This patch does a few things.
1) Makes /sys/bus/fcoe/ctlr_{create,destroy} interfaces.
These interfaces take an <ifname> and will either
create an FCoE Controller or destroy an FCoE
Controller depending on which file is written to.
The new FCoE Controller will start in a DISABLED
state and will not do discovery or login until it
is ENABLED. This pause will allow us to configure
the FCoE Controller before enabling it.
2) Makes the 'mode' attribute of a fcoe_ctlr_device
writale. This allows the user to configure the mode
in which the FCoE Controller will start in when it
is ENABLED.
Possible modes are 'Fabric', or 'VN2VN'.
The default mode for a fcoe_ctlr{,_device} is 'Fabric'.
Drivers must implement the set_fcoe_ctlr_mode routine
to support this feature.
libfcoe offers an exported routine to set a FCoE
Controller's mode. The mode can only be changed
when the FCoE Controller is DISABLED.
This patch also removes the get_fcoe_ctlr_mode pointer
in the fcoe_sysfs function template, the code in
fcoe_ctlr.c to get the mode and the assignment of
the fcoe_sysfs function pointer to the fcoe_ctlr.c
implementation (in fcoe and bnx2fc). fcoe_sysfs can
return that value for the mode without consulting the
LLD.
3) Make a 'enabled' attribute of a fcoe_ctlr_device. On a
read, fcoe_sysfs will return the attribute's value. On
a write, fcoe_sysfs will call the LLD (if there is a
callback) to notifiy that the enalbed state has changed.
This patch maintains the old FCoE control interfaces as
module parameters, but it adds comments pointing out that
the old interfaces are deprecated.

QUEUE_FLAG_DEAD is used to indicate that queuing new requests must
stop. After this flag has been set queue draining starts. However,
during the queue draining phase it is still safe to invoke the
queue's request_fn, so QUEUE_FLAG_DYING is a better name for this
flag.

Instead of creating a structure with an enum and a pointer
to a string, simply allocate an array of strings and use
the enum values for the indicies.
This means that we do not need to iterate through the list
of entries when looking up a string name by its enum key.
This will also help with a latter patch that will add
more fcoe_sysfs attributes that will also use the
fcoe_enum_name_search macro. One attribute will also do
a reverse lookup which requires less code when the
enum-to-string mappings are organized as this patch makes
them to be.

Currently fc_fcp_timeout doesn't check FC_RP_FLAGS_REC_SUPPORTED
flag first, this prevents REC request ever going out at all
to the target having REC support. So this patches fixes the
fc_fcp_timeout by checking FC_RP_FLAGS_REC_SUPPORTED flag first.
The changed order won't cause any issue during clearing
FC_RP_FLAGS_REC_SUPPORTED on failed IO with target not supporting
FC_RP_FLAGS_REC_SUPPORTED, since retry on failed IO would succeed.

megaraid use INIT_WORK to declare a hotplug_work, but cast the
hotplug_work from work_struct to delayed_work and
schedule_delayed_work on it. This is very dangerous, as other part of
delayed_work might be kernel memories allocated by others.
With commit 8852aac ("workqueue: mod_delayed_work_on() shouldn't queue
timer on 0 delay"), schedule_delayed_work() will check dwork->timer
before queue_work even when @delay is 0, this causes megaraid code to
hit the BUG_ON() in workqueue code. Change megaraid code to use
delayed work.

Eliminate the private_rport_attrs[] array and the SETUP_*() macros
used to set up that array since the information in that array
duplicates the information in the static device attributes. Also,
verify whether SRP_RPORT_ATTRS is large enough since it is easy to
forget to update that macro when adding new attributes.

Register transport attributes after the attribute array has been set
up instead of before. The current code can trigger a race condition
because the code reading the attribute array can run on another thread
than the code that initialized that array. Make sure that any code
reading the attribute array will see all values written into that
array.

When the qla2xxx driver loses access to multiple, remote ports, there is a race
condition which can occur which will keep the request stuck on a scsi request
queue indefinitely.
This bad state occurred do to a race condition with how the FCPORT_UPDATE_NEEDED
bit is set in qla2x00_schedule_rport_del(), and how it is cleared in
qla2x00_do_dpc(). The problem port has its drport pointer set, but it has never
been processed by the driver to inform the fc transport that the port has been
lost. qla2x00_schedule_rport_del() sets drport, and then sets the
FCPORT_UPDATE_NEEDED bit. In qla2x00_do_dpc(), the port lists are walked and
any drport pointer is handled and the fc transport informed of the port loss,
then the FCPORT_UPDATE_NEEDED bit is cleared. This leaves a race where the
dpc thread is processing one port removal, another port removal is marked
with a call to qla2x00_schedule_rport_del(), and the dpc thread clears the
bit for both removals, even though only the first removal was actually
handled. Until another event occurs to set FCPORT_UPDATE_NEEDED, the later
port removal is never finished and qla2xxx stays in a bad state which causes
requests to become stuck on request queues.
This patch updates the driver to test and clear FCPORT_UPDATE_NEEDED
atomically. This ensures the port state changes are processed and not lost.

FC transport on receiving bsg_job submission failure, calls bsg_job->job_done()
and sets the bsg_job->reply->result the returned value. In contrast, when the
success code (0) is returned fc transport doesn't call bsg_job->job_done() and
doesn't populate bsg_job->reply->result.

Update sd driver to use the callbacks defined in dev_pm_ops.
sd_freeze is NULL, the bus level callback has taken care of quiescing
the device so there should be nothing needs to be done here.
Consequently, sd_thaw is not needed here either.
suspend, poweroff and runtime suspend share the same routine sd_suspend,
which will sync flush and then stop the drive, this is the same as before.
resume, restore and runtime resume share the same routine sd_resume,
which will start the drive by putting it into active power state, this
is also the same as before.

Use of pm_message_t is deprecated and device driver is not supposed
to use that. This patch migrates the SCSI bus level pm callbacks
to call device's pm callbacks defined in its driver's dev_pm_ops.
This is achieved by finding out which device pm callback should be used
in bus callback function, and then pass that callback function pointer
as a param to the scsi_bus_{suspend,resume}_common routine, which will
further pass that callback to scsi_dev_type_{suspend,resume} after
proper handling.
The special case for freeze in scsi_bus_suspend_common is not necessary
since there is no high level SCSI driver has implemented freeze, so no
need to runtime resume the device if it is in runtime suspended state
for system freeze, just return like the system suspend/hibernate case.
Since only sd has implemented drv->suspend/drv->resume, and I'll update
sd driver to use the new callbacks in the following patch, there is no
need to fallback to call drv->suspend/drv->resume if dev_pm_ops is NULL.

When device is runtime suspended, put it to stopped power state to save
some power.
This will also make the behaviour consistent with what the scsi_pm.c
thinks about sd as the comment says:
sd treats runtime suspend, system suspend and system hibernate identical.
With this patch, it is now identical.
And sd_shutdown will also do nothing when it finds the device has been
runtime suspended, if we do not spin down the disk in runtime suspend
by putting it into stopped power state, the disk will be shut down
incorrectly.
And the the same problem can be solved for runtime power off after
runtime suspended case by this change.
With the current runtime scheme for disk, it will only be runtime
suspended when no process opens the disk, so this shouldn't happen a
lot, which makes it acceptable to spin down the disk when runtime
suspended. If some day a more aggressive runtime scheme is used, like
the 'request based runtime pm for disk' that Alan Stern and Lin Ming
has been working, we can introduce some policy to control this. But for
now, make it simple and correct by spinning down the disk.

The macro bit(n) is defined as ((u32)1 << n), and thus it doesn't work
with n >= 32, such as in mvs_94xx_assign_reg_set():
if (i >= 32) {
mvi->sata_reg_set |= bit(i);
...
}
The shift ((u32)1 << n) with n >= 32 also leads to undefined behavior.
The result varies depending on the architecture.
This patch changes bit(n) to do a 64-bit shift. It also simplifies
mv_ffc64() using __ffs64(), since invoking ffz() with ~0 is undefined.

store_host_reset() has tried to re-invent the wheel to compare sysfs strings.
Unfortunately it did so poorly and never bothered to check the input from
userspace before overwriting stack with it, so something simple as:

Force large capacity (> 0xFFFFFFFF blocks) drives to use READ/WRITE(16) instead
of READ/WRITE(10). Some(most/all?) USB enclosures do not like READ(10) commands

virtscsi_queuecommand was leaking memory when the virtio queue was full.

Fixed boot from san failure when SLI4 FC device presented on the same PCI bus
The request_firmware interface can induce delays while looking
for firmware files, even if no fw file is present. In some situations
the delays exceeded scan_wait timeouts, resulting in situations in which
the boot device had not been discovered in time. Boot Device does not
need to be on a lpfc device.
Change request_firmware use to be module paramater driven. Default is to
not attempt firmware download on boot. Add sysfs parameter to invoke
firmware update.

Removed the individual PCI DEVICE ID checking inside bnx2i. The device
type can easily be read from the corresponding cnic->flags. This will
free bnx2i from having to get updated for every new device ID that gets
added.

The MBX timeout value set to 100 and if adapter doesn;t
return response in that time driver will return from waiting
for completion with an error to the caller. In the earlier code
driver use to wait until MBX response comes from adapter.

Kernel used to panic while running IO is disable mode, as there
was an issue with getting the correct EQ on which completion has come.
Fix done is create workqueue per hba and work item for each EQ created.

This patch adds the support for the following two read-only sysfs attributes
to scsi_osd class members : osdname & systemid
These attributes will show up as below in sysfs class hierarchy:
/sys/class/scsi_osd/osdX/osdname
/sys/class/scsi_osd/osdX/systemid
The osdname & systemid are OSD device attributes which uniquely
identify a device on the network, while it's IP and certainly
it's /dev/osdX device path might change.
Userspace utilities (e.g. mkfs.exofs) can parse these attributes to
identify the correct OSD in safer and faster way.
(Today osd apps open each device in the system and send a
attributes query for these, in order to access the user
requested device)

scsi_register_driver will register a prep_fn() function, which
in turn migh need to use the sd_cdp_pool for DIF.
Which hasn't been initialised at this point, leading to
a crash. So reshuffle the init_sd() and exit_sd() paths
to have the driver registered last.

Implement support for WRITE SAME(10) and WRITE SAME(16) in the SCSI disk
driver.
- We set the default maximum to 0xFFFF because there are several
devices out there that only support two-byte block counts even with
WRITE SAME(16). We only enable transfers bigger than 0xFFFF if the
device explicitly reports MAXIMUM WRITE SAME LENGTH in the BLOCK
LIMITS VPD.
- max_write_same_blocks can be overriden per-device basis in sysfs.
- The UNMAP discovery heuristics remain unchanged but the discard
limits are tweaked to match the "real" WRITE SAME commands.
- In the error handling logic we now distinguish between WRITE SAME
with and without UNMAP set.
The discovery process heuristics are:
- If the device reports a SCSI level of SPC-3 or greater we'll issue
READ SUPPORTED OPERATION CODES to find out whether WRITE SAME(16) is
supported. If that's the case we will use it.
- If the device supports the block limits VPD and reports a MAXIMUM
WRITE SAME LENGTH bigger than 0xFFFF we will use WRITE SAME(16).
- Otherwise we will use WRITE SAME(10) unless the target LBA is beyond
0xFFFFFFFF or the block count exceeds 0xFFFF.
- no_write_same is set for ATA, FireWire and USB.

The REPORT SUPPORTED OPERATION CODES command can be used to query
whether a given opcode is supported by a device. Add a helper function
that allows us to look up commands.
We only issue RSOC if the device reports compliance with SPC-3 or
later. But to err on the side of caution we disable the command for ATA,
FireWire and USB.

SATA MICROCODE DOWNALOAD fails on isci driver. After receiving Register
Device to Host (FIS 0x34) frame Initiator resets phy.
In the frame handler routine response (FIS 0x34) was copied into wrong
buffer and upper layer did not receive any answer which resulted in
timeout and reset.
This patch corrects this bug.

Pass the sense reason as an explicit return value from the I/O submission
path instead of storing it in struct se_cmd and using negative return

The problem is that of_device_id->data is a const void pointer.
This is pretty silly in this specific instance, because for all
matched device IDs we set match->data to the same value,
&qpti_template.

typed abstraction.

It is possible for the target code to change the loop_id or s_id of a
target session in reaction to an FC fabric change. However, the
session structures are stored in tables that are indexed by these two
keys, and if we just change the session structure but leave the
pointers to it in the old places in the table, havoc can ensue. For
example, a new session might come along that should go in the old slot
in the table and overwrite the old session pointer.
To handle this, add a new tgt_ops->update_sess() method that also
updates the "by loop_id" and "by s_id" lookup tables when a session
changes, so that the keys where a session pointer is stored in these
tables always matches the keys in the session structure itself.

My draft of SPC-4 says the following about the SCSI name string in
inquiry VPD page 83h:
The SCSI NAME STRING field starts with either:
a) the four UTF-8 characters 'eui.' concatenated with 16, 24, or
32 hexadecimal digits (i.e., the UTF-8 characters 0 through 9
and A through F) for an EUI-64 based identifier (see
7.8.6.5). The first hexadecimal digit shall be the most
significant four bits of the first byte (i.e., most significant
byte) of the EUI-64 based identifier;
b) the four UTF-8 characters 'naa.' concatenated with 16 or 32
hexadecimal digits for an NAA identifier (see 7.8.6.6). The
first hexadecimal digit shall be the most significant four bits
of the first byte (i.e., most significant byte) of the NAA
identifier; or
c) the four UTF-8 characters 'iqn.' concatenated with an iSCSI
Name for an iSCSI-name based identifier (see iSCSI).
However, the .tpg_get_wwn method for tcm_qla2xxx formats the WWN so
the SCSI name string looks like "52:4a:93:7d:24:5f:b2:12,t,0x0001".
This patch corrects the code so that VPD 83h gives a SPC-compliant
SCSI name string like "naa.524a937d245fb212,t,0x0001" while leavig
other uses alone (so configfs will still work with ':' separated WWNs).

The ncr5380 driver is included by multiple board specific
drivers, which may or may not use the interrupt handler.
The oak variant doesn't, and should set the DONT_USE_INTR
macro.

Properly account for I/O in transit before returning from the RESET call.
In the absense of this patch, we could have a situation where the host may
respond to a command that was issued prior to the issuance of the RESET
command at some arbitrary time after responding to the RESET command.
Currently, the host does not do anything with the RESET command.

Currently it is possible to unmap one more block than user requested to
due to the off-by-one error in unmap_region(). This is probably due to
the fact that the end variable despite its name actually points to the
last block to unmap + 1. However in the condition it is handled as the
last block of the region to unmap.
The bug was not previously spotted probably due to the fact that the
region was not zeroed, which has changed with commit
be1dd78de5. With that commit we were able
to corrupt the ext4 file system on 256M scsi_debug device with LBPRZ
enabled using fstrim.
Since the 'end' semantic is the same in several functions there this
commit just fixes the condition to use the 'end' variable correctly in
that context.

This patch tries to shorten the path length of scsi_cmd_to_driver(). As only
REQ_TYPE_BLOCK_PC commands can be submitted without a driver, so we could
avoid the related NULL checking, as long as we make sure we don't use it for
REQ_TYPE_BLOCK_PC type commands. Plus, this fixes a bug where you get
different behaviors from REQ_TYPE_BLOCK_PC commands when a driver is and isn't
attached.

Support the LUN parameter change event. Currently, the host fires this event
when the capacity of a disk is changed from the virtual machine monitor.
The resize then appears in the kernel log like this:

virtio-scsi needs to report LUNs greater than 256 using the "flat"
format. Because the Linux SCSI layer just maps the SCSI LUN to
an u32, without any parsing, these end up in the range from 16640
to 32767. Fix max_lun to account for the possibility that logical
unit numbers are encoded with the "flat" format.

A long time ago, in v2.4, VM_RESERVED kept swapout process off VMA,
currently it lost original meaning but still has some effects:

remap_pfn_range() and io_remap_pfn_range() set VM_IO|VM_DONTEXPAND|VM_DONTDUMP.
remap_vmalloc_range() set VM_DONTEXPAND | VM_DONTDUMP.

In LUN RESET testing involving NetApp targets, it is observed that LUN
RESET is failing. The fc_fcp_resp() is not completing the completion
for the LUN RESET task since fc_fcp_resp assumes that the FCP_RSP_INFO
is 8 bytes with the 4 byte reserved field, where in case of NetApp targets
the FCP_RSP to LUN RESET only has 4 bytes of FCP_RSP_INFO. This leads
fc_fcp_resp to error out w/o completing the task completion, eventually
causing LUN RESET to be escalated to host reset, which is not very nice.
Per FCP-3 r04, clause 9.5.15 and Table 23, the FCP_RSP_INFO field can be either
4 bytes or 8 bytes, with the last 4 bytes as "Reserved (if any)". Therefore it
is valid to have 4 bytes FCP_RSP_INFO like some of the NetApp targets behave.
Fixing this by validating the FCP_RSP_INFO against both the two spec allowed
length.

SCSI errors were generated while writing to LUNs
connected via NPIV ports.
Debugging this it was found that the FCoE packets
transmitted via the NPIV ports were not tagged with
correct user priority as negotiated with peer by DCB
agent. This resulted in FCoE traffic going with priority
zero(0) that did not have priority flow control (PFC)
enabled for it. The initiator after transferring data
to the target never saw any reply indicating the transfer
was complete. This resulted in error recovery (ABTS) and
SCSI command retries by the scsi-mid layer; eventually
resulting in I/O errors.
This patch fixes this issue by keeping the FCoE user
priority information in the fcoe_interface instance
that is common for both the physical port as well as
NPIV ports connected to that physical port; instead
of storing it in fcoe_port structure that has a per
port instance.

firmware image version (3.1.0.0).
- The new convention is <firmware-image>-<firmware-version>.bin
- The change will enforce loading only compatible firmware with this driver
and also avoid over-writing the old firmware image in-order to load new

- Made changes such that once the PWWN is acquired from the fabric through FAA,
and if the FAPWWN configuration is modified on the switch side, driver should
show relevant information to the user.
- Added logic to cache the reason code when the given port is disabled implicitl
due to FAA error condition.
- If the port is disabled, while sending SCN to upper layer, update the
reason code appropriately. With this, BFA FC port state machine will enter
into faa_err_config state. This state will be shown to the user.

- Introduced support for D-Port which is a new port mode during which
link level diagnostics can be run.
- Provided mechanism to dynamically configure D-Port and initiate diagnostic

1. No support for multiple initiators in the loop
2. No public loop support. If attached to a loop with an FL_Port,
device continues to work as a private NL_Port in the loop

The ACARD driver calls udelay() with a value > 2000, which leads to to
the following compilation error on ARM:
ERROR: "__bad_udelay" [drivers/scsi/atp870u.ko] undefined!

This is because udelay is defined on ARM, roughly speaking, as
#define udelay(n) ((n) > 2000 ? __bad_udelay() : \
__const_udelay((n) * ((2199023U*HZ)>>11)))
The argument to __const_udelay is the number of jiffies to wait divided
by 4, but this does not work unless the multiplication does not
overflow, and that is what the build error is designed to prevent. The
intended behavior can be achieved by using mdelay to call udelay
multiple times in a loop.

"This is workqueue updates for v3.7-rc1. A lot of activities this
round including considerable API and behavior cleanups.
* delayed_work combines a timer and a work item. The handling of the
timer part has always been a bit clunky leading to confusing
cancelation API with weird corner-case behaviors. delayed_work is
updated to use new IRQ safe timer and cancelation now works as
expected.
* Another deficiency of delayed_work was lack of the counterpart of
mod_timer() which led to cancel+queue combinations or open-coded
timer+work usages. mod_delayed_work[_on]() are added.
These two delayed_work changes make delayed_work provide interface
and behave like timer which is executed with process context.
* A work item could be executed concurrently on multiple CPUs, which
is rather unintuitive and made flush_work() behavior confusing and
half-broken under certain circumstances. This problem doesn't
exist for non-reentrant workqueues. While non-reentrancy check
isn't free, the overhead is incurred only when a work item bounces
across different CPUs and even in simulated pathological scenario
the overhead isn't too high.
All workqueues are made non-reentrant. This removes the
distinction between flush_[delayed_]work() and
flush_[delayed_]_work_sync(). The former is now as strong as the
latter and the specified work item is guaranteed to have finished
execution of any previous queueing on return.
* In addition to the various bug fixes, Lai redid and simplified CPU
hotplug handling significantly.
* Joonsoo introduced system_highpri_wq and used it during CPU
hotplug.
There are two merge commits - one to pull in IRQ safe timer from
tip/timers/core and the other to pull in CPU hotplug fixes from
wq/for-3.6-fixes as Lai's hotplug restructuring depended on them."

were silent ones where the deprecated interfaces had been used by new

The st_mutex was created when the BKL was removed, and
prevents simultaneous st_open calls. It is better to

We set the capacity to zero when we discovered a device formatted with
an unknown DIF protection type. However, the read_capacity code would
override the capacity and cause the device to be enabled regardless.
Make sd_read_protection_type() return an error if the protection type is
unknown. Also prevent duplicate printk lines when the device is being
revalidated.

Issue:
For ISP83XX risc_intr getting clear two times in case of polling mode.
risc_intr getting clear from interrupt_service_routine() as well

If IRQ allocation fails, it means system has run out of interrupt
vectors, so fail probe_adapter in case of IRQ allocation failure
and dont retry adapter initialization.

Camram is reset by FW to default values after reboot/cold boot/pci reset.
In case of AER, PCI may need to reset so we need to set correct idc reg value
after PCIE error.

A quote from SPC-4: "While in the unavailable primary target port
asymmetric access state, the device server shall support those of

Don't restart lookup of more stargets in the multi-target case, just
arrange to traverse the list once, on the assumption that new targets
are always added at the end. There is no guarantee that the target will
change state in scsi_target_reap() so we can end up spinning if we
restart.

The length field in the host config packet is only 16-bit long, so
passing it 0x10000 (64K which is our standard PAGE_SIZE) doesn't
work and result in an empty config from the server.

Now that the iSeries code is gone the backend abstraction
in this driver is no longer necessary, which allows us to
consolidate the driver in one file.
The side effect is that the module name is now ibmvscsi.ko
which matches the driver hotplug name and fixes auto-load
issues.

Device can go to READY state from COLD and skip INITIALIZATION,
In this case driver will never set IDC version from function
qla4_8xxx_device_bootstrap().
Fix:
1. Set IDC version at start of function qla4_82xx_need_reset_handler().

Add "removable" module parameter to set the "removable" attribute of any
subsequently created debug block device. It is a writable driver option, so
that you can switch between removable and "fixed" media block devices in
between the add_host calls.
This is useful for being able to test the different behaviour/required
privileges in e. g. the udisks test suite.

while processing the stop firmware mailbox command on driver unload,
a interrupt is processed which causes kernel panic as the response
queue is not valid.
Fix is to clear the interrupt in free_adapter call just after
disabling the interrupts.

We always assign a dummy task context to a port in order to address a
silicon issue. We have 4 ports per controller. So when idle, there are always
exactly 4 TCs "active". The adaptive interrupt coalescing code uses number of
active TCs to figure out the coalescing values. However, we never hit "0" TCs
because of the 4 dummy TCs. Putting in fix so that we calculate this correctly.

This commit fixes a driver bug for SSP tasks that require task management
in the target after they complete in the SCU hardware. The problem was
manifested in the function "isci_task_abort_task", which tests
to see if the sas_task.lldd_task is non-NULL before allowing task
management; this bug would always NULL lldd_task in the SCU I/O completion
path even if target management was required, which would prevent
task / target manangement from happening.
Note that in the case of SATA/STP targets, error recovery is provided by
the libata error handler which is why SATA/STP device recovery worked
correctly even though SSP handling did not.

gcc 4.8 warns because the memset only clears sizeof(char *) bytes, not
the whole buffer. Use the correct buffer size and clear the whole sense

It does not make sense to translate ref tags with unexpected values.
Instead we simply ignore them and let the upper layers catch the

This fixes an issues seen where a Fabric RSCN event was received
while the link was down, which resulted in repeated attempts to log back
into the fabric, which then failed, resulting in the ibmvfc driver
taking the host offline. Fix this by delaying taking any action
regarding the fabric RSCN until the link comes back up.

If an abort request times out to the virtual fibre channel adapter,
the ibmvfc driver will kick off a reset of the adapter. This
patch ensures we wait for the both the abort request and the
request being aborted to be completed prior to exiting the
eh_abort handler. This fixes a bug where the ibmvfc driver
was erroneously returning success to the eh_abort handler
then later sending back a response to the same command.

- Changes to avoid discovering NPIV port as remote port by the other
NPIV ports created on same physical port when all the NPIV ports are
part of the same zone in a fabric.
- Provided mechanism to support maximum number of target ports for a
given initiator port (physical port + NPIV ports) irrespective of the
way in which the initiator and target ports are zoned in the fabric.
- Introduced module_parameter max_rport_logins to restrict number of
remote ports discovery which includes target and initiator remote ports.

export remote ports with valid pid or valid pwwn to the user space.
- Introduced old_pid field in the rport structure and made changes to
prevent re-creating a new remote port for an already existing rport
that is transitioning to a delete state. (Happens if we receive a RSCN

Stale information in the temporary fcport created in
qla2x00_configure_local_loop() causes qla2x00_get_port_database() call
to fail. This reschedules scan, which gets stuck continuously in the
rescheduling-of-scan loop due to the failure.

The qla2xxx firmware actually expects the task management response
code in a CTIO IOCB with SCSI status mode 1 to be in little-endian
byte order, ie the response code should be the first byte in the
sense_data[] array. The old code erroneously byte-swapped the
response code, which puts it in the wrong place on the wire and leads
to initiators thinking every task management request succeeds (since
they see 0 in the byte where they look for the response code).

If a command status of CMD_PROTOCOL_ERR is received, this
information should be conveyed to the SCSI mid layer, not
dropped on the floor. CMD_PROTOCOL_ERR may be received
from the Smart Array for any commands destined for an external
RAID controller such as a P2000, or commands destined for tape
drives or CD/DVD-ROM drives, if for instance a cable is
disconnected. This mostly affects multipath configurations, as
disconnecting a cable on a non-multipath configuration is not
going to do anything good regardless of whether CMD_PROTOCOL_ERR
is handled correctly or not. Not handling CMD_PROTOCOL_ERR
correctly in a multipath configaration involving external RAID
controllers may cause data corruption, so this is quite a serious
bug. This bug should not normally cause a problem for direct
attached disk storage.

Every fabric driver has to supply a se_tfo->set_fabric_sense_len()
method, just so iSCSI can return an offset of 2. However, every fabric
driver is already allocating a sense buffer and passing it into the
target core, either via transport_init_se_cmd() or target_submit_cmd().
So instead of having iSCSI pass the start of its sense buffer into the
core and then later tell the core to skip the first 2 bytes, it seems
easier for iSCSI just to do the offset of 2 when it passes the sense
buffer into the core. Then we can drop the se_tfo->set_fabric_sense_len()
everywhere, and just add a couple of lines of code to iSCSI to set the
sense data length to the beginning of the buffer right before it sends
it over the network.

This patch fixes the following kernel panic invoked by uninitialized fields
in the chip initialization for the 1G bnx2 iSCSI offload.
One of the bits in the chip initialization is being used by the latest
firmware to control overflow packets. When this control bit gets enabled
erroneously, it would ultimately result in a bad packet placement which would
cause the bnx2 driver to dereference a NULL ptr in the placement handler.
This can happen under certain stress I/O environment under the Linux
iSCSI offload operation.
This change only affects Broadcom's 5709 chipset.

The device limit of 128 tape drives was established in 2003 as a
significant increase from the 8 tape drives allowed previously.
We're seeing customer sites that between a large number of drives
and multipath are discovering more than 128 devices and running
into problems.
Now that we're not stuck having to store a pointer in array
and aren't limited by kmalloc failing on higher order allocs we can
lift the limit to fill the entire minor range based on the number
of modes.
Based on the current code, that's 2^17 devices.

st currently allocates an array to store pointers to all of the
scsi_tape objects. It's used to discover available indexes to use as the
base for the minor number selection and later to look up scsi_tape
devices for character devices.
We switch to using an IDR for minor selection and a pointer from
st_modedef back to scsi_tape for the lookups.

st_probe leaves a cdev pointer hanging around that is compared
during the error path and freed later. There's no need for the pointer
to hang around at all. So we free it immediately and simplify the error
handling.

st currently sets up and tears down class attributes manually for
every tape drive in the system. This patch uses a statically defined

The MAC_ADDR stored in driver private structure is of
unsigned char data type but strlcpy parameters is of
signed char data type. This conversion of data types
lead to change in the value.This changed value is passed
to the upper layer and junk characters were displayed
when "iscsiadm -m iface" command was run.
In case of iSCSI boot, since the the MAC_ADDR was coming
junk the boot was also not working

When the driver comes up in crashdump mode, it has to explicitly
issue command to FW for logging to the boot target. This fix issues

FC and iSCSI class set SCSI devices to transport-offline state after
fast_io_fail/replacement_timeout has fired, but after relogin, function
scsi_internal_device_unblock() is not setting scsi device state to running.
Due to this the devices even after being relogged in remain offline.

In qla4xxx_ep_connect(), qla_ep->dst_addr and dst_addr are type
struct sockaddr. We are copying sizeof(struct sockaddr_in6) bytes
from dst_addr to qla_ep->dst_addr which is 12 bytes larger. This
will cause memory corruption. So we change qla_ep->dst_addr to
struct sockaddr_storage which is of 128 byte, large enough to
hold sizeof(struct sockaddr_in6).

When using the commands below to write some data to a virtio-scsi LUN of the
QEMU guest(32-bit) with 1G physical memory(qemu -m 1024), the qemu will crash.
# sudo mkfs.ext4 /dev/sdb (/dev/sdb is the virtio-scsi LUN.)
# sudo mount /dev/sdb /mnt
# dd if=/dev/zero of=/mnt/file bs=1M count=1024
In current implementation, sg_set_buf is called to add buffers to sg list which
is put into the virtqueue eventually. But if there are some HighMem pages in
table->sgl you can not get virtual address by sg_virt. So, sg_virt(sg_elem) may
return NULL value. This will cause QEMU exit when virtqueue_map_sg is called
in QEMU because an invalid GPA is passed by virtqueue.

Finally, value assignment approach was adopted because:
Value assignment creates a well-formed scatterlist, because the termination
marker in source sg_list has been set in blk_rq_map_sg(). The last entry of the
source sg_list is just copied to the the last entry in destination list. Note
that, for now, virtio_ring does not care about the form of the scatterlist and
simply processes the first out_num + in_num consecutive elements of the sg[]
array.
I have tested the patch on my workstation. QEMU would not crash any more.

It turns out Smart Array logical drives do not support target
reset and when the target reset fails, the logical drive will
be taken off line. Symptoms look like this:

LUN reset is supported though, and is what we should be using.
Target reset is also disruptive in shared SAS situations,
for example, an external MSA1210m which does support target
reset attached to Smart Arrays in multiple hosts -- a target
reset from one host is disruptive to other hosts as all LUNs
on the target will be reset and will abort all outstanding i/os
back to all the attached hosts. So we should use LUN reset,
not target reset.
Tested this with Smart Array logical drives and with tape drives.
Not sure how this bug survived since 2009, except it must be very
rare for a Smart Array to require more than 30s to complete a request.

The scsi netlink code confuses the netlink port id with a process id,
going so far as to read NETLINK_CREDS(skb)->pid instead of the correct
NETLINK_CB(skb).pid. Fortunately it does not matter because nothing
registers to respond to scsi netlink requests.
The only interesting use of the scsi_netlink interface is
fc_host_post_vendor_event which sends a netlink multicast message.
Since nothing registers to handle scsi netlink messages kill all of the
registration logic, while retaining the same error handling behavior
preserving the userspace visible behavior and removing all of the
confused code that thought a netlink port id was a process id.
This was tested with a kernel allyesconfig build which had no problems.

It is a frequent mistake to confuse the netlink port identifier with a
process identifier. Try to reduce this confusion by renaming fields
that hold port identifiers portid instead of pid.
I have carefully avoided changing the structures exported to
userspace to avoid changing the userspace API.
I have successfully built an allyesconfig kernel with this change.

on PVR_foo for our PVR #defines. Currently we use PVR_ on 32-bit and PV_
on 64-bit.
So do that rename and remove the FIXME.
Seeing as we're touching all but one usage of __is_processor(), rename it
to something less ugly and more indicative of what it does, which is
simply to check the PVR version.

When device discovery is disabled during driver load time using module
parameter "disable_discovery=1" and when diag reset is issued then from logs,
it is observed that the devices get added, removed and then added with new
target ids.
So, in order to limit this turn-off the code which is deleting and devices
across host reset when the disable_discovery module parameter is turned on.

A new sysfs shost attribute called "BMR_status" is implemented to
report Backup Rail Monitor status.
This attribute is located in the path
/sys/class/scsi_host/host#/BMR_status
when reading this adapter attribute, then driver will output the state
of GPIO[24]. It returns "0" if BMR is healthy and it returns "1" for failure.
if it returns an empty string then it means that there was an error while
obtaining the BMR status. Then check dmesg for what error has occured.

Provide a "simple-dev-pm-ops" implementation that shuts down the domain
and the device on suspend, and resumes the device and the domain on
resume. All of the mechanics of restoring domain connectivity are
handled by libsas once isci has notified libsas that all links should be
back up. libsas is in charge of handling links that did not resume, or
resumed out of order.

libsas power management routines to suspend and recover the sas domain
based on a model where the lldd is allowed and expected to be
"forgetful".
sas_suspend_ha - disable event processing allowing the lldd to take down
links without concern for causing hotplug events.
Regardless of whether the lldd actually posts link down
messages libsas notifies the lldd that all
domain_devices are gone.
sas_prep_resume_ha - on the way back up before the lldd starts link
training clean out any spurious events that were
generated on the way down, and re-enable event
processing
sas_resume_ha - after the lldd has started and decided that all phys
have posted link-up events this routine is called to let
libsas start it's own timeout of any phys that did not
resume. After the timeout an lldd can cancel the
phy teardown by posting a link-up event.
Storage for ex_change_count (u16) and phy_change_count (u8) are changed
to int so they can be set to -1 to indicate 'invalidated'.

This is a particularly nasty SCSI ATA Translation Layer (SATL) problem.
SAT-2 says (section 8.12.2)
if the device is in the stopped state as the result of
processing a START STOP UNIT command (see 9.11), then the SATL
shall terminate the TEST UNIT READY command with CHECK CONDITION
status with the sense key set to NOT READY and the additional
sense code of LOGICAL UNIT NOT READY, INITIALIZING COMMAND
REQUIRED;
mpt2sas internal SATL seems to implement this. The result is very confusing
standby behaviour (using hdparm -y). If you suspend a drive and then send
another command, usually it wakes up. However, if the next command is a TEST
UNIT READY, the SATL sees that the drive is suspended and proceeds to follow
the SATL rules for this, returning NOT READY to all subsequent commands. This
means that the ordering of TEST UNIT READY is crucial: if you send TUR and
then a command, you get a NOT READY to both back. If you send a command and
then a TUR, you get GOOD status because the preceeding command woke the drive.

Changed our ordering on TEST UNIT READY commands meaning that SATA drives
connected to an mpt2sas now suspend and refuse to wake (because the mpt2sas
SATL sees the suspend *before* the drives get awoken by the next ATA command)
resulting in lots of failed commands.
The standard is completely nuts forcing this inconsistent behaviour, but we
have to work around it.
The fix for this is twofold:
1. Set the allow_restart flag so we wake the drive when we see it has been
suspended
2. Return all TEST UNIT READY status directly to the mid layer without any
further error handling which prevents us causing error handling which
may offline the device just because of a media check TUR.

megasas_probe_one() to megasas_init(). This prevents a crash when a user
loads the driver and tries to issue a poll() system call on the ioctl

If the specified max_queue_depth setting is less than the
expected number of internal commands, then driver will calculate
the queue depth size to a negitive number. This negitive number
is actually a very large number because variable is unsigned
16bit integer. So, the driver will ask for a very large amount of
memory for message frames and resulting into oops as memory
allocation routines will not able to handle such a large request.
So, in order to limit this kind of oops, The driver need to set
the max_queue_depth to a scsi mid layer's can_queue value. Then
the overall message frames required for IO is minimum of either
(max_queue_depth plus internal commands) or the IOC global
credits.

flush[_delayed]_work_sync() are now spurious. Mark them deprecated
and convert all users to flush[_delayed]_work().
If you're cc'd and wondering what's going on: Now all workqueues are
non-reentrant and the regular flushes guarantee that the work item is
not pending or running on any CPU on return, so there's no reason to
use the sync flushes at all and they're going away.
This patch doesn't make any functional difference.

Pull networking changes from David S Miller:
1) Remove the ipv4 routing cache. Now lookups go directly into the FIB
trie and use prebuilt routes cached there.
No more garbage collection, no more rDOS attacks on the routing
cache. Instead we now get predictable and consistent performance,
no matter what the pattern of traffic we service.
This has been almost 2 years in the making. Special thanks to
Julian Anastasov, Eric Dumazet, Steffen Klassert, and others who
have helped along the way.
I'm sure that with a change of this magnitude there will be some
kind of fallout, but such things ought the be simple to fix at this
point. Luckily I'm not European so I'll be around all of August to
fix things :-)

Commit 43a8d39d fixed the fact that wait_for_device_probe() was unable
to flush sd probe work. Now that sd probe work is once again flushable

In response to an async related regression James noted:
"My theory is that this is an init problem: The assumption in a lot of
our code is that async_synchronize_full() waits for everything ... even
the domain specific async schedules, which isn't true."
...so make this assumption true.
Each domain, including the default one, registers itself on a global domain
list when work is scheduled. Once all entries complete it exits that
list. Waiting for the list to be empty syncs all in-flight work across
all domains.
Domains can opt-out of global syncing if they are declared as exclusive
ASYNC_DOMAIN_EXCLUSIVE(). All stack-based domains have been declared
exclusive since the domain may go out of scope as soon as the last work
item completes.
Statically declared domains are mostly ok, but async_unregister_domain()
is there to close any theoretical races with pending
async_synchronize_full waiters at module removal time.

This is in preparation for teaching async_synchronize_full() to sync all
pending async work, and not just on the async_running domain. This
conversion is functionally equivalent, just embedding the existing list
in a new async_domain type.
The .registered attribute is used in a later patch to distinguish
between domains that want to be flushed by async_synchronize_full()
versus those that only expect async_synchronize_{full|cookie}_domain to
be used for flushing.

This patch changes virtio-scsi to use a new virtio_driver->scan() callback
so that scsi_scan_host() can be properly invoked once virtio_dev_probe() has
set add_status(dev, VIRTIO_CONFIG_S_DRIVER_OK) to signal active virtio-ring
operation, instead of from within virtscsi_probe().
This fixes a bug where SCSI LUN scanning for both virtio-scsi-raw and
virtio-scsi/tcm_vhost setups was happening before VIRTIO_CONFIG_S_DRIVER_OK
had been set, causing VIRTIO_SCSI_S_BAD_TARGET to occur. This fixes a bug
with virtio-scsi/tcm_vhost where LUN scan was not detecting LUNs.
Tested with virtio-scsi-raw + virtio-scsi/tcm_vhost w/ IBLOCK on 3.5-rc2 code.

The PCIE capability offset is saved during PCI bus walking. It will
remove an unnecessary search in the PCI configuration space if this
value is referenced instead of reacquiring it. Also, pci_is_pcie is a
better way of determining if the device is PCIE or not (as it uses the
same saved PCIE capability offset).

This patch implements the hotplug support for virtio-scsi.
When there is a device attached/detached, the virtio-scsi driver will be

We do not need the sglist after calling virtqueue_add_buf. Hence we
can "pipeline" the locked operations and start preparing the sglist
for the next request while we kick the virtqueue.
Together with the previous two patches, this improves performance as
follows. For a simple "if=/dev/sda of=/dev/null bs=128M iflag=direct"
(the source being a 10G disk, residing entirely in the host buffer cache),
the additional locking does not cause any penalty with only one dd
process, but 2 simultaneous I/O operations improve their times by 3%:

important now, it prepares the code for when we will add support

we introduce a separate lock for the sglist.

This is exposed in the case the FCP_DATA frames somehow got lost and fc_fcp got
the FCP_RSP, in fc_fcp_recv_resp(), since xfer_len is less than the expected_len
it resets the the timer to wait to 2 more jiffies in case the data frames are
already queued locally. However, for target does not support REC, it would just
send RJT w/ ELS_RJT_UNSUP. The rec response handler thus only clears the rport
flag for not doing REC later, but does not do fcp_io_complete() on the
associated fsp.
The fix is just check status of FCP_RSP being received already, i.e. using the
FC_SRB_RCV_STATUS flag, in fc_fcp_timeout before start sending REC. We should
have waited long enough if there is truely data frames queued locally.

The FC-GS-3 sepc requires to wait for least 3 times R_A_TOV per
sec 4.6.1 "If the Requesting_CT does not receive a Response
CT_IU from the Responding_CT within three times R_A_TOV,
it shall consider this to be a protocol error."
This means added four new states with management server
could add significant delay with multiple retries
on default 12 second timeout(3 * R_A_TOV), so instead
just skip these states on very first timeout on any of
these states to not stuck with states for such longer
period.

The lport_recv(), i.e., fc_lport_recv_req() may get called w/o the sequence ptr
being set in fr_seq(), particularly in the case of vn2vn mode, this may happen
if the passive fcp provider, e.g., tcm_fc, has not been registered yet.

Noticed that we can shuffle the code around in fcoe_percpu_receive_thread a bit
and avoid taking the fcoe_rx_list lock twice per iteration. This should improve
throughput somewhat. With this change we take the lock, and check for new
frames in a single critical section. Only if the list is empty do we drop the
lock and re-acquire it after being signaled to wake up.

v2) did some further cleanup on the patch by replacing the 2nd call of
spin_lock/splice_init with a goto to the top of the outer loop. This allows me
to change the inner while loop to an if conditional and remove the sencond check

strtoul returns an 'unsigned long' so there is no
reason to check if the value is less than zero.
strtoul already checks for the '-' character deep
in its bowels. It will return an error if the user
has provided a negative value and fcoe_str_to_dev_loss
will return that error to its caller.
This patch fixes the following Coverity reported warning:
CID 703581 - NO_EFFECT Unsigned compared against 0 - This
less-than-zero comparison of an unsigned value is never true. "*val < 0UL".

There is a race in scsi_bus_resume_common when set device's runtime
state to active after pm_runtime_put_sync(dev->parent).
Parent device may have been suspended so pm_runtime_set_active(dev) will

Commit d38bd3aef ("Add -Werror compilation flag") is causing build breakage
with random gcc incarnations. These look like gcc problems, but we shouldn't
break the build because of a bad gcc. Fix this by adding a make flag
WARNINGS_BECOME_ERRORS=1
which is the same as aic7xxx uses so ordinarily the build doesn't use -Werror

On the way to add a new sata_device field, noticed that libsas is
carrying port multiplier infrastructure that is explicitly disabled by
sas_discover_sata(). The aic94xx touches the unused port_no, so leave
that field in case there was some use for it.

The above commits seem to have confused the return value of
sas_ex_discover_dev which is non-zero on failure and
sas_ex_join_wide_port which just indicates short circuiting discovery on
already established ports. The result is random discovery failures
depending on configuration.
Calls to sas_ex_join_wide_port are the source of the trouble as its
return value is errantly assigned to 'res'. Convert it to bool and stop
returning its result up the stack.

sas_eh_bus_reset_handler() amounts to sas_phy_reset() without
notification of the reset to the lldd. If this is triggered from
eh-cmnd recovery there may be sas_tasks for the lldd to terminate, so
->lldd_I_T_nexus_reset is warranted.

The strategy handlers may be called in places that are problematic for
libsas (i.e. sata resets outside of domain revalidation filtering /
libata link recovery), or problematic for userspace (non-blocking ioctl
to sleeping reset functions). However, these routines are also called
for eh escalations and recovery of scsi_eh_prep_cmnd(), so permit them
as long as we are running in the host's error handler, otherwise arrange
for them to be triggered in eh_context.

A quick reading of scsi_error_handler() one could come away with the
impression that it does its wakeup event check while the task state is
TASK_RUNNING. In fact it sets TASK_INTERRUPTIBLE at the bottom of the
loop, but that is ~50 lines down.
Just set TASK_INTERRUPTIBLE at the top of loop and be done.

eh is woken up automatically by the presence of failed commands,
scsi_schedule_eh is reserved for cases where there are no failed
commands. This guarantees that host_eh_sceduled is only incremented

Rapid ata hotplug on a libsas controller results in cases where libsas
is waiting indefinitely on eh to perform an ata probe.
A race exists between scsi_schedule_eh() and scsi_restart_operations()
in the case when scsi_restart_operations() issues i/o to other devices
in the sas domain. When this happens the host state transitions from
SHOST_RECOVERY (set by scsi_schedule_eh) back to SHOST_RUNNING and
->host_busy is non-zero so we put the eh thread to sleep even though
->host_eh_scheduled is active.
Before putting the error handler to sleep we need to check if the
host_state needs to return to SHOST_RECOVERY for another trip through
eh. Since i/o that is released by scsi_restart_operations has been
blocked for at least one eh cycle, this implementation allows those
i/o's to run before another eh cycle starts to discourage hung task
timeouts.

When managing shost->host_eh_scheduled libata assumes that there is a
1:1 shost-to-ata_port relationship. libsas creates a 1:N relationship
so it needs to manage host_eh_scheduled cumulatively at the host level.
The sched_eh and end_eh port port ops allow libsas to track when domain
devices enter/leave the "eh-pending" state under ha->lock (previously
named ha->state_lock, but it is no longer just a lock for ha->state
changes).
Since host_eh_scheduled indicates eh without backing commands pinning
the device it can be deallocated at any time. Move the taking of the
domain_device reference under the port_lock to guarantee that the
ata_port stays around for the duration of eh.

The loop that waited for syncronous fib commands was causing a CPU stall
when a timeout actually occured.
1) Switch to using a more accurate timeout mechanism.
2) Do not pace the loop with udelay(). Use cpu_relax() to allow for
scheduling to occur.

This also stops using the "legacy crap" in Scsi_Host (shost->base is an
unsigned long).
This affected 32-bit systems that have 64-bit resource sizes, causing the
IO address to be truncated.

Introduce scsi_dh_attached_handler_name() to retrieve the name of the
scsi_dh that is attached to the scsi_device associated with the provided
request queue. Returns NULL if a scsi_dh is not attached.
Also, fix scsi_dh_{attach,detach} function header comments to document
@q rather than @sdev.

Avoid that the code for requeueing SCSI requests triggers a
crash by making sure that that code isn't scheduled anymore
after a device has been removed.
Also, source code inspection of __scsi_remove_device() revealed
a race condition in this function: no new SCSI requests must be
accepted for a SCSI device after device removal started.

When we call scsi_unprep_request() the command associated with the request
gets destroyed and therefore drops its reference on the device. If this was
the only reference, the device may get released and we end up with a NULL
pointer deref when we call blk_requeue_request.

Use blk_queue_dead() to test whether the queue is dead instead
of !sdev. Since scsi_prep_fn() may be invoked concurrently with
__scsi_remove_device(), keep the queuedata (sdev) pointer in
__scsi_remove_device(). This patch fixes a kernel oops that
can be triggered by USB device removal. See also

Other changes included in this patch:
- Swap the blk_cleanup_queue() and kfree() calls in
scsi_host_dev_release() to make that code easier to grasp.
- Remove the queue dead check from scsi_run_queue() since the
queue state can change anyway at any point in that function
where the queue lock is not held.
- Remove the queue dead check from the start of scsi_request_fn()
since it is redundant with the scsi_device_online() check.

On 64 bit systems the current code sets 32 bits of "seg" and leaves the
other 32 uninitialized. It doesn't matter since the variable is never
used. But it's still messy and we should fix it.

If bfad_thread_workq(bfad) was not BFA_STATUS_OK then we freed "im"
and then dereferenced it.
I did a little clean up because it seemed nicer to return directly
instead of doing a superfluous goto. I looked at other functions in
this file and it seems like returning directly is standard.

A quote from SPC-4: "While in the unavailable primary target port
asymmetric access state, the device server shall support those of
the following commands that it supports while in the active/optimized
state: [ ... ] d) SET TARGET PORT GROUPS; [ ... ]". Hence re-enable
sending STPG to a target port group that is in the unavailable state.

Currently the backoff algorithm for when to retry alua rtpg
requests progresses geometrically as so:
2, 4, 8, 16, 32, 64... seconds.
This progression can lead to un-needed delay in retrying
alua rtpg requests when the rtpgs are delayed. A less
aggressive backoff algorithm that is additive would not
lead to such large jumps when delays start getting long, but
would backoff linearly:
2, 4, 6, 8, 10... seconds.

Some storage arrays are known to return 'illegal request'
when an rtpg extended header request is made. T10 says the
array should ignore the bit, and return the non-extended
rtpg as the array doesn't support the request. Working
around this by retrying the rtpg request without the extended
header bit set when the extended rtpg request results in
illegal request.

During alua transitions, an array can return transitioning
status in response to rtpg requests. These requests get
retried for a maximum of 60 seconds by default before timing
out. Sometimes this timeout isn't sufficient to allow the
array to complete the transition. T10-spc4 addresses this
under 'Report Target Port Groups' command.
This update retrieves the timeout value from the storage
array if available and retries the transitioning rtpgs
for up to the 'implied transitioning timeout' value

ARCMSR_ARC1880_DiagWrite_ENABLE is 0x00000080 so (x | 0x00000080) is
never zero. The intent here was to test that loop until
ARCMSR_ARC1880_DiagWrite_ENABLE was turned on, but because the test was
wrong, we would do five loops regardless of whether it succeed or not.
Also I simplified the condition a little by removing the unused
assignement.
