Eric Lescouet 27729aadd3 USB: make hcd.h public (drivers dependency)
The usbcore headers: hcd.h and hub.h are shared between usbcore,
HCDs and a couple of other drivers (e.g. USBIP modules).
So, it makes sense to move them into a more public location and
to cleanup dependency of those modules on kernel internal headers.
This patch moves hcd.h from drivers/usb/core into include/linux/usb/

Christoph Egger 8af6096caf USB: remove obsolete config in kernel source (USB_HCD_DMA)
The configuration Option USB_HCD_DMA is not reachable in KConfig so
this piece of Code is effectively dead and useless. Remove it to avoid
confusion.

Lothar Wassmann 0a2fea2e0d USB: isp1362: fix build failure on ARM systems via irq_flags cleanup
There was some left over #ifdef ARM logic that is outdated but no one
really noticed.  So instead of relying on this tricky logic, properly
load and utilize the platform irq_flags resources.

Lothar Wassmann 96b85179b4 USB: isp1362: better 64bit printf warning fixes
Some hosts that treat the return value of sizeof differently from unsigned
long might still hit warnings.  So use %zu for sizeof() values.  This is a
better version of the previous commit b0a9cf297e.

Ken MacLeod 7949f4e164 USB: isp1362: fix pulldown register defines and conf logic
HCHWCFG_PULLDOWN_DS2 and HCHWCFG_PULLDOWN_DS1 were swapped.  Incorrect
operator precedence in isp1362_hc_start() hid part of the problem.
This fixes a problem where Port 1 in Host mode fails to see disconnects.
