These modes are not necessarily for OOB only. Particularly, MTD_OOB_RAW
affected operations on in-band page data as well. To clarify these
options and to emphasize that their effect is applied per-operation, we
change the primary prefix to MTD_OPS_.

The older add_mtd_device()/add_mtd_partitions() and their removal
counterparts will soon be gone. Replace uses with mtd_device_register()
and mtd_device_unregister().

2GB xD card, and 4MB SmartMedia ROM card share same ID, so to make both work
split xD and smartmedia ID tables.
Hardware driver must be able to know which type it handles (and probably just one).
