A big changeset, but it's all pretty dumb.
struct rds_rdma_op was already embedded in struct rm_rdma_op.
Remove rds_rdma_op and put its members in rm_rdma_op. Rename
members with "op_" prefix instead of "r_", for consistency.
Of course this breaks a lot, so fixup the code accordingly.

This eliminates a separate memory alloc, although
it is now necessary to add an "r_active" flag, since
it is no longer to use the m_rdma_op pointer as an
indicator of if an rdma op is present.
rdma SGs allocated from rm sg pool.
rds_rm_size also gets bigger. It's a little inefficient to
run through CMSGs twice, but it makes later steps a lot smoother.

BUGging on a runtime error code should be avoided. This

The existing code treated page_shift as a variable, when in fact we
always want to have the fastreg page size be the same as the arch's
page size -- and it is, so this doesn't need to be a variable.

The RDS_LL_SEND_FULL bit should be set when we stop transmitted due to
flow control. Otherwise the send worker will keep trying as opposed to
sleeping until we unthrottle. Saves CPU.

Support for iWARP NICs is implemented as a separate
RDS transport from IB. The code, however, is very
similar to IB (it was forked, basically.) so let's keep
it in one changeset.
The reason for this duplicationis that despite its similarity
to IB, there are a number of places where it has different
semantics. iwarp zcopy support is still under development,
and giving it its own sandbox ensures that IB code isn't
disrupted while iwarp changes. Over time these transports
will re-converge.
