This replaces the remaining defines which are available in "public"
include/ directory and are re-defined by the storage gadget.
This is patch is basicaly search & replace followed by the removal of
the defines.

This moves the BOT data structures for CBW and CSW from drivers internal
header file to global include able file in include/.
The storage gadget is using the same name for CSW but a different for
CBW so I fix it up properly. The same goes for the ub driver and keucr
driver in staging.

The reason of above dump message is calling dma_poll_alloc with can-schedule
mem_flags at atomic context.
To fix this problem, below changes are made:
- fsl_req_to_dtd doesn't need to be protected by spin_lock_irqsave,
as struct usb_request can be access at process context. Move lock
to beginning of hardware visit (fsl_queue_td).
- Change the memory flag which using to allocate dTD descriptor buffer,
the memory flag can be from gadget layer.
It is tested at i.mx51 bbg board with g_mass_storage, g_ether, g_serial.

This is a flexible USB Audio Class 2.0 compliant gadget driver that
implements a simple topology with a virtual sound card exposed at
the function side.
The driver doesn't expect any real audio codec to be present on the
function - the audio streams are simply sinked to and sourced from a
virtual ALSA sound card created. The user-space application may choose
to do whatever it wants with the data received from the USB Host and
choose to provide whatever it wants as audio data to the USB Host.
Capture(USB-Out) and Playback(USB-In) can be run at independent
configurations specified via module parameters while loading the driver.
Make this new version as the default selection by a new Kconfig choice.

This patch adds SS descriptors to the ACM & generic serial gadget. The
ACM part was tested with minicom + dummy + send / receive files over
ttyACM <=> ttyGS0.
The generic serial part (f_serial) was not tested (haven't found a
driver on the host side).
The nokia & multi gadget use HS at most.

Since commit 72c973dd aka ("usb: gadget: add usb_endpoint_descriptor to
struct usb_ep) the descriptor is part of the ep. Most gadgets like
g_zero or masstorage call config_ep_by_speed() to grab an available
endpoint which may be used for FS/HS/SS bulk/iso/intr and in a second
they assign the proper descriptor by calling config_ep_by_speed(). This
is good so far. A few of them like ncm call config_ep_by_speed() only if
ep->desc not assigned earlier. That means ep->desc is never assigned if
the endpoint was used by another gadget before it was removed.
Some of those gadgets also assign ep->driver_data to NULL on reset or
ep_disable part _but_ keep a reference to this endpoint. At ep_enable
time they assign driver_data to their private data. This probably needs
a clean up of its own.

Problem:
pch_udc continues operation even if VBUS becomes Low.
pch_udc performs D+ pulling up before VBUS becomes High.
USB device should be controlled according to VBUS state.
Root cause:
The current pch_udc is not always monitoring VBUS.
Solution:
The change of VBUS is detected using an interrupt of GPIO.
If VBUS became Low, pch_udc handles 'disconnect'.
After VBUS became High, a pull improves D+, and pch_udc
handles 'connect'.

Problem:
In USB Suspend, pch_udc handles 'disconnect'.
Root cause:
The current pch_udc is not monitoring VBUS.
When USB cable is disconnected, USB Device Controller generates
an interrupt of USB Suspend.
pch_udc cannot distinguish it is USB Suspend or disconnect.
Therefore, pch_udc handles 'disconnect' after an interrupt of
USB Suspend happend.
Solution:
VBUS is detected through GPIO.
After an interrupt produced USB Suspend, if VBUS is Low,
pch_udc handles 'disconnect'.
If VBUS is High, pch_udc handles 'suspend'.

If the stream check fails then we leave ep->desc assigend but we return
with an error code. The caller assumes the endpoint is not enabled
(which is the case) but it can not enable it again due to this
assigment.

Declare the fsg_module_parameters fields as "bool" so that they can match the types
passed in FSG_MODULE_PARAM_ARRAY macro.

Currently the UASP gadget fails to bind on an UDC which does not provide
stream support. This is true for all udc in tree except for dummy and
dwc3 since they don't support SuperSpeed.
There is no need to test for the availability of stream support on those
UDCs because we will never even try to use them. I think it is sane to
assume that StreamSupport is always available on SuperSpeed since it is
one of the key features.
The host side will only allocate on SS so this part is also fine.

The max_ep is the number of endpoint * 2.
But in dtd_complete_irq, it does again * 2, it will deference wrong memory
after scanning max_ep - 1.
The another similar problem is at USB_REQ_SET_FEATURE (the pipe number
should be 0 and max_ep - 1).

UDC core will call disconnect() and unbind() for us upon the gadget
removal, so we should not do it ourselves. Otherwise, a composite
gadget will explode, for example. Others might too.
This was introduced during conversion to new style gadget in 2c7f0989
(usb: gadget: langwell: convert to new style).

dummy_hcd provides (alloc|free)_stream() callbacks but there are not
doing anything. The transfer side also lacks matching of streams. This
patch changes this and implements stream allocation / de-allocation
support and proper urb <=> req matching.
The UDC side exposes a limit of 16 streams. DWC3, the only USB3 UDC has
no limitations in this regard except that it _needs_ to know that
streams will be used at the ep_enable time. At the host side, there is
no real limit either: XHCI can allocate any number of streams as long as
it does not run out of memory. The UAS gadget currently requests 16
streams and the UAS host side fallbacks from the requested 256 down to
16 which is fine.
From the UASP point of view (the only specified user), the number of
used streams does not really matter. The only limitation is that the
host may not use a higher stream than the gadget requested and can deal
with.
The dummy stream support has been modelled after current UAS + XHCI +
DWC3 + UASP usage which helps me testing:
- the device announces that each ep supports 16 streams (even it could
more than that).
- the device side looks into Companion descriptor at ep_enable time and
enables them according to it.
- the host side tries to enable the requested number of streams but the
upper limit is the Comanion descriptor. None (zero streams) is an
error condition, less is okay.

USB Suspend interrupts occur frequently.
CAUSE:
When it is called pch_udc_reconnect() in USB Suspend, it repeats reset and
Suspend.
SOLUTION:
pch_udc_reconnect() does not enable all interrupts. When an enumeration event
occurred the driver enables all interrupts.

ISSUE:
After a USB cable is connect/disconnected, the system rarely freezes.
CAUSE:
Since the USB device controller cannot know to disconnect the USB cable, when
it is used without detecting VBUS by GPIO, the UDC driver does not notify to
USB Gadget.
Since USB Gadget cannot know to disconnect, a false setting occurred when the
USB cable is connected/disconnect repeatedly.

While playing with uasp I noticed that it does not work with dummy_hcd.
The problem is that uasp requires a stream capable endpoint which it is
requesting at bind time like every other gadget. dummy_hcd however
initializes the max_stream value after connect once it knows if it runs
at SS or not.
I don't think that it is might be wrong to initialize the stream
capability even at HS speed. The gadget may not use this descriptor at
HS speed so it should not cause any damage.

We should not change gadget driver's descriptors just
because we think it's right to do so.
There are several of reasons which would support this
statement but it suffices to say that this was probably
never tested because it updates bmAttributes without
asking the driver if it's ok to do so.
This means that e.g. on UASP gadget it would enable
stream support even for the command endpoint which must
not have stream support enabled.
In fact, this change is fixing the bug introduced by
commit a59d6b9 (usb: gadget: add streams support to
the gadget framework) which was caught when testing
UASP gadget with dwc3 driver.

This patch impletments system suspend/resume functions for Marvell
otg controller.
If OTG is enabled, OTG driver will do most of the work.
If not, we will check clock gating.
If clock gating is enabled, the UDC will be start/stop automatically.
If not, UDC will be start/stop in suspend/resume functions.

This commit renames the “speed” field of the usb_gadget_driver
structure to “max_speed”. This is so that to make it more
apparent that the field represents the maximum speed gadget
driver can support.
This also make the field look more like fields with the same
name in usb_gadget and usb_composite_driver structures. All
of those represent the *maximal* speed given entity supports.
After this commit, there are the following fields in various
structures:
* usb_gadget::speed - the current connection speed,
* usb_gadget::max_speed - maximal speed UDC supports,
* usb_gadget_driver::max_speed - maximal speed gadget driver
supports, and
* usb_composite_driver::max_speed - maximal speed composite
gadget supports.

With Peiyu's patch "gadget: mass_storage: adapt logic block size to bound block
devices" (http://www.spinics.net/lists/linux-usb/msg50791.html), now mass storage
can adjust logic block size dynamically based on real devices.
Then there is one issue caused by it, if two luns have different logic block size,
mass storage can't work.
Let's check the current software flow:
1. get_next_command(): call received_cbw();
2. received_cbw(): update common->lun = cbw->Lun, but common->curlen is not updated;
3. do_scsi_command(): in READ_X and WRITE_X commands, common->data_size_from_cmnd is
updated by common->curlun->blkbits;
4. check_command(): update common->curlun according to common->lun
As you can see, the step 3 uses wrong common->curlun, then wrong common->curlun->blkbits.
If the two luns have same blkbits, there isn't issue. Otherwise, both will fail.
This patch moves the common->curlun update to step 1, then make sure step 3 gets right
blkbits and data_size_from_cmnd.

The ci13xxx_udc driver checks the number of endpoints in the udc
controller, however some routines expect that the hardware has 16
bidirectional endpoints.
This patch improves the driver to work on controllers with less than
16 endpoints like the udc controller found on freescale's
mx23 and mx28.

In order to support multiple SoC kernel image, compile-time dependency
on a specific SoC type should be avoided.
fsl_udc_clk_finalize is already protected by cpu_is_mx35(), so remove

Some ISO gadgets, like audio, has SYNC attribute as well as
USB_ENDPOINT_XFER_ISOC for their bmAttributes at ISO endpoint
descriptor. So, it needs to use usb_endpoint_xfer_isoc to judge
ISO XFER.

Several UDC drivers had a gadget driver's speed sanity check of the
form of:
driver->speed != USB_SPEED_HIGH
or:
driver->speed != USB_SPEED_HIGH && driver->speed != USB_SPEED_FULL
As more and more gadget drivers support USB SuperSpeed, driver->speed
may be set to USB_SPEED_SUPER and UDC driver should handle such gadget
correctly. The above checks however fail to recognise USB_SPEED_SUPER
as a valid speed.
This commit changes the two checks to:
driver->speed < USB_SPEED_HIGH
or:
driver->speed < USB_SPEED_FULL
respectively.

(Some drivers used a mix of these interfaces and alloc_pages())

This patch (as1502) removes the UTF8-to-UTF16 conversion routine in
the USB gadget library and replaces it with a call to the equivalent
function in the NLS library.
The only downside worth noting is that the NLS library routine
requires the output buffer to be 16-bit aligned. This is always true
in the gadget code, because the output buffer is always a
usb_request buffer being used to send a string descriptor.

The inline documentation of _gadget_stop_activity() states that the
function should be called holding the udc->lock. This however will
result in a deadlock, because _gadget_stop_activity() takes the udc->lock.
During normal operation _gadget_stop_activity() is always called unlocked,
but in ci13xxx_stop() it's called locked, this results in the following
deadlock during rmmod of a gadget driver.
This patch fixes the deadlock by calling _gadget_stop_activity() always
unlocked, the inline documentation is adjusted accordingly.

The current driver sets the request's dma addr (mReq->req.dma) to 0 to
mark the DMA address as not valid. However some gadget drivers
(e.g. gadgetfs) set the request's dma addr to DMA_ADDR_INVALID to mark
the address as invalid. This leads to bogus data send because the
ci13xxx_udc driver assumes the request has already been mapped.
This patch fixes the problem, by using DMA_ADDR_INVALID instead of 0
to mark the request's DMA address as invalid.

Under certain circumstances lockdep finds an inconsistent lock state in
gadgetfs. The problem can be reproduced with a hardware using the
ci13xxx_udc driver and the gadgetfs test program (needs a patch to support
the ci13xxx_udc, though):

The dynamic jack allocation of the MIDI gadget currently links all
external jacks to one single instance of an embedded jack. According to
the spec, this is only valid if these streams always carry the same data
stream, as described in the USB MIDI 1.0 spec, chapter 3.3.1.
Also, genius Windows 7(tm) terminates it's life cycle instantly with a
blue screen of death once a device with more than one input and output
port with the current implementation is connected.
While at it, and because it grew again by this change, allocate the
temporary function pointer list on the heap, not on the stack.

The USB-IF CV compliance tester is getting stricter, and it would
be valid for it to fail a mass-storage device that accepts an
invalid USB_BULK_RESET_REQUEST request. Although it doesn't do
that yet, let's be proactive and fix that now.

The udc-newstyle needs device_register in probe() of platform_device.
If it doesn't call, kernel panic happens in the sysfs_create_dir() when
we run modprobe a gadget driver.

usb_gadget_disconnect() is responsible of removing
data pullups. Before doing that we must, first, tell
gadget driver we're disconnecting (by calling disconnect
method on gadget driver structure), unbind the gadget
driver and stop the controller.

With module.h being implicitly everywhere via device.h, the absence
of explicitly including something for EXPORT_SYMBOL went unnoticed.
Since we are heading to fix things up and clean module.h from the
device.h file, we need to explicitly include these files now.
Use the lightweight version of the header that has just THIS_MODULE
and EXPORT_SYMBOL variants.

There is a race, reproduced rarely if you unload the module
when host finishes mass storage device initialization (reading
partition table and so on): fsg_unbind() code first closes
lun files then waits for worker thread to finish its work, as
the result the thread may operate on already closed device

The controller will prime failure sometimes when do the iperf test.

The ep enable / disable functions can be called from interrupt
context, and they are not race safe on SMP systems. The critical
data can be modified in more than one routing.
Make them race safe by using IRQ-safe spinlock functions.

For the code doesn't restrict controller ep must be ep0, so we will go
through all the eps and check if there is a setup package received.

Some platforms will use usb to download images, the controller may not
be stopped correctly when start kernel. In some cases, it may have some
pending interrupts, and they will be triggered immediately when we finish

This patch do the following things:
1. Add header and Copyright for marvell usb driver.
2. Add mv_usb.h in include/linux/platform_data, make the driver
fits all the marvell platform using the same ChipIdea usb ip.
3. Some SOC may has mutiple clock sources, so let me define it
in mv_usb_platform_data and give two helper functions named
udc_clock_enable/udc_clock_disable to deal with the clocks.
4. Different SOCs will have some difference in PHY initialization,
so we will remove file mv_udc_phy.c and add two funtions in
mv_usb_platform_data, let the platform relative driver to realize it.
5. Rewrite probe function according to the modification list above. Find
it will kernel panic when probe failed. The root cause is as follows:
When probe failed, the error handle may call device_unregister()
which in return will call gadget_release.In current code,
gadget_release have two issues:
1: the_controller is a NULL pointer.
2: if we free udc here, then the following code in probe
will access NULL pointer.

The Synopsys USB device controller requires all OUT transfer request
lengths to be aligned to max packet size. The mass storage gadgets do
not meet this requirement for Super Speed. The gadgets already have a
function which performs this alignment for CBW packets, so use it for
data packets too.
The alternative would be to implement bounce buffers in the DWC3
driver, but that could have a significant impact on performance.
This version is based upon a more-correct patch written by Alan
Stern.

The code in the MIDI gadget was already sort of prepared for multi-port
configuration, so the streaming logic itself didn't need much tweaking.
However, the descriptors change when the number of ports do, and so some
rework of the the preparation algorithms were necessary.
Successfully tested on Linux and Max OS X hosts for both input and
output streams.

to avoid code duplication and rename some symbols to bring them in sync

This patch adds f_midi.c to implement a USB gadget function that works
with the composite framework, so it can be combined with other USB
functions.
The code for the ALSA/MIDI logic was taken from the midi device gadget,
other parts have been rewritten to benefit from the dynamic descriptor
allocation features.
This was successfully tested on an OMAP3 board.

SH7757 has a USB function with internal DMA controller (SUDMAC).
This patch supports the SUDMAC. The SUDMAC is incompatible with
general-purpose DMAC. So, it doesn't use dmaengine.

R8A66597 has the pin of WR0 and WR1. So, if one write-pin of CPU
connects to the pins, we have to change the setting of FIFOSEL
register in the controller. If we don't change the setting,
the controller cannot send the data of odd length.

The allocated chardevice region range is only 1 device but on
unregister it currently tries to deregister 2.
Found this while doing a insmod/rmmod/insmod/rm... of the module

In a few places in the kernel, the code prints
a human-readable USB device speed (eg. "high speed").
This involves a switch statement sometimes wrapped
around in ({ ... }) block leading to code repetition.
To mitigate this issue, this commit introduces
usb_speed_string() function, which returns
a human-readable name of provided speed.
It also changes a few places switch was used to use
this new function. This changes a bit the way the
speed is printed in few instances at the same time
standardising it.

On Audio class, the wLength field of the Setup
packet, contains the data payload size of the
following Data phase. Instead of harcoding values,
use wLength.
This also fixes a bug where Gadget driver had to
receive 3 bytes, but it was queueing a ZLP.

While testing g_audio with HighSpeed UDC on a
FS Hub, we had no configurations to present to
the host. That's because both speeds where
mutually exclusive.

FSG_NUM_BUFFERS is set to 2 as default.
Usually 2 buffers are enough to establish a good buffering pipeline.
The number may be increased in order to compensate a for bursty VFS
behaviour.
Here follows a description of system that may require more than
2 buffers.

* latency cost for wake up and/or frequency change
* DMA for IO

* Data transfer from MMC via VFS to USB.
* DMA shuffles data from MMC and to USB.

This patch (as1481) fixes a problem affecting g_file_storage and
g_mass_storage when running at SuperSpeed. The two drivers currently
assume that the bulk-out maxpacket size can evenly divide the SCSI
block size, which is 512 bytes. But SuperSpeed bulk endpoints have a
maxpacket size of 1024, so the assumption is no longer true.
This patch removes that assumption from the drivers, by getting rid of
a small optimization (they try to align VFS reads and writes on page
cache boundaries). If a command's starting logical block address is
512 bytes below the end of a page, it's not okay to issue a USB
command for just those 512 bytes when the maxpacket size is 1024 -- it
would result in either babble (for an OUT transfer) or a short packet
(for an IN transfer).
Also, for backward compatibility, the test for writes extending beyond
the end of the backing storage has to be changed. If the host tries
to do this, we should accept the data that fits in the backing storage
and ignore the rest. Because the storage's end may not align with a
USB packet boundary, this means we may have to accept a USB OUT
transfer that extends beyond the end of the storage and then write out
only the part of the data that fits.

Now the mass storage driver has fixed logic block size of 512 bytes.
The mass storage gadget read/write bound devices only through VFS, so the
bottom level devices actually are just RAW devices to the driver and connected
PC. As a RAW, hosts can always format, read and write it right in 512 bytes
logic block and don't care about the actual logic block size of devices bound
to the gadget.
But if we want to share the bound block device partition between target board
and PC, in case the logic block size of the bound block device is 4KB, we
execute the following steps:
1. connect a board with mass storage gadget to PC(the board has set one
partition of on-board block device as file name of the mass storage)
2. PC format the mass storage to VFAT by default logic block size and
read/write it
3. disconnect boards from PC
4. target board mount the partition as VFAT
Step 4 will fail since kernel on target thinks the logic block size of the
bound partition as 4KB.
A typical error is "FAT: logical sector size too small for device (logical
sector size = 512)"
If we execute opposite steps:
1. format the partition to VFAT on target board and read/write this partition
2. connect the board to Windows PC as usb mass storage gadget, windows will
think the disk is not formatted
So the conclusion is that only as a gadget, the mass storage driver has no any
problem. But being shared VFAT or other filesystem on PC and target board, it
will fail.
This patch adapts logic block size to bound block devices and fix the issue.

The usb-interrupt is requested before the endpoints are initalised.
If an interrupt happens in the time between request_irq and the init
of the endpoint-data (as seen on the Qisda ESx00 ebook-platforms),
it is therefore possible for the interrupt handler to access endpoint-
data before its creation resulting in a null-pointer dereference.
This patch simply moves the irq request below the endpoint init.

The DesignWare USB3 is a highly
configurable IP Core which can be
instantiated as Dual-Role Device (DRD),
Peripheral Only and Host Only (XHCI)
configurations.
Several other parameters can be configured
like amount of FIFO space, amount of TX and
RX endpoints, amount of Host Interrupters,
etc.
The current driver has been validated with
a virtual model of version 1.73a of that core
and with an FPGA burned with version 1.83a
of the DRD core. We have support for PCIe
bus, which is used on FPGA prototyping, and
for the OMAP5, more adaptation (or glue)
layers can be easily added and the driver
is half prepared to handle any possible
configuration the HW engineer has chosen
considering we have the information on
one of the GHWPARAMS registers to do
runtime checking of certain features.
More runtime checks can, and should, be added
in order to make this driver even more flexible
with regards to number of endpoints, FIFO sizes,
transfer types, etc.
While this supports only the device side, for
now, we will add support for Host side (xHCI -
see the updated series Sebastian has sent [1])
and OTG after we have it all stabilized.

This is a patch to fix an issue with the HID gadget which, at the moment,
returns STALL on a HID descriptor request. Essentially, the patch changes
the hid gadget such that a request for the HID descriptor is handled by
copying the descriptor into the response buffer, rather than falling
through the default case, in which the request is answered by a STALL.

options

to see Inventra options", even though you now need to enable
both of them to see Inventra options.
Fix the dependency and drop the anyway unnecessary comment.

specifies the exponent instead of the actual size in bytes. The only
valid value on SS is 9 which denotes 512 bytes.

The driver had to decide how many events to allocate when the v4l2_fh struct
was created. It was possible to add more events afterwards, but there was no
way to ensure that you wouldn't miss important events if the event queue
would fill up for that filehandle.
In addition, once there were no more free events, any new events were simply
dropped on the floor.
For the control event in particular this made life very difficult since
control status/value changes could just be missed if the number of allocated
events and the speed at which the application read events was too low to keep
up with the number of generated events. The application would have no idea
what the latest state was for a control since it could have missed the latest
control change.
So this patch makes some major changes in how events are allocated. Instead
of allocating events per-filehandle they are now allocated when subscribing an
event. So for that particular event type N events (determined by the driver)
are allocated. Those events are reserved for that particular event type.
This ensures that you will not miss events for a particular type altogether.
In addition, if there are N events in use and a new event is raised, then
the oldest event is dropped and the new one is added. So the latest event
is always available.
This can be further improved by adding the ability to merge the state of
two events together, ensuring that no data is lost at all. This will be
added in the next patch.
This also makes it possible to allow the user to determine the number of
events that will be allocated. This is not implemented at the moment, but
would be trivial.

Drivers that supported events used to be rare, but now that controls can also
raise events this will become much more common since almost all drivers have
controls.
This means that keeping struct v4l2_events as a separate struct make no more
sense. Merging it into struct v4l2_fh simplifies things substantially as it
is now an integral part of the filehandle struct.

Fix up trivial conflicts in fs/xfs/linux-2.6/xfs_super.c due to the new
shrinker callout for the inode cache, that clashed with the xfs code to
start the periodic workers later.
Btrfs needs to be able to control how filemap_write_and_wait_range() is called
in fsync to make it less of a painful operation, so push down taking i_mutex and
the calling of filemap_write_and_wait() down into the ->fsync() handlers. Some
file systems can drop taking the i_mutex altogether it seems, like ext3 and
ocfs2. For correctness sake I just pushed everything down in all cases to make
sure that we keep the current behavior the same for everybody, and then each
individual fs maintainer can make up their mind about what to do from there.

M66592 has the pin of WR0 and WR1. So, if one write-pin of CPU
connects to the pins, we have to change the setting of FIFOSEL
register in the controller. If we don't change the setting,
the controller cannot send the data of odd length.

This patch rescues renesas_usbhs compile from
commit 193ab2a (usb: gadget: allow multiple gadgets to be built)
CONFIG_USB_RENESAS_USBHS compile renesas_usbhs main code which
is shared between Host/Gadget.
CONFIG_USB_RENESAS_USBHS_UDC add mod_gadget to it.
It had lost USB_GADGET_DUALSPEED

This has been lightly tested using a Linux host.

didn't support it. When we sent the SET_FEATURE for TEST_MODE to
the driver, the request was successful, but the module didn't enter
the TEST_MODE.

BUSWAIT is a 4-bit-wide value that controls the number of access waits
from the CPU to on-chip USB module. b'0000 inserts 0 wait (2 access
cycles) and b'1111 inserts 15 waits (17 access cycles, hardware
initial value), respectively.
BUSWAIT value depends on peripheral clock frequency supplied to on-chip
of each CPU, hence should be configurable through platform data.
Note that this patch assumes that b'0000 (0 wait, 2 access cycles) is
rerely used and considered as invalid. If valid 'buswait' data is not
provided by platform, initial b'1111 (15 waits, 17 access cycles) will
be applied as a safe default.

When we run rmmod a gadget driver, the driver will call
disable_controller(). Then, because the bit of USBE in SYSCFG0 was
cleared in on_chip=1 mode, we could not connect the usb when we run
insmod a gadget driver next time.
This patch also cleans up probe() and ->stop() about unnecessary
init_controller().

- remove pointer u32 abuse in fusb300_fill_idma_prdtbl().
It is assigned the dma_addr to a pointer and then back.
Poor families may have to recycle variables but we don't
- don't free req.buf in error case. We don't do it in the
ok case so it is probably wrong to do it in error case.
- return in error case. There is no reason to continue
without data and performing ops on an invalid pointer.
- The if (d) statement is bogus since an invalid DMA pointer
is ~0 on some architecutres. And since we return for the
invalid case we don't need it.

There is a bug in Samsung's UDC driver, which is completely disabling
the USB device when a custom UDC command is used.
Following patch seems to get the right behavior (e.g. enabling pull-up
instead of disabling then Vcc is applied).
