emitted
by unnecessarily included header files).

Somewhere in the rewrite of the work queues my cleanup of SAK handling
got broken. Maybe I didn't retest it properly or possibly the API
was changing so fast I missed something. Regardless currently
triggering a SAK now generates an ugly BUG_ON and kills the kernel.
Thanks to Alexey Dobriyan <adobriyan@openvz.org> for spotting this.
This modifies the use of SAK_work to initialize it when the data
structure it resides in is initialized, and to simply call
schedule_work when we need to generate a SAK. I update both
data structures that have a SAK_work member for consistency.
All of the old PREPARE_WORK calls that are now gone.
If we call schedule_work again before it has processed it
has generated the first SAK it will simply ignore the duplicate
schedule_work request.

- Use DEFINE_TIMER for single (platform dependent) watchdog timers and
do not init them at run-time in these cases.
- del_timer_sync is common in most cases -- we want to wait for timer
function if it's still running.

Many struct file_operations in the kernel can be "const". Marking them const
moves these to the .rodata section, which avoids false sharing with potential
dirty data. In addition it'll catch accidental writes at compile time to
these shared resources.

Of kernel subsystems that work with pids the tty layer is probably the largest
consumer. But it has the nice virtue that the assiation with a session only
lasts until the session leader exits. Which means that no reference counting
is required. So using struct pid winds up being a simple optimization to
avoid hash table lookups.
In the long term the use of pid_nr also ensures that when we have multiple pid
spaces mixed everything will work correctly.

Every call to is_orphaned_pgrp passed in process_group(current) which is racy
with respect to another thread changing our process group. It didn't bite us
because we were dealing with integers and the worse we would get would be a
stale answer.
In switching the checks to use struct pid to be a little more efficient and
prepare the way for pid namespaces this race became apparent.
So I simplified the calls to the more specialized is_current_pgrp_orphaned so
I didn't have to worry about making logic changes to avoid the race.

To properly implement a pid namespace I need to deal exclusively in terms of
struct pid, because pid_t values become ambiguous.
To this end session_of_pgrp is transformed to take and return a struct pid
pointer. To avoid the need to worry about reference counting I now require my
caller to hold the appropriate locks. Leaving callers repsonsible for
increasing the reference count if they need access to the result outside of
the locks.
Since session_of_pgrp currently only has one caller and that caller simply
uses only test the result for equality with another process group, the locking
change means I don't actually have to acquire the tasklist_lock at all.
tiocspgrp is also modified to take and release the lock. The logic there is a
little more complicated but nothing I won't need when I convert pgrp of a tty
to a struct pid pointer.

The code to look at tty_old_pgrp and send SIGHUP and SIGCONT when it is
present only executes when disassociate_ctty is called from do_exit. Make
this clear by adding an explict on_exit check, and explicitly setting
tty_old_pgrp to 0.
In addition fix the locking by reading tty_old_pgrp under the siglock.

The aim of this patch set is to start wrapping up the struct pid conversions.
As such this patchset culminates with the removal of kill_pg, kill_pg_info,
__kill_pg_info, do_each_task_pid, and while_each_task_pid.
kill_proc, daemonize, and kernel_thread are still in my sights but there is
still work to get to them.
The first three are basic cleanups around disassociate_ctty, while working on
converting it I found several issues. tty_old_pgrp can be a tricky concept to
wrap your head around.
1 tty: Make __proc_set_tty static.
2 tty: Clarify disassociate_ctty
3 tty: Fix the locking for signal->session in disassociate_ctty
These just stop using the old helper functions.
4 signal: Use kill_pgrp not kill_pg in the sunos compatibility code.
5 signal: Rewrite kill_something_info so it uses newer helpers.
Then the grind to convert the tty layer and all of it's helper functions to
struct pid.
6 pid: Make session_of_pgrp use struct pid instead of pid_t.
7 pid: Use struct pid for talking about process groups in exit.c
8 pid: Replace is_orphaned_pgrp with is_current_pgrp_orphaned
9 tty: Update the tty layer to work with struct pid.
A final helper function update.
10 pid: Replace do/while_each_task_pid with do/while_each_pid_task
And the removal of the functions that are now unused.
11 pid: Remove now unused do_each_task_pid and while_each_task_pid
12 pid: Remove the now unused kill_pg kill_pg_info and __kill_pg_info
All of these should be fairly simple and to the point.

release_mem contains two copies of exactly the same code. Refactor these
into a new helper, release_tty. The only change in behaviour is that the
driver reference count is now decremented after the master tty has been
freed instead of before.

Minor number 0 (under the raw major) is reserved for the rawctl device
file, which is used to query, set, and unset raw device bindings. However,
the ioctl interface does not protect the user from specifying a raw device
with minor number 0:
$ sudo ./raw /dev/raw/raw0 /dev/VolGroup00/swap
/dev/raw/raw0: bound to major 253, minor 2
$ ls -l /dev/rawctl
ls: /dev/rawctl: No such file or directory
$ ls -l /dev/raw/raw0
crw------- 1 root root 162, 0 Jan 12 10:51 /dev/raw/raw0
$ sudo ./raw -qa
Cannot open master raw device '/dev/rawctl' (No such file or directory)
As you can see, this prevents any further raw operations from
succeeding. The fix (from Steve Fernandez) is quite simple--do not
allow the allocation of minor number 0.

Mostly so people can see the work in progress. This enhances the encode
function which isn't currently used in the base tree but is when using some of
the testing tty patches.
This resolves a problem with some hardware where applications got confusing
information from the tty ioctls. Correct but confusing.
In some situations asking for, say, 9600 baud actually gets you 9595 baud or
similar near-miss values. With the old code this meant that a request for
B9600 got a return of BOTHER, 9595 which programs interpreted as a failure.
The new code now works on the following basis
- If you ask for specific rate via BOTHER, you get a precise return
- If you ask for a standard Bfoo rate and the result is close you get a Bfoo
return
- If you ask for a standard Bfoo rate and get something way off you get a

Fix some RCU problem pointed out by Paul McKenney of IBM. These are:
The wholesale move of the command receivers list into a new list was not
safe because the list will point to the new tail during a traversal, so the
traversal will never end on a reader if this happens during a read.
Memory barriers were needed to handle proper ordering of the setting of the
IPMI interface as valid. Readers might not see proper ordering of data
otherwise.
In ipmi_smi_watcher_register(), the use of the _rcu suffix on the list is
unnecessary.
This require the list_splice_init_rcu() patch previously posted.

moxaXmitBuff is almost unused -- only one byte from the whole PAGE_SIZE bytes
is used. Do not alloc so much space for almost anything. Also remove lock
protecting this page allocation.

driver_data are initialzed to NULL from tty layer, no need to do it in the
driver. In this case it cases oops, since driver_data may be NULL for a short
while for another closing process.

Because brd->info is not NULLed, resources are released twice. NULL it in
pci_remove function. Also take care of retval and releasing in pci_probe --
mxser_initbrd alreasy releases resource, do not do it again in fail path in
probe function.

Avoid oopsing when stress-testing open/close -- port->tty is NULL sometimes,
but is expected to be non-NULL, since dereferencing. Receive/transmit chars
iff ASYNC_CLOSING is not set and ASYNC_INITIALIZED is set. Thanks Sergei for

This does several things.
- It moves looking up of the current foreground console into process
context where we can safely take the semaphore that protects this
operation.
- It uses the new flavor of work queue processing.
- This generates a factor of do_SAK, __do_SAK that runs immediately.
- This calls __do_SAK with the console semaphore held ensuring nothing
else happens to the console while we process the SAK operation.
- With the console SAK processing moved into process context this
patch removes the xchg operations that I used to attempt to attomically
update struct pid, because of the strange locking used in the SAK processing.
With SAK using the normal console semaphore nothing special is needed.

Remove the memory manager parameter from the put_block function, as this
makes the client code a lot cleaner. Prepare buffer manager for lock and
unlock calls.
Fix buggy aligned allocations.
Remove the stupid root_node field from the core memory manager.
Support multi-page buffer offset alignments
Add improved alignment functionality to the core memory manager.
This makes an allocated block actually align itself and returns any
wasted space to the manager.

We don't unmap them on the suspend path, so on resume
trying to remap will fail, and then result in an
oops the next time something tries to access them.

This patch allows drm to populate an agpgart structure with pages of its own.
It's needed for the new drm memory manager which dynamically flips pages in and out of AGP.
The patch modifies the generic functions as well as the intel agp driver. The intel drm driver is
currently the only one supporting the new memory manager.
Other agp drivers may need some minor fixing up once they have a corresponding memory manager enabled drm driver.
AGP memory types >= AGP_USER_TYPES are not populated by the agpgart driver, but the drm is expected
to do that, as well as taking care of cache- and tlb flushing when needed.
It's not possible to request these types from user space using agpgart ioctls.
The Intel driver also gets a new memory type for pages that can be bound cached to the intel GTT.

Change SysRq showBlockedTasks from sysrq-X to sysrq-W and show that in the
Help message.
It was previously done via X, but X is already used for Xmon on ppc & powerpc
platforms and this collision needs to be avoided.
All callers of register_sysrq_key() are now marked in the sysrq op/key table.
I didn't mark 'h' as Help because Help is just printed for any unknown key,
such as '?'.
Added some omitted sysrq key entries in the sysrq.txt file.

Currently in resuming path graphics device's pci space restore is
behind host bridge, so resume function wrongly accesses graphics
device's space. This makes resuming failure which crashed X.
here's a patch to restore device's pci space early, which makes
resuming ok with X.

This changes all HWRNG driver initcalls to module_init(). We must probe
the RNGs after the major kernel subsystems are already up and running (like
PCI).

Add a load option to intel-rng to allow skipping the FWH detection,
necessary in case the BIOS has locked read-only the firmware hub space.
Also prevent any attempt to write to firmware space if it cannot be write
enabled (apparently caused hangs on some systems not having an FWH and thus
also not having a respective RNG).

The PCWD cards normally use the heartbeat that is set via
the dip-switches of the card. There are only 3 switches,
thus 8 combinations that each have a certain heartbeat.
The card can however be programmed with a heartbeat from
1 till 65535 seconds. This is what our driver does: it
programs the heartbeat on the card.
There are however a lot of people that don't know that
we set the heartbeat of the watchdog card to the value
provided by the heartbeat module parameter. Instead they
think that the heartbeat value is the same as set by the
dip-switches.
This patch changes the driver so that at startup you can
take the heartbeat from the dip-switches. You do this
by setting the heartbeat module parameter to 0. This
patch also makes this the default behaviour.

set_param_str() cannot use kstrdup() to duplicate the parameter. That's
fine when the driver is compiled as a module but it sure is not when built
into the kernel as the kernel parameters are parsed before the kmalloc
slabs are setup.

On the G965, the GTT size may be larger than is required to cover the
aperture. (In fact, on all hardware we've seen, the GTT is 512KB to the
aperture's 256MB). A previous commit forced the aperture size to 512MB on
G965 to match GTT, which would likely result in hangs at best if users
tried to rely on agpgart's aperture size information. Instead, we use the
resource length for the aperture size and the system's reported GTT size
when available for the GTT size.
Because the MSAC registers which had been read for aperture size detection
on i9xx chips just cause a change in the resource size, we can use generic
code for aperture detection on all i9xx.

This patch is to speed up flipping of pages in and out of the AGP aperture as
needed by the new drm memory manager.
A number of global cache flushes are removed as well as some PCI posting flushes.
The following guidelines have been used:
1) Memory that is only mapped uncached and that has been subject to a global
cache flush after the mapping was changed to uncached does not need any more
cache flushes. Neither before binding to the aperture nor after unbinding.
2) Only do one PCI posting flush after a sequence of writes modifying page
entries in the GATT.

Cleanup the s3c2410_wdt driver's exit point by
using labels instead of multiple returns. Also
remove the checks for the resources having been
allocate in the exit, as we will now either have
fully allocated or not allocated the resources
at-all.

Replace r300_check_offset() with generic radeon_check_offset(), which doesn't
reject valid offsets when the framebuffer area is at the very end of the card's
32 bit address space. Make radeon_check_and_fixup_offset() use
radeon_check_offset() as well.
This fixes https://bugs.freedesktop.org/show_bug.cgi?id=7697 .
