ppa cannot handle highmem pages, and like imm, which already has
this patch, the device is slow, so performance is not a big issue,
so just force pages to be in low memory (hence mapped).

Wrap a highly common idiom. Makes the code easier to read, helps pave
the way for sdev->{id,channel} removal, and adds a token that can easily
by grepped-for in the future.
There are a couple sdev_id() and scmd_printk() updates thrown in as well.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
