d4450261e5 ARM: CPU hotplug: fix reporting of spurious wakeups
The original scheme for reporting spurious wakeups was broken - it
tried to use printk() from a context which wasn't coherent with the
other CPUs, which risks corrupting the printk() data.

Fix this by noting the number spurious wakeups, and only report them
when we are properly woken - when we will be coherent with the rest
of the system.

3705ff6da5 ARM: Fix subtle race in CPU pen_release hotplug code
There is a subtle race in the CPU hotplug code, where a CPU which has
been offlined can online itself before being requested, which results
in things going astray on the next online/offline cycle.

What happens in the normal online/offline/online cycle is:

	CPU0			CPU3
	requests boot of CPU3
	pen_release = 3
	flush cache line
				checks pen_release, reads 3
				starts boot
				pen_release = -1
	... requests CPU3 offline ...
				... dies ...
				checks pen_release, reads -1
	requests boot of CPU3
	pen_release = 3
	flush cache line
				checks pen_release, reads 3
				starts boot
				pen_release = -1

However, as the write of -1 of pen_release is not fully flushed back to
memory, and the checking of pen_release is done with caches disabled,
this allows CPU3 the opportunity to read the old value of pen_release:

	CPU0			CPU3
	requests boot of CPU3
	pen_release = 3
	flush cache line
				checks pen_release, reads 3
				starts boot
				pen_release = -1
	... requests CPU3 offline ...
				... dies ...
				checks pen_release, reads 3
				starts boot
				pen_release = -1
	requests boot of CPU3
	pen_release = 3
	flush cache line

Fix this by grouping the write of pen_release along with its cache line
flushing code to ensure that any update to pen_release is always pushed
out to physical memory.

86e62b9336 ARM: SMP: remove smp_mpidr.h

now do not use hard_smp_processor_id(), we no longer need to read the
hardware processor ID.  Remove the include providing this function.

bbc81fd432 ARM: CPU hotplug: remove bug checks in platform_cpu_die()
platform_cpu_die() is entered from the CPU's own idle thread, which
can not be migrated to other CPUs.  Moreover, the 'cpu' argument
comes from the thread info, which will always be the 'current'
CPU.  So remove this useless bug check.

3c030beabf ARM: CPU hotplug: move cpu_killed completion to core code
We always need to wait for the dying CPU to reach a safe state before
taking it down, irrespective of the requirements of the platform.
Move the completion code into the ARM SMP hotplug code rather than
having each platform re-implement this.

05c74a6cbc ARM: SMP: consolidate the common parts of smp_prepare_cpus()
There is a certain amount of smp_prepare_cpus() which doesn't belong
in the platform support code - that is, code which is invariant to the
SMP implementation.  Move this code into arch/arm/kernel/smp.c, and
add a platform_ prefix to the original function.

8975b6c0fd ARM: SMP: Clean up ncores sanity checks
scu_get_core_count() never returns zero cores, so we don't need to
check and correct if ncores is zero.

Tegra was missing the check against NR_CPUS, leading to a potential
bitfield overflow if this becomes the case.

7627dc802a ARM: GIC: private a standard get_irqnr_preamble assembler macro
Provide a standard get_irqnr_preamble assembler macro for platforms
to use, which retrieves the base address of the GIC CPU interface
from gic_cpu_base_addr.  Allow platforms to override this by defining
HAVE_GET_IRQNR_PREAMBLE.

ff2e27ae0b ARM: GIC: consolidate gic_cpu_base_addr to common GIC code
Every architecture using the GIC has a gic_cpu_base_addr pointer for
GIC 0 for their entry assembly code to use to decode the cause of the
current interrupt.  Move this into the common GIC code.

384895330e ARM: GIC: Remove MMIO address from gic_cpu_init, rename to gic_secondary_init
We don't need to re-pass the base address for the CPU interfaces to the
GIC for secondary CPUs, as it will never be different from the boot CPU
- and even if it was, we'd overwrite the boot CPU's base address.

Get rid of this argument, and rename to gic_secondary_init().

This allows us to use smp_cross_call() to trigger a number of different
software generated interrupts, rather than combining them all on one

725ca4adae ARM: 6507/1: RealView: Correct data alignment in headsmp.S for CONFIG_THUMB2_KERNEL
Directives such as .long and .word do not magically cause the
assembler location counter to become aligned in gas.  As a result,
using these directives in code sections can result in misaligned data
words when building a Thumb-2 kernel (CONFIG_THUMB2_KERNEL).

This is a Bad Thing, since the ABI permits the compiler to assume that
fundamental types of word size or above are word- aligned when
accessing them from C.  If the data is not really word-aligned, this
can cause impaired performance and stray alignment faults in some
circumstances.

In general, the following rules should be applied when using data word
declaration directives inside code sections:

    * .quad and .double:
         .align 3

    * .long, .word, .single, .float:
         .align (or .align 2)

    * .short:
        No explicit alignment required, since Thumb-2
        instructions are always 2 or 4 bytes in size.
        immediately after an instruction.

6451d7783b arm: remove machine_desc.io_pg_offst and .phys_io
Since we're now using addruart to establish the debug mapping, we can
remove the io_pg_offst and phys_io members of struct machine_desc.

The various declarations were removed using the following script:

0ea1293009 arm: return both physical and virtual addresses from addruart
Rather than checking the MMU status in every instance of addruart, do it
once in kernel/debug.S, and change the existing addruart macros to
return both physical and virtual addresses. The main debug code can then
select the appropriate address to use.

This will also allow us to retreive the address of a uart for the MMU
state that we're not current in.

2971944582 ARM: 6307/1: mmci: allow the card detect GPIO value not to be inverted
On some platforms, the GPIO value from the gpio_cd pin doesn't need to
be inverted to get it active high.  Add a cd_invert platform data
parameter and change existing platforms using GPIO for CD (only
Realview) to enable it.

06385e4909 ARM: 6278/2: fix regression in RealView after the introduction of pclk
The patch to add the apb_pclk to the AMBA/PrimeCell bus broke
RealView, since the clockdevice is not registered at probe() time.
This moves clock initialization to a core_initcall()

74bc80931c ARM: Fix Versatile/Realview/VExpress MMC card detection sense
The MMC card detection sense has become really confused with negations
at various levels, leading to some platforms not detecting inserted
cards.  Fix this by converting everything to positive logic throughout,
thereby getting rid of these negations.

48f1d5a3ce ARM: 6204/1: Fixups for the RealView PB1176
This is a number of basic fixes to the PB1176 that makes it tick
properly:

- Detect MMC insertion/removal even when PL061 GPIO is not compiled
  in. The register to read this status directly is removed on the
  PB1176.
- Define the UART3 on the DevChip (where is actually is) and define
  the new UART4 serial port on the FPGA.
- Also define the clocks for these two UARTs apropriately.
- Remove the false notion that the PB1176 should have its CLCD
  on the ISSP, this is not the case, it is in the DevChip.
- Remove the defintions and the previously commented-out PL081
  DMAC. As confirmed by mail this was found to be broken on the
  PB1176 board and removed from the subsequent FPGA images.

2503a5ecd8 ARM: 6201/1: RealView: Do not use outer_sync() on ARM11MPCore boards with L220
RealView boards with certain revisions of the L220 cache controller (ARM11*
processors only) may have issues (hardware deadlock) with the recent changes to
the mb() barrier implementation (DSB followed by an L2 cache sync). The patch
redefines the RealView ARM11MPCore mandatory barriers without the outer_sync()
call.

3081e43b97 ARM: 5890/1: Fix incorrect Realview board IRQs for L220 and PMU
In anticipation of PMU support for Realview boards, the IRQs defined
for some of these boards need updating.

This patch removes incorrect L220 IRQ definitions, corrects incorrect PMU
